ADDENDUM NO. 4 The Architect/ Engineer issues this addendum, applicable to the above named project, to all known Contractors before receipt of proposal. This addendum includes Item Number 4-1 thru 4-39. This addendum item shall be fully incorporated into the Bidding/Contract Documents and have the same force and effect as though originally included. Aquatherm polypropylene piping is an approved material substitution for the heat pump and hydronic piping. If polypropylene piping is used it will have to be fire wrapped in accordance with ASTM E84, NFPA 255 and UL 723. See enclosed material specification.

The undersigned, having examined the plans, project manuals and related documents, and the site of the

proposed work, and being familiar with all of the conditions surrounding the construction of the proposed

project, including the availability of labor, hereby proposes to furnish all labor, materials and supplies to do

the work in accordance with the Contract Documents and terms and condition described below:

Recognizing that time will be of the essence, the undersigned proposes, upon execution of the Agreement

Between Owner and Contractor or upon receipt of notice to proceed from the Owner (whichever comes

first), to immediately start the Work of the Contract. The Agreement Between Owner and Contractor will

be executed as soon as possible after the opening and award of bids. The undersigned proposes to bring

the work to a state of Substantial Completion on or before June 29, 2018.

Markup percentage for all changes in the work shall not exceed 10%. This shall include all overhead and

profit associated with each change. See Section 01 26 00, “Contract Modification Procedures”, for

administrative and procedural requirements for handling and processing contract modification. See

Section 01 21 00, “Allowances” for exceptions to this requirement.

Execution of Agreement: The undersigned will, within fourteen (14) days of receipt of notice of acceptance

of this proposal, enter into agreement with the Owner on the Owner’s agreement form and will deliver the

required Performance Bond and Labor and Materials Payment Bond.

A. Pre-installation Conference: Before installing, conduct conference at project site. Notify Architect and other participants at least 5 working days before conference.

1.5 DELIVERY, STORAGE AND HANDLING

A. Packaged Materials: Deliver packaged materials in containers. Protect materials from deterioration during delivery and while stored at site.

B. Bulk Materials:

1. Do not dump or store bulk materials near structures, utilities, walkways and pavements including pervious concrete unit pavement, or on existing turf areas or plants.

2. Provide erosion-control measures to prevent erosion or displacement of bulk materials, discharge or soil-bearing water runoff, and airborne dust reaching adjacent properties, water conveyance systems, or walkways.

1.6 PROJECT CONDITIONS

A. Coordinate bioretention locations, installation, and sequencing with other work to avoid interference and to assure proper placement.

1.7 SPECIAL PROJECT WARRANTY

A. The contractor shall provide replacement soil for all bioretention areas which may wash away or settle during the first 2 years following the completion and acceptance of bioretention construction.

2. PRODUCTS

2.1 PRODUCTS

A. Soil Mix:

1. Planting mix of Fine Sand and Compost mixed to the following proportion. Material %by volume Compost 30% Topsoil 30% Fine Sand 70%

2. Planting mix shall be thoroughly mixed 3. Final mix shall have a pH of between 5.5 and 8.0 4. Final mix shall have an organic matter content (tested by ignition) of between 3 and 5%.

Sample of final mix test for organic matter shall be screen through a ¼” mesh rather then the standard 2mm screen.

5. Final mix shall contain no other material or substances that may be harmful to plant growth, or prove a hindrance to the planting or maintenance operations.

6. Provide a two gallon sample of each mix ratio with the required soil test results. 7. Clay Content: less than 10%

B. Compost:

1. Shall be mature, weed free, and produced by aerobic decomposition of organic matter. Compost feedstock shall be yard waste trimmings and/or source-separated municipal solid waste to produce a fugally dominated compost. The product must not carry and visible refuse or other physical contaminants, substances toxic to plants, or over 5% sand, silt, clay or rock material by dry weight. The product shall possess no objectionable odors. The product must meet all applicable USEPA CRF, Title 40, Part 503 Standards for Class A biosolids. The moisture level shall be such that no visible water or dust is produced when handling the material.

2. Compost shall be dark brown in color, approximately the color of dark chocolate candy (70% chocolate). Black compost and compost the color of milk chocolate shall be rejected.

3. Compost shall have a strong aerobic (sweet) odor. Compost lacking a strong aerobic odor or which has an anaerobic (sour) odor shall be rejected.

