Dabigatran Association With Higher Risk of Acute Coronary Events: Meta-analysis ofNoninferiority Randomized Controlled TrialsUchino, K et al.The authors concluded that dabigatran is associated with an increased risk of MI or ACS in abroad spectrum of patients when tested against different controls. Clinicians should considerthe potential of these serious harmful cardiovascular effects with use of dabigatran.Arch Intern Med. Published online January 9, 2012. doi:10.1001/archinternmed.2011.1666

Effect of Aspirin on Vascular and Nonvascular Outcomes: Meta-analysis ofRandomized Controlled TrialsSeshasai, S et al.The authors conclude that despite important reductions in nonfatal MI, aspirin prophylaxis inpeople without prior CVD does not lead to reductions in either cardiovascular death or cancermortality. Because the benefits are further offset by clinically important bleeding events, routine use of aspirin for primary prevention is not warranted and treatment decisions need tobe considered on a case-by-case basis.Arch Intern Med. Self-monitoring of blood glucose in patients with type 2 diabetes mellitus who are notusing insulinMalanda, UL et al.The authors of this Cochrane Review concluded that when diabetes duration is over one year,

the overall effect of self-monitoring of blood glucose on glycaemic control in patients with type2 diabetes who are not using insulin is small up to six months after initiation and subsidesafter 12 months. Furthermore, based on a best-evidence synthesis, there is no evidence thatSMBG affects patient satisfaction, general well-being or general health-related quality of life.http://onlinelibrary.wiley.com/doi/10.1002/14651858.CD005060.pub3/abstract

Getting it together for mental health care: Payment by Results, personalisation andwhole system workingNational Development Team for InclusionThis paper considers how the two policy initiatives of Payment by Results (PbR) andpersonalisation need to be developed in tandem with one another, and in the context of wholesystem commissioning, if the aims of both are to be achieved. It raises questions for

It concludes with specific issues that need to beconsidered nationally and locally.http://www.ndti.org.uk/who-were-concerned-with/mental-health/payment-by-results-and-personalisation-in-mental-health-services/

Interferon beta for secondary progressive multiple sclerosis (SPMS)La Mantia, L et al.The authors of this Cochrane Review concluded that recombinant interferon (IFN) beta doesnot prevent the development of permanent physical disability in SPMS. This treatmentsignificantly reduces the risk of relapse and of short -term relapse-related disability. Overall, these results show that IFNs' anti-inflammatory effect is unable to retard progression, whenestablished. The authors believe this review gives conclusive evidence on the clinical efficacyof IFNs versus placebo in SPMS.http://onlinelibrary.wiley.com/doi/10.1002/14651858.CD005181.pub3/abstract

Lithium toxicity profile: systematic review and meta-analysisMcKnight, RF et al.The authors concluded that lithium is associated with increased risk of reduced urinaryconcentrating ability, hypothyroidism, hyperparathyroidism, and weight gain. There is littleevidence for a clinically significant reduction in renal function in most patients, and the risk ofend-stage renal failure is low. The risk of congenital malformations is uncertain; the balanceof risks should be considered before lithium is withdrawn during pregnancy. Because of theconsistent finding of a high prevalence of hyperparathyroidism, calcium concentrations shouldbe checked before and during treatment. The Lancet, Early Online Publication, 20 January 2012. doi:10.1016/S0140-6736(11)61516-X
