the Leading Causeof Blindness

Cataract

It is a reversible disease and excellent vision can be obtained

by a procedure in which the cloudy natural lens is replaced by an

artificial lens. This however can only be done in a hospital

under antiseptic conditions and by a

skilled ophthalmologist. Unfortunately these

facilities are not available in small towns and

rural areas.

Cataract is theleading cause of blindness/

Impaired vision in Pakistan.

It is a disease ofaging but can occur

in children due to genetic causes,

trauma, diabetes etc.

With the average per capita income of a

Pakistani being Rs 19,000 per month the choice before them is stark: Get the surgery done and starve or slowly descend

into a world of darkness. Starvation is the ultimate

end in the latter too but it is deferred.

The procedure costs a minimum

of Rs 15,000 per eye and can go upto

Rs 150,000.

LRBT wasestablished

in December 1984 because the Founders

thought that no human being should be faced with such

a choice.

Starting witha humble mobile unit

in 1985 LRBT has grown to a network of 77 facilities:

19 purpose built hospitals and 58 clinics.

With exception of 3 Tertiary hospitals and a

secondary hospital in Quetta all the rest are located in small towns

and rural areas where poverty is the highest, there is complete lack

of ophthalmic facilities and where they are available it is beyond the

reach of 80 % of Pakistanis due to affordability.

In the last 34 years we have performed 2.8 million cataract

surgeries, absolutely free for the poor,

the highest of all eye care providers including

the Government.

This network covers 78 % of

Pakistanis who can access these

facilities within a bus ride of a maximum of

3 hours.

Noor

Generally, people believe that cataract in a baby is unheard of yet it is possible and comes as a huge shock to the parents. Living in a small town and being uneducated gave birth to a lot of theories by well-meaning relatives with the consensus being that the child would be blind throughout her life.

Despite the hopelessness surrounding him Bahadar Ali remained strong in his quest to find a cure for his seven month old baby girl’s vision problems.

Baby Noor was only 3 months old when her mother realized that she did not focus on things and later on developed white opacities in the center of the eye. Having an older son she knew there was something wrong with her daughter’s eyes. She shared her concerns with her husband who left no stone unturned for her treatment but her condition did not improve. Bahadur Ali, a car mechanic by profession had exhausted all his resources and with no savings left he felt extremely helpless but he had firm belief in miracles and went on and borrowed money and took her to a clinic where a colleague had gone for his mother’s cataract surgery. A ray of hope emerged when the doctor said she had congenital cataract and it could be cured with surgery, the only issue being that she would have to be operated under general anesthesia and that facility was not available at the clinic. Seeing Bahadur’s condition the doctor advised him to go to LRBT Korangi. Baby Noor was carefully examined and had successful surgeries in both her eyes. The joy her parents experienced when she started showing interest in surroundings and responded by smiling was their best gift ever.

Bila

lParents want to provide a perfect life for their children. Bilal was ecstatic at the birth of his first born, Sohaib. He made sure that his son was never deprived of anything and sent both his children to school. As a result he had to live in a one room rented house with his wife, children and ailing parents. Bilal worked from 7 am till dark as a laborer doing back breaking work every day, yet he was never too tired to play with his children. Unlike others in his locality he made sure that his children went for regular checkups and that their vaccinations were up to date.

One day while playing outside with other children, one of the toys hit Sohaib in his left eye. He experienced piercing pain and started to cry, his mother immediately rushed outside and put ice on her son’s eye to stop it from swelling. Sohaib calmed down enough after ten minutes to tell his mother what had happened. She examined his eye closely, there was no swelling but it was red. As soon as Bilal reached home they took him to an eye doctor who gave drops and assured them their son’s eye will heal. Unfortunately that was not the case as they discovered when Sohaib’s teacher called the parents to tell them that he was unable to see the board properly and to take him to an Ophthalmologist. Sohaib was diagnosed with traumatic cataract as a result of the injury to his eye and to save their son from going blind in that eye, Bilal would have to pay Rs. 30,000. He made every effort asking friends and relatives for help but despite their contributions he was unable to raise enough funds. His world came crashing down, he saw his son’s future disintegrate in front of his eyes. Seeing his distress his employer enquired what was worrying Bilal and upon hearing about Sohaib’s ailment he advised him to go to LRBT Korangi Hospital immediately. Sohaib was examined in the pediatric ward and underwent surgery. When his bandage came off the miracle they were praying for happened and Sohaib could see. The dark clouds had lifted and the sun was shining again. Joyfully Bilal hugged his son and thanked the doctors and institution for giving Sohaib a new life.