4. Testing: The results of Compost analysis shall be provided by the Compost supplier. Before delivery of the Compost, the supplier must provide the following documentation:

a. Feedstock percentage in the final Compost product b. A statement that the Compost meets federal and state health and safety

regulations

C. PLANTING BED TOPSOIL

1. New topsoil that is fertile, friable, natural loam, dark in color, free of subsoil, clay lumps, brush, weeds, and other debris; and free of roots, stumps, stones larger than one inch in any dimension; and free of other extraneous or toxic matter harmful to plant growth. Topsoil should be obtained from local sources. It should have an acidity range (pH) of 5.5 - 7.5, and an organic matter content between 2 and 8 percent. Loam topsoil must have 7 to 20 percent clay, 28 to 60 percent silt, and less than 52 percent sand particles. Not more than 10 percent of topsoil weight can be gravel or stones.

3. EXECUTION

3.1 EXAMINATION

A. Examine areas and conditions for compliance with requirements for earthwork, pavement work, erosion control, seeding and other conditions affecting performance of work.

1. Do not begin installation before final grading is completed unless otherwise permitted by Landscape Architect

B. Proceed with installation only after unsatisfactory conditions have been removed.

3.2 PREPARATION

A. Outline and stake location of bioretention area for Landscape Architect approval.

3.3 INSTALLATION

A. Rip bottom and sides of the Bioretention cells to a minimum depth of 18” prior to installation of soils.

B. Install bioretention locations indicated and noted on the drawings

C. Installation of soils must be done in a manner that will ensure adequate Infiltration

D. Installed in 12” to 18” lifts and lightly compacted. Minimal compaction effort can be applied to the soil by tamping with a bucket from a dozer or backhoe

E. Lifts are not compacted but performed to prevent possibility of excessive settlement

F. No additional manual compaction of soil is necessary. Rake soil material as needed to level out. Overfill above the proposed surface invert to accommodate natural settlement to proper grade. Depending upon the soil material, up to 20% natural compaction may occur

G. Sieve analysis, pH, porosity, permeability and organic matter tests must be performed per each bioretention area.

H. It is very important to minimize compaction of both the base of the bioretention area and the required backfill. When possible, use excavation hoes to remove original soil. If bioretention areas are excavated using a loader, equipment with narrow tracks or narrow tires, rubber tires with large lugs, or high pressure tires will cause excessive compaction resulting in reducing infiltration rates and storage volumes and is not acceptable. Compaction will significantly contribute to design failure. Compaction can be alleviated at the base of the bio retention facility by using a primary tilling operation such as a Chisel Plow, Ripper, or Subsoiler. These tilling operations are to refracture the soil profile through the 12 inch compaction zone. Substitute methods must be approved by the landscape architect. Rototillers typically do not till deep enough to reduce the effects of compaction from heavy equipment.

I. When back filling the bioretention facility, do not use heavy equipment within the bioretention basin. Heavy equipment can be used around the perimeter of the basin to supply soils and sand. Grade bioretention materials with light equipment such as a compact loader or a dozer/ loader with marsh tracks.

J. In order to speed up the natural compaction process, presoaking the placed soil may be performed. Significant settlement can occur after the first presoak, and additional settlement may occur subsequent to the initial wetting. If time and construction scheduling permits, it is preferable to allow natural settlement to occur with the help of rain events to presoak the soil medium.

3.4 MAINTENANCE

A. Begin maintenance immediately after installation.

B. Protect Bioretention Soil from sedimentation from adjacent soils with silt fence and check for signs of erosion after rainstorms. Repair gullies or other signs of erosion.

C. Replenish mulch when needed.

D. Remove accumulated sediment; replace top inch of soil when clogged.

E. Check overflow pipe and remedy clogging as needed.

3.5 CLEANUP AND PROTECTION

A. During bioretention work, keep pavements clean and work areas in an orderly condition.

3.4 PIPING INSTALLATIONS 24

A. Install hangers and supports at intervals specified in the applicable Plumbing or 25

Mechanical Code and/or as recommended by pipe manufacturer. 26

B. Support vertical piping at each floor and as specified in the applicable 28

Plumbing or Mechanical Code. Piping 2” (63mm) or smaller shall be installed 29

with mid-story guides. 30

C. Fire stopping shall be provided to both be compatible with the Aquatherm 32

Piping and meet the requirements of ASTM E 814 or ULC S115 , “Fire Tests of 33

Through-Penetration Firestops”. Pipe insulations or fire resistive coating shall 34

