1. PURPOSE

To describe the methods by which Mohawk College (“Mohawk”) intends to identify, assess and

control hazards in confined spaces in accordance with Ontario regulatory requirements. Refer to

Appendix A for the Confined Space Inventory.

2. SCOPE

This Program applies to all Mohawk employees and contractors who manage or coordinate

“confined space” entries, and those employees and contractors who enter confined spaces or

conduct “related work”.

Mohawk College employees will not enter confined spaces or perform related work with respect to

confined spaces.

Mohawk College will hire contractors that are qualified with adequate training, knowledge and

experience to perform all aspects of confined space work and related work with confined spaces.

Only the Occupational Health and Safety Department may approve deviations from the

requirements of this Program.

3. DEFINITIONS

“Acceptable atmospheric levels” means that,

the atmospheric concentration of any explosive or flammable gas or vapour is less than,

o 25 per cent (%) of its lower explosive limit for inspection work with no source of

ignition,

o 10% of its lower explosive limit for cold work,

o 5% of its lower explosive limit and less than 23% oxygen for hot work,

the oxygen content of the atmosphere is at least 19.5% but not more than 23% by volume,

the exposure to atmospheric contaminants does not exceed any applicable limit set out in

Regulation 833 of the Revised Regulations of Ontario, 1990 (Control of Exposure to

Biological or Chemical Agents) or Ontario Regulation 490/09 (Designated Substances).

“Atmospheric hazard” means:

the accumulation of flammable, combustible or explosive agents,

an oxygen content in the atmosphere that is less than 19.5% or more than 23% by volume,

or

the accumulation of atmospheric contaminants, including gases, vapours, fumes, dusts or

mists, that could,

o result in acute health effects that pose an immediate threat to life, or

o interfere with a person’s ability to escape unaided from a confined space

“Attendant” means an individual assigned to ensure the safety of all entrants working in a

confined space. The attendant performs all duties outlined under the responsibilities section and

will be identified on the CSE permit

“Confined space” is defined as a fully or partially enclosed space,

that is not both designed and constructed for continuous human occupancy, and

in which “atmospheric hazards” may occur because of its construction, location or

contents or because of work that is done in it.

“Confined Space Entry Plan/Permit” means a document reviewed and completed prior to all

confined space entries to ensure controls are in place for all the hazards that have been

identified. This includes entry equipment requirements, atmospheric testing requirements, rescue

protocols, and all required preparation information. Refer to Appendix C and D for Confined

Space Entry Permits/Plans and Rescue Procedures respectively.

“Coordination Document” means a document prepared by the “Lead Employer” that is used to

ensure that employer duties are performed in a way to protect the health and safety of all workers

who perform work in the confined space when workers of more than one employer perform work

in the same confined space. Refer to Appendix E for a sample coordination document.

“Entrant” means an individual authorized and assigned to enter a confined space for the purpose

of performing work or rescue. The Entrant performs all duties outlined in the responsibilities

section and will be identified on the Confined Space Entry Permit/Plan.

“Entry Supervisor” means an individual authorized because of their knowledge, training and

experience in supervising and monitoring confined spaces. The Entry Supervisor creates and

terminates all confined space entry permits and performs all duties outlined in the responsibilities

section. This individual may be an Entrant or Attendant.

“Hazard Assessment” means an evaluation process conducted on each space to determine its

associated hazards, precautions required for entry, and classification under this confined space

program. Refer to Appendix B for Hazard Assessments.

“Lead Employer” means an employer who contracts for the services of one or more other

employers or independent contractors in relation to one or more confined spaces that are located

the lead employer’s own workplace, or in another employer’s workplace.

“Qualified” means, for the purposes of this Program, acceptable evidence of experience in the

recognition, evaluation and control of confined space hazards, and includes:

Certified Industrial Hygienist (CIH),

Registered Occupational Hygienist (ROH),

Certified Safety Professional (CSP),

Canadian Registered Safety Professional (CRSP)

Professional Engineer (P. Eng.), or

Other combination of education, training and experience acceptable to Mohawk.

“Related work” means:

Work that is performed near a confined space in direct support of work inside the

confined space (e.g. attendants; persons conducting gas testing).

“Restricted space”, for the purposes of this Program, means:

A space that is fully or partially enclosed, and that is not both designed and constructed

for continuous human occupancy and that is not classified as a confined space. A

restricted space may become a confined space if an atmospheric hazard(s) may be

generated by the work conducted in the space.

5. ROLES AND RESPONSIBILITIES

5.1 Facilities Management

5.1.1 Ensure that resources and support are provided to successfully implement this Program.

5.1.2 Ensure hazard assessments and entry plans have been completed prior to permitting

entry into a confined space.