Sofi

a

“When my husband lost his job due to a serious accident that left him handicapped for life all we were left with was a 2 roomed rented house, eight children and my self-determination. These were my weapons to fight life’s battle as I had never gone to school nor was I equipped with any skill but with the spectre of being cast out of the house by the landlord and begging for food and shelter on the streets I was ready to take the challenge, head on. Out of eight of my children four were school going. I promised myself that I will come out as a warrior and will never let my children beg for their needs” recalls Sofia.

Days and sleepless nights went by and the poor couple would stay up thinking how to run their affairs. Life had been difficult enough having to manage on in his meagre wages but this was also was gone. The savings ended, small assets sold and now to top it they were burdened with loans from relatives.

And then came some good news: a neighbor told them about the canteen in the neighborhood children’s school was looking for a homemade food supplier. Since Sofia was a good cook she approached the management to allow her to become the supplier. Her test products got great remarks and the family finally started to earn. Better days started and slowly life was becoming a little normal when Sofia started to feel blurring of objects. She went to a neighborhood doctor who diagnosed cataract and advised her to go to an eye hospital for surgery costing Rs. 30,000. This was way beyond her means. So she carried on until she started facing issues with school management due to late deliveries. She shared the reason with the school administrator who advised her to go to LRBT which offered free eye treatment for the poor. She couldn’t believe this so with some hesitation but faint hope she visited LRBT eye hospital where she was operated and her sight was restored. All totally free.

Today Sofia supplies food to 3 schools in the neighborhood and her 2 other children have also started going to school. Her dream of educating her children and providing them opportunities to move up the socio-economic ladder is now on the way to realization.

Sa

qib

Saqib has always loved working with his hands. He remembered his love of drawing from an early age and then the hours he would spend watching his father craft beautiful pieces from a block of wood. His father seeing Saqib’s keen interest in his work started teaching him carpentry, Saqib would come back home from school and spend hours carving out small toys for his 5 younger siblings. In time he mastered the art and became an excellent carpenter.

In the beginning he made furniture for a rich housewife who would then sell the pieces on order to her friends and family, but unfortunately she moved abroad taking Saqib’s steady source of income with her. Saqib then got a job with a contractor who made houses and Saqib did the woodwork. Saqib was happy in his job, doing what he loved and managed to earn enough to support his aging parents, an unmarried sister, his wife and two children.

A year ago Saqib felt that his vision was deteriorating and he often made mistakes in measurement and took twice as long to get a task done. His employer eventually had to lay him off and Saqib was sitting at home unemployed for a couple of months. He went to a local doctor who prescribed him eye drops temporarily but told him to go see an Ophthalmologist. With no source of income he could not afford to go to a good doctor, a friend found him work as a security guard in his employer’s home and Saqib was thankful for being employed once again.

However, the vision in his left eye continued to worsen and Saqib feared losing his current job too. It was while he was talking to a couple of guards in the street that someone mentioned LRBT i and its free eye care services for the poor. Hopeful Saqib went for a checkup and was diagnosed with cataract. He knew he could hardly afford the examination but there was no way he could pay for the surgery. His fears were put to rest, when the counselor told him his entire treatment would be totally free.

After a successful surgery he is now able to work. He hopes to start searching for a job as a carpenter again and continue doing what he loves most – playing with his children. ‘‘‘‘‘‘‘‘

Ali is one of the 3 million children suffering from eye ailments for whom LRBT is the only hope. Two years ago, only 6 at the time Ali lost his eye sight while playing cricket in his neighborhood. Local doctors and home remedies worsened the case and eventually he lost his sight in that eye. He had to quit school as his other eye also started getting affected due to exertion and additional strain. His father, Syed Ghulam a daily wage earner, was devastated to see his son turning blind but was helpless as he could not afford corneal transplant advised by various ophthalmologists.