be removed where the pipe passes through a fire stop and, if required by the 35

firestop manufacturer, for 3 inches beyond the firestop outside of the fire 36

barrier. 37

D. When installed in systems with pumps in excess of 7.5 HP, piping shall be 39

protected from excessive heat generated by operating the pump at shut-off 40

conditions. Where the possibility exists that the pump will operate with no 41

flow, the protection method shall be a temperature relief valve or comparable 42

level of protection, set to a maximum temperature of 185°F. 43

E. If heat tracing or freeze protection is specified for the piping, it should be 1

installed on the pipe interior or exterior. It must be suitable for use with plastic 2

piping and be self-regulating to ensure that the surface temperature of the pipe 3

and fittings will not exceed 70°C (158°F). 4

F. While still accessible all piping shall be pressure/leak tested to the 6

manufacturer’s standards. Tests shall be carried out using water, compressed air 7

or a mixture of the two. The test pressure shall be 1.5 times the operating 8

pressure or 150 psi, whichever is greater. Any leaks detected shall be repaired 9

at the contractor’s expense by removing the leaking part and replacing with new 10

parts welded per the pipe manufacturer’s guidelines. See www.aquatherm.com 11

3. ALL SANITARY SEWER WILL BE DONE UNDER A PLUMBERS PERMIT, AND SHALL COMPLY WITH ALLAPPLICABLE BUILDING CODES.

4. PRIOR TO FINAL ACCEPTANCE ALL SEWER MAIN SHALL BE AIR TESTED BY THE CONTRACTOR @ 5 LBSFOR 15 MINUTES, IN ACCORDANCE WITH THE LINCOLN PLUMBING CODE.

IF NOT ALREADY SO, THE CONTRACTOR SHALL CONSTRUCT SANITARY SEWER WITH ONE20 FOOT NOMINAL LENGTH OF PRESSURE PIPE MATERIAL CENTERED ON THE WATER MAIN AS PROVIDED INSECTION 22.02(B), SUCH THAT THE MAXIMUM POSSIBLE SEPARATION BETWEEN THE WATER MAIN AND THESEWER PIPE JOINTS WILL RESULT. THE BACKFILL MATERIAL SHALL BE SELECT, LOW PERMEABILITY SOIL.

1. THE CONTRACTOR SHALL MAINTAIN STRICT LATERAL CLEARANCE AS SHOWN ON THE PLANSFOR ALL UTILITY LINES.

2. IN ANY OF THE MAIN OR SERVICE ROUTES ARE ADJUSTED IN THE FIELD BY THE CONTRACTOR,THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROVIDING TO THE ENGINEER A RECORD OFTHE EXACT LOCATIONS, AT THE CONTRACTOR'S EXPENSE, A LICENSED SURVEYOR SHALL BERETAINED TO LOCATE THE CONDUIT ROUTES. THE CONTRACTOR SHALL PROVIDE THESURVEYED LOCATIONS TO THE ENGINEER IN ELECTRONIC FORMAT.

3. ALL TRENCHES SHALL BE BACKFILLED AND COMPACTED.

4. THE CONTRACTOR SHALL RESTORE ANY DISTURBED AREA TO ITS PREVIOUS CONDITION.

5. THE CONTRACTOR SHALL VERIFY ALL VERTICAL AND HORIZONTAL CROSSINGS OF ALLPROPOSED AND EXISTING UTILITIES PRIOR TO INSTALLATION OF CONDUIT. CONTACT THEENGINEER WITH ANY CONFLICTS.

6. ALL CONDUIT STUBS SHALL BE CAPPED AND MARKED ABOVE GROUND WITH REBAR AND A FLAG.

GENERAL NOTES

1. SANITARY SEWERS SHALL BE SEPARATED BY AT LEAST 10 FT. HORIZONTALLY FROM ANY EXISTING ORPROPOSED PARALLEL WATER MAINS, MEASURED EDGE TO EDGE.