5.1.3 Ensure that each entrance to a confined space is:

Adequately secured against unauthorized entry; or

Has been provided with adequate barricades, adequate warning signs regarding

unauthorized entry, or both;

5.1.4 Ensure that adequately trained and qualified workers or contractors perform work in

confined spaces, or perform related work with respect to confined spaces.

5.1.5 Ensure contractors are provided with a copy of the confined space program, hazard

assessment, confined space entry plan/permit and coordination document as required.

5.1.1 Ensure contractors work in accordance with O. Reg. 632/05 and Mohawk College’s

Confined Space Entry program, or with one the contractor provides that provides equal or

greater protection for the workers.

5.1.2 Ensure that an Entry Supervisor is in place to oversee all confined space entries.

5.1.3 Ensure copies of closed Confined Space permits are retained for inspection purposes

and copies forwarded to Occupational Health and Safety.

5.1.4 Ensure the Occupational Health and Safety Department is notified of confined space

entries prior to the entry being performed.

5.1.5 Ensure the coordination of contractors that enter confined spaces or perform related work

at Mohawk.

5.2 Contractors

5.2.1 Work in accordance with O. Reg. 632/05 and Mohawk College’s Confined Space Entry

program, or one that provides equal or greater protection.

5.2.2 Ensure all workers working in a confined space or performing related work with respect to

a confined space have adequate training to perform the work safely.

5.2.3 Provide their own entry and rescue equipment.

5.2.4 Prepare and complete a Coordination Document as required.

5.2.5 Provide copies of closed Confined Space permits to Mohawk College.

5.2.6 Notify their Mohawk College contact person of any problems or concerns regarding the

confined space entry program.

5.3 Occupational Health and Safety Department

5.3.1 Coordinate the development of the Confined Space Program, including assisting with the

identification, hazard assessment and designation of all confined spaces at Mohawk

College facilities.

5.3.2 Ensure that the workplace is assessed for confined spaces by a suitably qualified person

and who has adequate knowledge, training and experience in confined spaces.

5.3.3 Ensure a written inventory of confined spaces is prepared and maintained.

5.3.4 Maintain a written record of the qualifications of the person who conducted the

Assessments.

5.3.5 Ensure that the Entry Plans are reviewed as often as is necessary to ensure that the

plans and permits remain accurate and adequate.

5.3.6 Ensure that the following records are retained:

Hazard Assessments

Plans

Records of Training

Entry Permits (including air testing results)

Rescue Equipment Inspection Records

Co-ordination Documents

for at least the longer of the following periods:

one year after the document was created; or

the period that is necessary to ensure that at least the two most recent records

that relate to a specific confined space.

5.3.7 Ensure that all Mohawk employees authorizing or contracting services for work in

confined spaces receive adequate training in consultation with the JHSC.

5.3.8 Obtain copies of closed Confined Space Entry permits from Facilities Management.

5.3.9 Ensure that the following records are retained:

Hazard Assessments

Plans

Records of Training

Entry Permits (including air testing results)

Rescue Equipment Inspection Records

Co-ordination Documents

for at least the longer of the following periods:

one year after the document was created; or

the period that is necessary to ensure that at least the two most recent records

that relate to a specific confined space.

5.3.10 Review and update this Program in consultation with the Joint Health and Safety

Committee as required.

5.4 Attendant

5.4.1 For each confined space entry, an attendant shall be assigned and shall:

5.4.2 Be stationed outside and near the entrance to the confined space (if there is more than

on entry point, the one that provides the best communication with the entrant shall be

used);

5.4.3 Be in constant communication with all workers inside the space (using the means of

communication described in the relevant plan);

5.4.4 Be provided with a device for summoning immediate on-site rescue response;

5.4.5 Never enter the confined space at any time unless relieved by another qualified Attendant;

5.4.6 Continuously monitor the safety of the entrant(s) inside the confined space at all times

5.4.7 Not perform duties that may interfere with the primary duty of monitoring and protecting

the entrant(s).

5.4.8 Prevent entry of unauthorized individuals during confined space work.

5.4.9 Provide assistance (from outside the space) to the entrant(s).

5.4.10 Order the evacuation of the confined space if:

A condition, which is not allowed on the permit, is observed.

The entrant exhibits or reports unusual health symptoms.

A situation outside the space is detected which could endanger those working

inside the space (such as activation of an emergency evacuation alarm)

An uncontrolled hazard is detected inside the space.

The Attendant cannot effectively and safely perform all required duties.

5.5 Entry Supervisor(s)

5.5.1 Ensure that a written hazard assessment on the space to be entered has been

conducted.