The only boy and youngest of five siblings, Ali was the apple of his parent’s eye and loved by everyone for his bubbly personality, ready smile and cheerful disposition. After this incident Ali underwent a drastic change, he stopped going outside to play and refused to meet his friends. Despite their best attempts, his sisters could not cajole him to participate in any activity. He was usually found sitting alone in a corner somewhere, lost in his thoughts. He repeatedly asked his mother if he would remain blind for the rest of his life; his mother struggled to control her tears while she consoled her son, secretly hoping for a miracle. It broke their hearts that they could not end his misery.

And then emerged a beacon of hope: Ali’s uncle who was visiting from Karachi seeing the misery of the child told them about LRBT free eye hospital in Karachi offered to take them there. His eye condition was alarming and LRBT doctors were doubtful of the surgery’s success, but as Ali had his entire life ahead of him they took the

risk. Ali’s Keratoplasty was performed successfully and today he is able to see.

Syed Ghulam took a couple of days off from work to come see his son and accompanied him to his follow-up visit to the hospital, he was overcome with emotion when the doctors assured him that Ali’s eye was healing well and he should be able to travel to his hometown soon. He thanked the doctors and said.

Khawaja Zafar Hassan, a Founder Trustee, the 2nd Chairman and Chairman Emeritus since December 2014, left this world on 6th February 2019. We will miss his wise counsel, his passion and commitment to the cause of eradicating avoidable blindness. May his soul rest in peace. Earlier on 29th November 2018 we lost Zain Anjarwala another Trustee. May God grant him eternal peace too.

Our purpose built Chiniot Hospital funded completely by the Chiniot Anjumun Islamia opened its doors in April 2019. The hospital has a large catchment area and will provide quality eyecare services absolutely free to 14 million poor patients living in Chiniot, Faisalabad and Jhang Districts.

An additional 1300 square yards of land was gifted by Mr. Usman Peracha, a Trustee, which enlarges the frontage of our Mandra Hospital on the GT Road enabling the construction of a new Building which will house all the departments, currently scattered, in one premise. This will ensure smooth patient flow and also provide more space to expand our eye care activities. The new construction is being funded by a generous donor from the UAE.

There is an urgent need for an eye hospital in the Rahim Yar Khan area which has a high level of poverty and with no free facilities available a relatively high level of blindness and impaired vision. We are seeking a donor for this project.

Our phased upgradation of equipment continues and we spent Rs 178 million this year on acquiring the latest state of the art machines to deliver high quality care to our patients. This brings the total expenditure under this head of 364 million in the last 2 years. Our hospitals are the best equipped in the country with Korangi in particular possessing equipment which is even not available in many teaching hospitals in UK and USA. This plus our experienced doctors and our technicians deliver high quality eye care helping us in surpassing the WHO bench marks of surgical outcomes by wide margins.

With Diabetes afflicting 17% of our population our focus on Diabetic Retinopathy treatment continues and in addition to Medical Retina at 11 of our hospitals 6 of our Hospitals now provide surgical treatment as well with the 7th Mandra being added in 2019/2020.

All our achievements were made possible34 years would still be suffering a life of darkness and despair. We would also like express our gratitude to our volunteers, the media and our staff whose devotion and dedication has made LRBT the largest provider of eye care in the world.

All our achievements were made possible by our large hearted generous donors without whom the 42 million patients that we have treated so far in our existence of 34 years would still be condemned to a life of darkness and despair. We would also like express our gratitude to our volunteers, the media and our staff whose devotion and dedication has made LRBT the largest provider of eye care in the world.

Chairman’s ReportK. Zafar Hassan1923 – 2019Chairman (Emeritus) LRBT(Founder Trustee of LRBT, 2nd LRBT Chairman)

“Humans like you are very rare. You were the rarest combination of integrity, compassion, vision and passion.”

Pakistan lost a great humanitarian on the 6th of February 2019. Khawaja Zafar Hassan gave 30 years of his life to LRBT, an organisation which played a major role in halving the prevalence of blindness in the country. His services contributed to removing darkness from the lives of millions of underprivileged Pakistanis. He and his team helped LRBT achieve its vision to become the largest eye care network in the world. He has left behind a great team that will continue the mission with the same zeal & passion.

