to act in their best judgment to obtain proper medical treatment for my child. I understand that the 2014 Spring Break Basketball Camp does not provide insurance, and that I am fully
responsible for my child’s medical expenses.
I expressly assume all risk of injury relating to or arising out of this participation, howsoever caused, and accept personal responsibility for the damages following such injury, perma-
nent disability or death.
I grant full permission to the 2014 Spring Break Basketball Camp organizers to use, without further notification, authorization or compensation, any photographs, video, recordings or
other record of my child’s participation in any and all media, whether now known or hereafter.