5.5.2 Ensure that all workers, before entering or performing related work in a confined space,

are adequately trained.

5.5.3 Review the entry plans prior to all entries and as often as necessary to ensure that the

plans and permits remain accurate and adequate.

5.5.4 Ensure that no worker enters a confined space unless an adequate on-site rescue

procedure has been developed and is ready for immediate implementation.

5.5.5 Ensure a coordination document is prepared by the lead employer where workers of

more than one employer (e.g. contractors) will perform work in the same confined space

or related work with respect to the same confined space. A copy of the coordination

document must be provided to each employer involved in the entry and to the Joint

Health and Safety Committee (JHSC) or Health and Safety Representative of each

employer involved in the entry. (Refer to Appendix D for a sample Coordination

Document).

5.5.6 Ensure that every worker who enters a confined space or conducts related work follows

the plan.

5.5.7 Ensure that the appropriate acceptable atmospheric level is noted on the permit to

facilitate proper interpretation of air testing results.

5.5.8 Sign the permit to verify that the requirements of the relevant plan have been met.

5.5.9 Ensure that air testing is conducted with calibrated instruments that are in good working

order and are appropriate for the hazards identified in the relevant assessment.

5.6 Entrant(s)

5.6.1 Review the Hazard Assessment and Confined Space Entry Permit/Plan prior to entering

and performing work in a confined space.

5.6.2 Implement the precautions identified on the Confined Space Entry Permit/Plan prior to

entry. This includes safety equipment and rescue protocols outlined in the entry specific

Plan.

5.6.3 Sign the permit to confirm they understand the hazards and the control equipment

needed to safely complete work in the confined space.

5.6.4 Properly wear all required personal protective equipment and utilize continuous air

monitoring unless noted otherwise in the entry specific guidelines and Confined Space

Entry Permit/Plan.

5.6.5 Maintain communication with the Attendant as necessary to enable the Attendant to

monitor the status of the Entrant(s) and enable the Attendant to alert the Entrant(s) of the

need to evacuate the space.

5.6.6 Alert the Attendant whenever:

The Entrant experiences any unusual health symptoms.

The Entrant detects a condition that is not allowed by the permit.

A gas monitor/evacuation alarm is activated.

5.6.7 Inform the Entry Supervisor, Mohawk College confined space contractor contact person

or Occupational Health and Safety Department of any problems encountered with the

confined space program and permit system.

6. SPECIFIC PROGRAM ELEMENTS

6.1 Identifying Confined Spaces/Restricted Spaces and Initial Assessment

6.1.1 A survey of the workplace shall be completed by a qualified person to identify all potential

confined spaces. Identification of spaces was conducted at Mohawk’s Fennell and

Stoney Creek campuses by a qualified person from Pinchin Ltd.

6.1.2 Each potential confined space shall be included in a Confined Space Inventory (see

Appendix A) and identified as a confined space or restricted space.

6.1.3 Hazard Assessments shall be conducted by a person with adequate knowledge, training,

and experience for all potential confined spaces. This may be done initially (typically in

conjunction with the identification) or prior to the first entry. Initial assessments were

conducted at Mohawk’s Fennell and Stoney Creek campuses by a qualified person from

Pinchin Ltd., in consultation with the Occupational Health and Safety Consultant and

assistance from Facilities Management staff. These assessments are provided in

Appendix B.

6.1.4 The assessment of each confined space shall consider:

the hazards that may exist due to the design, construction, location or use of

contents of the space; and

the hazards that may develop while work is done inside the confined space (e.g.

welding, cleaning, etc.).

Additional or associated hazards include:

o Oxygen deficiency or enrichment

o Flammable, combustible or explosive atmospheres.

o Toxic atmospheres

o Engulfment

o Mechanical, electrical or hydraulic hazards

o Temperature extremes

o Noise

o Access/egress

6.1.5 The Hazard Assessment document (either a new one or the reviewed and verified

original) shall be signed and dated by the person conducting the assessment. Once

completed, the document shall be forwarded to the Occupational Health and Safety

Department.

6.1.6 If two or more confined spaces are of similar construction and present the same hazards,

their assessments may be recorded in a single document, but each confined space shall

be identified on the assessment document.

6.1.7 Each confined or restricted space shall be clearly identified.

6.1.8 A copy of the Hazard Assessment, and record of qualifications of the person who

conducted the assessment, shall be provided, on request, to the JHSC, H&S

Representative, or to any worker who performs work to which the hazard assessment

relates (if the workplace has no JHSC or H&S Representative).