More than 40 million families that have benefited from LRBT pray that the Almighty grant him eternal peace.

He will always be missed, respected and remembered.

OPD VISITS / SURGERIESDuring the year ending June 2019, 3.1 million patients visited our 19 hospitals and 58 Community Eye Health Centers, a growth of 3 % over the previous year. During the same period 284,523 major and minor surgeries were performed. Growth in major surgeries was 5 % over last year. Cataract, which is the leading cause of curable blindness constituted 90% of major surgeries. Other surgeries included vitreo-retinal procedures, Trabulectomy, Cornea transplants, DCR, Squint etc.

17,142 retinal procedures were performed a growth of 7 % over last year. This included 15,377 patients who were treated with Avastin for Age Related Macular Degeneration and Diabetic Retinopathy. 33,904 laser procedures were also performed. 353 corneas were transplanted at our Lahore Tertiary, Korangi Tertiary and Quetta Hospitals.

We remain the biggest provider of Cataract, Vitreo-retinal and Corneal transplant surgeries for the poor people in the country.

In our aim to ensure the highest quality of surgical care we regularly evaluate ourselves against the WHO/ICEH benchmark for good cataract surgery. This is expressed in terms of visual outcomes. In 2018/19 we were 7 percentage points higher than the benchmark.

EXPENSESOur operating expenses during the year were Rs. 1,286 million being 16.4% above last year. Main reasons are growth in patient visits and increase in cost of medicines besides normal annual salary increment. Capital Expenditure during the year was 281 million which is 24% higher than last year. This includes medical equipment at Rs.178 million, hospital buildings Rs.70 million and generators Rs.14 million and other furniture, fixtures, and office equipment Rs.19 million. Our overhead expenses reduced to 4.43% (2018: 4.64%) of operating expenses and are among the lowest in the world.

EXPANSION OF CAPACITY KorangiA project to house the consultant clinics and a pediatric wing with comfortable waiting areas was urgently required due to steady growth in the number of patients visiting the Tertiary facility to seek specialized treatment. This project was launched on 1st March 2019 and is expected to be completed towards the end of February 2020.Concrete slab of the roof has been laid while masonry work and electrical wiring are in progress presently. Ms. Khurshid Bhaimia, our longtime and loyal supporter is financing this project. The Bhaimia family had earlier sponsored the entire second floor of the main Hospital building. In the year 2011 the family again financially supported the construction of the new Outpatients Department.

Upgradation of the vitreo retinal unit and its allied services to enable LRBT in providing its services in the best possible environment had been underway since May 2018. This project generously sponsored by Amreli Steels Ltd was completed in three months and was fully operational in the last week of July 18.

ChiniotA new 50 bedded facility located in the outskirts of Chiniot on Jhang Road was handed over to LRBT by the Chiniot Anjuman Islamia in a simple yet impressive ceremony held at the Hospital premises in April 2019.

We are grateful to the Chiniot Anjuman Islamia for their continuing support to LRBT.

ShahpurThere has been a significant increase in the number of patients visiting this facility with the addition of a state of the art vitreo retinal unit. As there was paucity of space being felt especially for the patients being examined at the refraction station, therefore, two independent refraction rooms for males and females with waiting areas have been added so that this activity can take place in a seamless manner. Under a project, LRBT was also required by its partner, Sightsavers, to open an optical shop within the Hospital to provide quality eye wear at affordable prices to the poor patients who visit the facility. This shop is fast nearing completion and shall be handed over towards the end of the third quarter this year.

Toba Tek SinghThis primary eye care center has been operating since November 2008 and at an average of 120 patient visits every working day. Last year over 36,000 persons were examined, 7015 were refracted and 3648 requiring further investigation, treatment or surgery were referred to our nearest Secondary Hospital located in Lar, a 3 –hour bus ride away.

One of GLT’s long standing supporters, Mr. Arshad Ali offered to upgrade this center through renovation and remodeling. Civil works commenced in October 2018 and were completed in mid-January this year.

SOPHISTICATED EQUIPMENT The following state of the art equipment was provided to our various Hospitals to enable our doctors to provide quality eye care to the poor people who visit them ensuring that free treatment does not mean second rate treatment.

Patron: The Right Honourable The Lord Dobbs

Board of Advisors: Mr. Khalid Aziz, LVO, DL, The Most Hon. the Marquis of Salisbury, DL, Sir Sherard Cowper-Coles KCMG LVO, Miss Susan Farrington, Mr Ram Gidoomal CBE, Lord Hannay of Chiswick GCMG, Sir Christopher MacRae, KCMG and Lady MacRae, The Rt. Rev’d. Michael Nazir-Ali, former Bishop of Rochester, The Rt. Hon. Sir Malcolm Rifkind KCMG QC, Lord Michael Sandberg, CBE.

Staff: The Trustees are pleased to report that Nusrat Mirza, a former Trustee, accepted the appointment of CEO of the Trust effective 9th April 2019. Salina Zaheen was promoted to the position of Manager Administration and Marketing and has made significant contributions since her appointment on 1st January 2018.

Volunteers: The Trust thanks the following core volunteers who have lent invaluable support despite their work commitments: Zara Zaheen, Rehan Aziz, Imran Azam, Scherezade Azim, Zeshan Mirza, Ameera El Syed, Ava Mehta, Sara Zafar, Aatif Khan, Mukti Adnani, Asad Ansari. Additionally, the following occasional volunteers also provided valued support: Nizar Din, Rehan Rajput, Sarah Jane Wilford, Khaver Idrees, Ansar Rafiq, Heera Rahman, Afshan Mirza, Asad Mirza, Wajeha Qureshi, Amaad Mahmood, Zoya Mufti.

Fundraising: October 2018 Gala: This year’s Gala evening was held at The Savoy Hotel and hosted by our Patron, Lord Michael Dobbs. Amongst the 230 guests were some well-known stars of stage and screen including Trustee Art Malik and veteran actor Hugh Quarshie. We received extraordinary support from celebrities who had sent ‘good wishes’ video messages, including a heartfelt message of support from Dame Judy Dench. We auctioned some rare ‘money can’t buy’ items such as a walk-on part in a successful British television series. The incomparable Lord Jeffrey Archer very generously conducted the auction with his deft manipulation of the audience and trademark sharp wit. Ms Rahila Zakir, consultant surgeon at the Western Eye Hospital in London shared insights into curable blindness and provided an opportunity for guests to experience visual impairment with vision simulation glasses. The highlight of the evening was a live video link up with Fatima, a patient who received a life transforming cornea transplant and she spoke about the positive changes that have resulted from surgery. The audience responded most generously, with the event raising £105,660 net of expenses.

Two Ramadan fundraising dinners: Held in London and Manchester, these functions successfully built upon the relationships that had been developed with new donors as a result of similar events in the previous year. As the live video link with Fatima, a blind young patient whose eyesight had been restored, had proved to be so successful at the gala, this initiative was repeated at the Ramadan events. The charismatic Imam and Broadcaster, Ajmal Masroor once again fundraised and in Manchester, Shahid Khan sang nausheeds. The total sum raised from both events was £131,215 net of expenses.

The Ramadan Zakat TV, digital and postal campaigns: These campaigns generated £89,696 during the year.

Report of the Trustees of the Graham Layton Trust

Christmas Campaign: A postal and digital Christmas campaign provided an opportunity to touch base with donors once again, when significant further contributions were received.

International Women’s Day: On 8th March, this was marked by creating a 30 second video of Qurat, a young woman who received a cornea transplant from LRBT and subsequently started working for a bank. The surgery meant she became economically independent and reinforced the wider benefits to individuals and society of supporting eye care. The video was edited from footage from the 2017 film trip to Pakistan.

Young professionals’ dinner: GLT held a small dinner for young working professionals at Shola, a new Asian restaurant before its official opening in London’s West End. The event, held in March was a great success, as it provided young professionals an opportunity to mingle with like-minded individuals while helping to raise awareness of GLT and promoted the upcoming Macchu Picchu trek being undertaken by a GLT volunteer

Other events: The clinic in Havelian, which was officially opened in April 2019, continues to be supported by a generous UK donor. In agreement with LRBT, the UK donor would provide the running costs for the clinic for which we are most grateful.

We continue to build relationships with individuals and communities via mosques and this year received £3,775 from such donations.

