Si raccomanda di identificare l’ostruzione sulla base del FEV1/VC <5° percentile del predetto (<88% del predetto, usando teorici CECA) (ATS/ERS ’05).

Misura il flusso di aria espulso con la massima forza possibile.
• Si esegue 2 volte al giorno, eseguendo ogni volta 3 prove, di cui si sceglie il valore di PEF più alto.
• Tali valori vengono quindi riportati su un “diario del respiro”.

FENO per la diagnosi e il monitoraggio dell’asma (II)
Il monitoraggio di FENO permette di valutare se l’asma è ben controllata soprattutto nei pazienti steroido-naive ed in coloro che sono in trattamento con dosaggi di CS inalatori medio-bassi (Smith NEJM 2005, Shaw AJRCCM 2007, Michils ERJ 2008).
Valori di FENO <25-30 ppb si associano di norma ad un buon controllo (Gelb Chest 2006, Michils ERJ 2008).
Il monitoraggio di FENO può aiutare a predire le riacutizzazioni in pazienti controllati dalla terapia in cui gli steroidi vengono ridotti o sospesi (Pijnenburg Thorax 2005, Zacharasiewicz AJRCCM 2005)
L’uso del FENO per adeguare la terapia antinfiammatoria non è stato confermato dagli studi più recenti (Szefler Lancet 2008; DeJongste AJRCCM 2009)

• 6 pazienti su 7 asmatici che utilizzano steroidi non raggiungono nell’ultimo anno un buon controllo della malattia .
• La maggior parte dei soggetti con malattia non controllata sono sottotrattati
• L’italia e’ il paese dove si usano meno corticosteroidi inalatori e i pazienti hanno l’asma meno controllata
• Tra i pazienti che assumo steroidi solo il 30% ha un piano terapeutico scritto e solo il 22% ha eseguito una spirometria nell’ultimo anno
• L’asma poco controllata e’ associata a tosse cronica, sovrappeso e sensibilizzazione al Cladosporium