2. AT ALL WATER MAIN CROSSINGS, SANITARY SEWERS SHALL BE LAID AT SUCH AN ELEVATION THAT THETOP OF THE SANITARY SEWER IS AT LEAST 18 INCHES BELOW THE BOTTOM OF THE WATER MAIN. INTHOSE INSTANCES WHERE THE BOTTOM OF THE WATER MAIN IS LESS THAN 18 INCHES, ABOVE THE TOPOF THE SANITARY SEWER OR THE SANITARY SEWER IS LOCATED ABOVE THE WATER MAIN, THESANITARY SEWER SHALL BE CONSTRUCTED USING A 20 FOOT LENGTH OF PVC PRESSURE PIPE, MEETINGTHE REQUIREMENTS OF AWWA C900 FOR DR18, PRESSURE RATING 150 PSI, CENTERED ON THE WATERMAIN.

3. ALL SANITARY SEWER WILL BE DONE UNDER A PLUMBERS PERMIT, AND SHALL COMPLY WITH ALLAPPLICABLE BUILDING CODES.

4. PRIOR TO FINAL ACCEPTANCE ALL SEWER MAIN SHALL BE AIR TESTED BY THE CONTRACTOR @ 5 LBSFOR 15 MINUTES, IN ACCORDANCE WITH THE LINCOLN PLUMBING CODE.

5. PRIOR TO FINAL ACCEPTANCE, THE CONTRACTOR SHALL HAVE THE SEWER MAIN TV INSPECTED AND THECOST OF SUCH WORK SHALL BE SUBSIDIARY TO THE PRICE BID FOR SEWER WITH INSTALLATION.

6. REFER TO PLUMBING PLANS FOR SANITARY SEWER TIE-INS WITH BUILDING.

7. ALL PIPE, SERVICE PIPE, AND FITTINGS SHALL BE SCHEDULE 40 PVC.

SANITARY SEWER SERVICE NOTES

1. PRIOR TO FINAL ACCEPTANCE ALL WATER MAIN PIPE SHALL BE PRESSURE TESTED BY THE CONTRACTOR.

2. PRIOR TO FINAL ACCEPTANCE ALL WATER MAIN PIPE SHALL BE DISINFECTED BY THE CONTRACTOR.

IF NOT ALREADY SO, THE CONTRACTOR SHALL CONSTRUCT SANITARY SEWER WITH ONE20 FOOT NOMINAL LENGTH OF PRESSURE PIPE MATERIAL CENTERED ON THE WATER MAIN AS PROVIDED INSECTION 22.02(B), SUCH THAT THE MAXIMUM POSSIBLE SEPARATION BETWEEN THE WATER MAIN AND THESEWER PIPE JOINTS WILL RESULT. THE BACKFILL MATERIAL SHALL BE SELECT, LOW PERMEABILITY SOIL.

4. AT ALL WATER MAIN CROSSINGS, SANITARY SEWERS SHALL BE LAID AT SUCH AN ELEVATION THAT THE TOPOF THE SANITARY SEWER IS AT LEAST 18 INCHES BELOW THE BOTTOM OF THE WATER MAIN.

5. ALL WATER MAIN CONSTRUCTION TO BE DONE UNDER A PLUMBERS PERMIT AND SHALL COMPLY WITH ALLAPPLICABLE BUILDING CODES.

6. ALL WATER METERS, PUMPS, AND BACKFLOW PREVENTERS ARE LOCATED INSIDE BUILDING. REFER TOPLUMBING PLANS FOR DESIGN AND LOCATIONS.

7. WATER SERVICE PIPE SHALL HAVE A 5.0' MINIMUM BURY DEPTH AS MEASURED FROM FINISHED GROUND TOTOP OF PIPE.

8. SITE SHALL BE TO FINISHED GRADE PRIOR TO INSTALLATION OF WATER SERVICE.

9. MATERIAL OF PIPE FROM TAP OF PUBLIC WATER MAIN TO METER IN BUILDING SHALL BE TYPE "K" SEAMLESSCOPPER (3/4" TO 3"), DUCTILE IRON PIPE (3" AND LARGER), OR AWWA C900 PVC (4" AND LARGER) PER LOCALCODES AND REGULATIONS.

10. REFER TO PLUMBING PLANS FOR WATER SERVICE TIE-INS WITH BUILDING.

THE PERMITTEE SHALL COMPLY WITH THE GENERAL NPDES PERMIT FOR STORM WATER DISCHARGEFROM CONSTRUCTION SITES TO WATER OF THE STATE OF NEBRASKA, ISSUED JANUARY 1, 2008.

1.