6.2 Pre-Entry Hazard Assessment Review and Verification

6.2.1 Before each entry in to a confined space or restricted space, the Entry Supervisor shall

ensure that a written Hazard Assessment has been conducted on the space to be

entered.

6.2.2 The space specific Hazard Assessment (Appendix B) shall be reviewed by the Entry

Supervisor, prior to entry into the space, to verify whether there are any changes to the

hazards identified. This will verify the existing Hazard Assessment is valid for the work to

be performed or determine the need for a new assessment to be conducted (refer to pg.

3 of the specific Hazard Assessment).

6.2.3 If an assessment has not been previously conducted on the space, one shall be

conducted by a qualified person prior to entry into the confined space.

6.3 Entry Plans and Permits

6.3.1 Before any worker enters a confined space, Facilities Management shall ensure that an

entry plan (including necessary procedures) and permit have been developed and

implemented by a competent person to control the actual and potential hazards identified

in the associated Hazard Assessment.

6.3.2 Entry plans and permits shall be reviewed by Occupational Health and Safety as often as

necessary, to ensure they remain accurate and adequate.

6.3.3 Entry Plans and Permits may be combined into one document. A general Plan/Permit for

the identified “pits” (see 6.3.4 below) includes all information required by O. Reg. 632/05

and is provided in Appendix C.

6.3.4 If two or more confined spaces are of similar construction and present the same hazards,

one plan/permit can be developed for the group of spaces identified on the Hazard

Assessment document.

6.3.5 A separate entry permit is required before each confined space entry.

6.3.6 The Entry Supervisor shall ensure that every worker who enters a confined space or

conducts related work follows the plan.

6.3.7 The Entry Supervisor shall ensure that the appropriate acceptable atmospheric levels are

noted on the permit to facilitate proper interpretation of air testing results.

6.3.8 The Entry Supervisor shall sign the permit to verify the requirements of the relevant plan

have been met.

6.3.9 The permit shall be closed at the end of each shift, unless a competent person verifies (in

writing on the entry permit and at the beginning of the next shift) that the permit and work

conditions continue to comply with the requirements of the relevant plan.

6.4 On-Site Rescue Procedures and Equipment

6.4.1 The Entry Supervisor shall ensure that no worker enters a confined space unless an

adequate written on-site rescue procedure (including equipment) has been developed

and is ready for immediate implementation, in accordance with the relevant plan. A non-

entry Rescue Plan/Procedure for the identified “pits” is provided in Appendix D.

6.4.2 Rescue equipment shall be inspected by a person with adequate knowledge, training and

experience as often as is necessary to ensure it is in good working order. A written

record of the inspection shall be maintained at the site where it is normally stored and on

the applicable permit.

6.4.3 The Entry Supervisor shall ensure that no worker enters a confined space until an

adequate number of persons trained in the rescue procedures are available on-site, for

immediate implementation of the rescue procedures. The person(s) must be trained in:

6.4.3.1 The on-site rescue procedures,

6.4.3.2 First aid and cardio-pulmonary resuscitation, and

6.4.3.3 The use of the rescue equipment required by the relevant plan.

6.5 Preventing Unauthorized Entry

6.5.1 If there is the possibility of unauthorized entry into a confined space, the Entry Supervisor

shall ensure that each entrance to the space shall be:

6.5.1.1 adequately secured against unauthorized entry; or

6.5.1.2 has been provided with adequate barricades, adequate warning signs regarding

unauthorized entry, or both.

6.6 Atmospheric Testing

6.6.1 Atmospheric testing must be performed before anyone enters the space, and at adequate

intervals, during each confined space entry to ensure that acceptable atmospheric

contaminant levels are maintained in the confined space.

6.6.2 Atmospheric testing shall be conducted, in accordance with the relevant plan, by a

person with adequate knowledge, training and experience.

6.6.3 The Entry Supervisor shall ensure that the testing is done in a manner that does not

endanger the tester.

6.6.4 Re-testing of a space during a permit period shall also be performed, before any worker

re-enters the space, anytime the space has been both unoccupied and unattended (e.g.

lunch breaks).

6.6.5 The Entry Supervisor shall ensure that air testing is conducted with adequately

maintained and calibrated instruments that are in good working order, and are

appropriate for the hazards identified in the relevant assessment.

6.6.6 Air testing must be performed for the following gases, in the following sequence and

recorded on the permit.

Oxygen content (must be between 19.5% - 23%)

Flammable and combustible gases and vapours:

o Less than 25% for inspection work with no source of ignition.

o Less than 10% for cold work.

o Less than 5% LEL and less than 23% Oxygen for hot work.

Other toxic materials:

Hydrogen sulphide (H2S: must be below 10 ppm)

6.6.7 The results of all sampling shall be recorded on the Confined Space Entry Plan/Permit.

6.7 Training

6.7.1 Occupational Health and Safety Department shall ensure that all Mohawk College

employees that authorize or contract services for work in confined spaces receive

adequate General training. Training will include, but not necessarily be limited to:

Regulatory requirements and the contents of this Program

Recognition of hazards associated with confined spaces

6.7.2 If Mohawk College employees are required to enter a confined space or conduct related

work with respect to a confined space, they shall receive the above general training and:

Plan-specific training, in accordance with the relevant plan, to work safely and

properly

6.7.3 General and Plan-Specific Training may be combined into one training program.

6.7.4 The training program(s) shall be developed in consultation with the JHSC as applicable.

6.7.5 Required training records shall be retained by Occupational Health and Safety.

6.8 Multiple Employer Entries and Contractors Performing Confined Space Work

6.8.1 Mohawk College must ensure contractors work in accordance with O. Reg. 632/05 and

this Confined Space Entry program, or the contractor’s program if it provides equal or

greater protection.

6.8.2 If employees from more than one employer participate in a confined space entry (e.g.

enter the space or conduct related work) the employer who takes on the role of Lead

Employer must prepare and completes a Coordination Document. A Coordination

Document template is provided in Appendix E.

6.8.3 Copies of the Coordination Document must be provided to each employer involved in the

planned entry and the JHSC or H&S Representative of each employer involved in the

entry.

On-Site Confined Space Rescue Procedures for Below Grade Pits (e.g. Sump, Storm, Sanitary, Cable, Hydro, Water Valve, Catch Basins)

Specific Space Identifier: _______________________

Before any worker enters a confined space, adequate written on-site rescue procedures that apply to the confined space must be developed and ready for immediate implementation by an on-site rescue team. This non-entry rescue procedure for below grade pits is to be used in conjunction with the Plan/Permit for this space. The Hazard Assessment for this space must also be reviewed with the rescue team.

1. Rescue Team 1.1 A minimum two-person rescue team shall be available for immediate implementation of this

procedure prior to entry in this space being permitted. They must be designated as Rescue Member 1 and Rescue Member 2. Both members shall remain in an area close to the space, for the duration of work, in order to be prepared for immediate implementation of rescue. For this space, the rescue members will be stationed at the top of the pit/chamber at the opening.

1.2 The rescue team members must be trained on the specific procedures for this confined space; the use of relevant rescue equipment and methods; and in first aid and cardiopulmonary resuscitation (CPR). The specific responsibilities of the rescue team members shall be communicated and reviewed in a pre-entry meeting.

1.3 The members shall have a means of receiving communication from the Attendant (see below) and

shall be immediately ready to perform rescue (see below), should he/she be summoned by the Attendant. Note: The confined space “Attendant” must remain in place outside and near the entrance to the

confined space. The Attendant is not technically part of the “rescue team”. However, as long as it does not affect their Attendant duties, the Attendant (if adequately trained in rescue procedures) may assist during the rescue.

2 Methods of Communication

2.1 Communication methods must be established between the Entrant(s) and the Attendant (E A),

and between the Attendant and the Rescue team (A R). 2.2 If verbal communication is not sufficient for the space, two-way radios and cell phones must be used

as appropriate. 2.3 The Attendant or any member of the Rescue Team shall call Mohawk Security at 905.575.2003

and/or 911 directly, to initiate emergency services prior to, or concurrently to proceeding with the rescue.

3 Evacuation and Rescue Procedure

3.1 The Entrant shall climb out of the space immediately if they experience any unusual health symptoms; if the gas monitor alarm is activated; or if instructed to do so by the Attendant.

3.2 If the Entrant is unable to exit the space by “self-rescue”, the Rescue Team Members will be summoned by the Attendant to perform vertical, non-entry rescue/retrieval. The Attendant or any member of the Rescue Team shall call Mohawk Security at 905.575.2003 and/or 911 directly, to initiate emergency services prior to, or concurrently to proceeding with the rescue.

3.3 Rescue Member 1 shall turn the winch (connected to the tripod or davit system) to wind in the lifeline

connected to the full body harness of the Entrant.

3.4 Rescue Member 2 (or person not turning the winch) shall assist by guiding or helping to manoeuvre the Entrant and lifeline.

3.5 Once the Entrant is pulled to the opening of the space, he/she shall be lifted out/removed from the space and First Aid / CPR will be performed if required.

4.0 Rescue Equipment 4.1 Rescue equipment must be readily available and appropriate for entry into the confined space.