Matthew Wallen ran the London Marathon to support GLT and raised £2,053 from his own network, helping widen the awareness of GLT.

Aatif Khan undertook a trek to Macchu Picchu and raised £1,129 for GLT

Fundraising financial reportIncome: The fundraising total for the year was £557,196 of which £347,518 was raised from various fund-raising events held during the year. The Trust’s remittances to LRBT amounted to £387,153.

The Trust’s income has included significant quarterly donations from the Lady Fatimah Trust and generous one-off donations from individuals.

The Financial period also saw the continued support of generous benefactors from Europe and the Middle East as well as the UK. GLT is sincerely thankful for these large private donations from individuals who are passionate about the work of LRBT and want to see the organisation grow and develop further.

GLT continues to successfully increase its focus on generating interest in its cause amongst the younger generation of volunteers, students and donors.

Outgoings: During this 12-month period, GLT remitted £145,326 specifically for the purchase of IOLs in Pakistan for use in cataract operations in all LRBT Hospitals and the purchase of Medical eye equipment of £178,868. GLT was well able to meet its commitment of funding the supply of intra-ocular lenses to LRBT’s hospitals in 2018/19 from its reserves and the net income generated this year.

GLT is expecting, on a best endeavours’ basis, to meet LRBT’s current forecast of the cost of IOLs of approximately £293,000 to Pakistan during the 12 months to December 2019.

Finally, the Trustees would like to record their thanks to all the many donors for their generosity and to the supporters and volunteers who have, according to their means and time available, contributed so generously to GLT and to its performance in this period.

GOVERNANCE

TrusteesNoaman Sanni M.D. and Jaap Westrik.

Jaap Westrik was formally appointed a Trustee in February 2019 and he also visited hospitals in Pakistan in October 2018 to see first-hand the excellent work LRBT does in Pakistan.

The team in the US continues to look for trustees and advisors to help support and develop fundraising initiatives across the United States.

Consultants and VolunteersAmal Piracha, Kainat Akram, Muhammad Talal Javed Ghuman, Jason Clement and Cindy Arquiett

Fundraising The fundraising total for the year was $163,800. This is almost double the amount raised in 2017 and is highest amount raised since LRBT USA was formed. Ramadan was the most productive month for donations which were received via the website online, text to donate and checks by donors across the United States.

The donor database is growing with many donors making large private donations. LRBT USA is sincerely thankful to these individuals who are passionate about the work of LRBT and want to see the organization grow and develop further.

Stop Diabetic Blindness CampaignLRBT USA launched the campaign to tackle the risks of losing eye sight – also known as diabetic retinopathy. The aim is to raise money to launch screenings for diabetic retinopathy across LRBT hospitals in Pakistan.

Noaman Sanni, M.D. has been championing the campaign. The message has been : “People living with diabetes are at particular risk of sight loss – but this complication is avoidable. If you have diabetes and are over 12 years you should be having your eyes screened for retinopathy by visiting a LRBT hospital in Pakistan. The screening looks for specific changes in the eye that cause blindness, and it is effective at reducing your risk of sight loss.”

RamadanAhead of the holy month, many plans were set in motion from nationwide TV advertisements, digital marketing including social media, newsletters and offline engagement with donors. LRBT USA also registered on a social platform, Benevity which helped raise donations from employees at LinkedIn, Apple Inc and Microsoft.

Social mediaFacebook has grown 60% in followers in the year and Instagram is strong with an increased volume of engagement and followers. Once a follower is gained, each person is directly

messaged to ask for continued support by sharing our posts, joining LRBT USA’s newsletter and also donating.

Newsletters To engage existing and potential donors, 16 newsletters were distributed in the year. There were many responses and online donations post distribution from existing supporters.

Celebrities Renowned singer Hadiqa Kiani, Anchor person Mubashir Lucman and Actor Javed Sheikh showed support by filming videos to help raise awareness of LRBT and the impactful work the organization does. These videos were promoted on social media, YouTube and newsletters.

Thank youLRBT USA would like to express their gratitude and sincere thanks to all donors for their generosity to help provide free eye care for the poor in Pakistan.

A sincere thank you to Mrs. Munira Ahmed for supporting the organization and her plans to help build the organizations presence in San Francisco, California.
