The Flow for the Day: Step 1- Intake: Includes Registration and

Guest/Volunteer Match Volunteers are asked to remain with

their guest(s) for the duration of their visit.

The guest intake packet will include: questionnaire, photo release, passport, name tag, and client authorization form.

Meal tickets for both the guest(s) and the volunteer will be distributed at the match point

The Flow for the Day:Step 2 – The Services: Complete Intake Questionnaire and Establish

service priorities for the day. Service priorities are Veteran’s Affairs, Housing,

Employment, and Medical. If they are a Veteran, encourage them to begin at

the Veterans Area. When the guest arrives in an area, they will start

at the Triage table for that area. *** Please make sure that each provider you

visit documents all comments and actions for your Guest in your Guest’s passport and on their questionnaire. ***

The Flow for the Day:Step 3 – Discharge / Exit Interview: (Quality Control) Submit all Guest forms (used for data analysis later).

Looking at the passport, the Discharge team member helps the guest make arrangements to get to any follow-up appointments and connects them to service agencies that may be appropriate for their needs.

**Please explain to your guest the importance of their passport to services and that it needs to be presented at every follow-up appointment.**

Step 4 – Gift Bags: Each guest is given a “gift bag” that contains some

basic, necessary hygiene products like toothbrushes and toothpaste, and various other items.

REMINDERS Wear comfortable shoes! Dress appropriately for

the weather. PHC is an outdoor event. Rain or shine. Bring water! It is not too late to recruit a friend. Safety

To address issues of safety and behavioral health crises, PHC has a HELP ZONE crisis resolution team, as well as police personnel on site.

Logistics The Project Homeless Connect Services will be

primarily on the 2nd floor Main Concourse. There are additional services on the third floor, and some located on Blackwell Street and at the YMCA. Please review the service list and site map for more information.

Guest Interaction:

Treat guests with dignity Talk with them, not at them. Respect guest privacy When handling paperwork, do not

review it unless necessary Ask for help, if you need it. Do NOT promise services. Do

not say, “You will get …”