THE PERMITTEE SHALL REMOVE ACCUMULATED SEDIMENT FROM BEHIND SILT FENCES DURING THE LIFE OF THIS CONTRACT.ACCUMULATION OF SEDIMENT MUST NOT BE ALLOWED TO REACH ABOVE HALF THE HEIGHT OF THE FENCE. ALL EROSIONCONTROL FEATURES SHALL BE KEPT IN WORKING ORDER DURING THE LIFE OF THIS CONTRACT AT NO ADDITIONAL COST TO THEOWNER.

DUST BLOWING CONTROLTHE PERMITTEE SHALL USE EITHER OF THE FOLLOWING MEASURES OR A COMBINATION OF BOTH IF NECESSARY:A. TANK TRUCK OR OTHER SUCH EQUIPMENT TO SPREAD WATER ON THE SOIL SURFACE.B. INSTALL PICKET FENCE WIND BREAKS. FENCES SHALL BE PLACED AT THE RIGHT ANGLE TO THE PREVAILING WIND CURRENTS.

5. THE PERMITTEE SHALL PROVIDE A STABILIZED SURFACE FOR EACH CONSTRUCTION ENTRANCE AS DELINEATED ON THE SITEPLAN. THE STABILIZED SURFACE SHALL CONSIST OF A 6" THICK BASE OF CRUSHED ROCK OVER A WOVEN GEOTEXTILE FABRICMANUFACTURED BY MIRAFI, PRODUCT #HP 370 OR EQUAL AS APPROVED BY ARCHITECT. AN UPDATED COPYOF THE SWPPP SHALL BE ON SITE AT ALL TIMES. THE PERMITTEESHALL MAINTAIN AN UPDATED COPY AT ALL TIMES ANY WORK ISBEING PERFORMED ON SITE. THE PLAN SHALL BE UPDATEDWHENEVER EROSION AND/OR SEDIMENT CONTROLS ARE MODIFIEDAS SET FORTH IN THE PLAN. THIS DOES NOT CIRCUMVENT THEPERMITTEE'S RESPONSIBILITY TO OBTAIN APPROVAL FORMODIFICATIONS THAT MAY CONCURRENTLY FALL UNDER THEJURISDICTION OF OTHER GOVERNMENTAL AUTHORITIES. THEPERMITTEE SHALL MAINTAIN A RECORD OF ALL MODIFICATIONS OFCONTROLS ON THE PLAN. THE RECORD SHALL INCLUDE DATE OFMODIFICATION AND REASON(S) FOR MODIFICATION.

THE SITE SHALL BE INSPECTED TO IDENTIFY MAINTENANCE NEEDS AND/OR SWPPP DEFICIENCIES ONCE EVERY FOURTEEN(14) CALENDAR DAYS, AND WITHIN 24 HOURS AFTER A PRECIPITATION EVENT OF 0.5 INCHES OR GREATER. ANY DELAY IN THEREPLACEMENT OR MAINTENANCE OF NON-FUNCTIONAL BMP'S BEYOND SEVEN (7) CALENDAR DAYS SHALL BE DOCUMENTEDIN THE SWPPP WITH SUFFICIENT DETAIL AS TO EXPLAIN THE REASON FOR THE DELAY.

QUALIFIED PERSONNEL SHALL INITIATE AND COMPLETE CORRECTIVE ACTIONS TO ADDRESS ANY MAINTENANCE NEEDS ORDEFICIENCIES AS SOON AS POSSIBLE. MAINTENANCE AND REPAIR OF SILT FENCES, CURB FILTERS OR ANY OTHER BMPSHALL BE COMPLETED WITH IN 7 DAYS AFTER DEFICIENCIES ARE DISCOVERED.

THE PERMITTEE SHALL MAINTAIN RECORD LOGS OF SITE INSPECTION AND MAINTENANCE ACTIVITIES FOR AT LEAST THREEYEARS FROM THE DATE THAT PERMIT COVERAGE EXPIRES OR IS TERMINATED. THE PERMITTEE MUST PROVIDE NDEQ, THEARCHITECT, AND THE OWNER/OWNER'S REPRESENTATIVE, WITH ACCESS AND COPIES OF THESE RECORDS UPON REQUEST.A COPY OF THESE RECORDS MUST BE MAINTAINED ON SITE.AT A MINIMUM THESE RECORDS MUST INCLUDE:
