PROJECT NARRATIVE UPC 105047 PM2R-060-F14,P401 IM-PM02(377) and UPC 105301 0081-060-F14,P401 STP-081-2(294) The purpose of this project is to Plant Mix a portion on Interstate 81NB in Montgomery County. The project will include mainline resurfacing, pavement line markings, installation of pavement markers and guardrail improvements where applicable. Roadway Safety Assessments have been completed for this project. This project will improve subgrade drainage and yielding subgrade issues and provide 15 to 20 years design life. This project is eligible for federal funding and being on the Interstate system, the inclusion of dedicated pedestrian or bicycle facilities is not applicable. This project has been reviewed by the Environmental Division to determine applicable permits required. All work will be performed within existing pavement structure and no utilities will be impacted. Based on a 12/10/13 advertisement date a fixed completion date for this work has been established as 06/30/15.

Salem District Interstate Plant Mix General Notes Salem District Notes: Tonnage is included in all contract items for wedges, connections, crossovers, and turn lanes as shown. Routes to be restricted during certain times are indicated within the contract line item. Routes where work is to be performed only during certain dates are indicated within the contract line item. This restriction applies to all contract work within that line item. All Routes: No asphalt course shall be opened to traffic or have subsequent pavement overlays placed when the temperature of the pavement is above 150 degrees Fahrenheit. All Counties: The contractor shall remove the raised snow plowable pavement markers prior to paving. The cost of removing raised snow plowable pavement markers shall be included in the bid price of other items. Where raised snow plowable pavement markers are removed, the contractor shall patch these areas at the time of removal. The cost of this patching shall be included in the bid price of other items. The contractor will be responsible for the disposal of the removed markers. All Counties: Any gouged areas resulting in excessive depth from planing operations shall be scratched and repaired at the contractor’s expense. All Counties: All Interstate Routes will be required to have non-polishing aggregate in all surface mixes, regardless of traffic count. All Counties: When milling is required in areas containing manhole covers and other obstructions, the contractor will be required to remove pavement adjacent to same. Removal of pavement adjacent to same will be included in the milling price. Milling and pavement inlay shall be required for all overlay locations with overhead structures in order to prevent decreasing the clearance between the structure and the roadway in accordance with the ACOT-1 standard in the 2008 VDOT Road and Bridge Standards. The contractor is responsible for the clean-up of all milling/excavating debris to assure that no build-up of the millings/excavating is allowed to cause drainage problems along the edge of the pavement.

Where the 1.5” overlay of the right shoulder is called for, the pavement drop-off at the face of guardrail shall not be located any closer than 2’-0” in front of the face of guardrail. The drop-off shall be limited to a range of 1½” in height with a 4:1 or flatter wedge to minimize the effect of the drop-off on a vehicle. Any drop-off that should happen to fall within 2’-0” of the face of guardrail shall be corrected by the Contractor with a method suitable to the VDOT engineer and shall be at no additional cost to the Department. The " PHASE 1: Inside Shoulder Patching for Maintenance of Traffic" located elsewhere in this contract is to note the details for the spot patching in left side shoulder on I-81 NB county Milepost 15.98 to 20.21 in Montgomery Co. The "Patching Detail" located elsewhere in this contract is to note the locations and details for the patching on I-81 NB county Milepost 15.98 to 20.21 in Montgomery Co. If excavation is used in lieu of milling for pavement demolition of the asphalt layers, the asphalt shall be sawcut around the perimeter of the excavation and the price for sawcutting shall be included in the price for pavement demolition (24430 – Demolition of Pavement). The bottom of the patches shall be graded as shown in the "Patching Detail" to properly drain to UD-7s. The cost for grading shall be included in the base bid line item for "24430 – Demolition of Pavement". After placement of the No. 1 Aggregate (10065 – AGGR. MATL. NO. 1), a method of compaction consisting of at least three passes with a steel drum vibratory roller weighing no less than 10 tons shall be used. The Milling and Overlay Detail located elsewhere in this contract is to note the detail of milling and overlay on I-81 NB county Milepost 15.98 to 20.21 in Montgomery Co. Intermediate layer shall be placed (over/through the patches) as soon as possible following completion of the patching. Base Asphalt (BM-25.0) shall be subject to traffic for no more than 14 calendar days. UD-7s are required on the right side only and shall be placed outside of the rumble strips approximately 2 feet from the right edge line. Outlets shall be supplied approximately every 200 feet and in all sag locations. The Guardrail Schedule located elsewhere in this contract is to note the locations for the installation and removal of guardrail items on I-81 NB county Milepost 15.98 to 20.21 in Montgomery Co.

Detour signs on northbound Route 11/eastbound Route 460 will be left in place after construction is complete and will become property of the Department. All remaining detour signs will be removed with the project. The construction sequence for I-81 NB county Milepost 15.98 to 20.21 in Montgomery Co. shall be as follows unless approved otherwise by the Engineer:

Phase 1: Spot patching in left side shoulder adjacent to outside lane patches Phase 2: Full depth patching in outside lane Phase 3: Mill outside lane and place back SM-19.0D asphalt Phase 4: Install underdrains Phase 5: Mill outside shoulder from MM 123 to MM 124.6 and place back IM-19.0D asphalt Phase 6: Mill inside lane and place back IM-19.0D asphalt Phase 7: Install and replace guardrail Phase 8: Place asphalt surface course and permanent pavement markings Phase 9: Install rumble strips All Counties: If the contractor chooses to use slag as an alternative to natural aggregate, adjustments shall be made to compensate for the difference in specific gravity of the slag compared to the natural aggregate. The pay total for the roadway section where slag is being used as an alternate material shall be based on the tonnage of a mix using 100% natural aggregate at the plant where the mix is being produced. The following will be used to determine the Adjusted Pay Total: A = Line Item Tonnage B = % Asphalt (decimal form) C = Specific Gravity of Slag Mix D = Specific Gravity of Natural Aggregate Mix E = Design Lbs/Sq. Yd. (Natural Agg. Mix) F = Tons of Slag Produced Estimated Slag Mix Tonnage, G = [A-(A x B)] x (C/D) + (A x B) Slag Mix Lbs./Sq. Yd. = E x (G/A) Adjusted Pay Total = F x (A/G) All overlay locations containing bridge structures, shall require a smooth transition for entering and exiting the structure and shall be in compliance with Supplemental Section 515 of the contract documents..

A Truck Mounted Attenuator (TMA) shall be used during installation of pavement markings, pavement markers or, loop detectors, as shown in the current Virginia Work Area Protection Manual or current MUTCD or as directed by the Engineer. All traffic control devices and signs necessary for maintenance of traffic shall be furnished, installed, maintained, and removed by the Contractor. All traffic control device locations shall be marked by the Contractor and reviewed by the Engineer prior to installation. All signs installed shall be in accordance with both the current Virginia Work Area Protection Manual and the current MUTCD for Streets and Highways. Construction and permanent pavement marking shall be installed in accordance with the manufactures practices and the current VDOT Road and Bridge Specifications. All existing pavement marking shall be replaced as the existing is currently marked. Prior to installing Raised Snow Plowable Pavement Markers, the pavement surface shall be dry and the minimum temperature shall be no less than stated in the Specifications. The clear zone is to be free of stored materials and parked equipment. Sight distance at interchanges and entrances shall not be impacted by parked construction vehicles or equipment. There will be no work allowed on Interstate 77 in Carroll County between the preceding Friday and the Monday following the NASCAR races at Martinsville Speedway and Bristol Motor Speedway Holidays: In addition to the aforementioned restrictions in the contract, the following will apply to all line items unless waived in writing by the Engineer. All traffic lanes shall be open to traffic per the specifications listed Section 108.02—Limitation of Operations of the Road and Bridge Specifications or Volume 2 of the contract. Note: The VDOT Engineer reserves the right to monitor traffic conditions impacted by the work, and the Engineer shall have the authority to impose additional restrictions for other holidays or special events as determined necessary in the event that safety or other conditions warrant, and to change and/or alter the work hour time frame(s) accordingly.

GENERAL INFORMATION SHEET

The information contained in the SWPPP General Information sheets is intended to comply with the requirements of the VSMP General Permit For Discharges Of Stormwater From Construction Activities (the VSMP Construction Permit) issued July 1, 2009 and VDOT’s approved Annual ESC and SWM Standards and Specifications. The SWPPP General Information sheets are to be completed and included in the construction plan set (or other such documents) for land disturbance activities that disturb an area equal to or greater than 10,000 square feet, or equal to or greater than 2,500 square feet in the area defined as Tidewater, Virginia in the Virginia Chesapeake Bay Preservation Act. Information shown on the SWPPP General Information sheets is to be updated/revised by the VDOT RLD as necessary in order to reflect changes that may occur during the construction phase of the land disturbing activity. The updated/revised sheets shall be maintained with the designated record set of plans for the land disturbance activity. * Denotes information that is to be provided/completed by the VDOT RLD. ** Denotes information that is to be provided/completed by the contractor. Note: Effective July 1, 2013, responsibility for the VSMP General Permit for Discharges of Stormwater from Construction Activities (the VSMP Construction Permit) was transferred from the DCR to the DEQ. SECTION I GENERAL INFORMATION

1. Activity Description - F14 Bonus OA Montgomery County Interstate Plant Mix, patching , and UD installation

2. This land disturbance activity site is located in Montgomery County and approximately 2.76 acres (0.405 acres Construction and 2.355 acres Maintenance) will be disturbed by excavation, grading or other construction activities.

3. This proposed Construction activity disturbs less than one acre (less than 2,500 square feet in a designated Chesapeake Bay Preservation Area) and is exempt from the VSMP Regulations and coverage under the VSMP General Permit for Discharges of Stormwater from Construction Activities (the VSMP Construction Permit) as issued by the DEQ. This proposed Maintenance activity is exempt from the VSMP Regulations and coverage under the VSMP General Permit For Discharges Of Stormwater From Construction Activities (the VSMP Construction Permit) as issued by the DEQ because it is considered a routine maintenance activity (i.e., the proposed activity is intended to maintain the original line and grade, hydraulic capacity or original construction of the project or involves the paving of an existing roadway with a compacted or impervious surface and the reestablishment of existing associated ditches and shoulders).

4. The ESC and SWM plans (where applicable) for this land disturbance activity has been developed in accordance with VDOT's Annual Erosion and Sediment Control and Stormwater Management Standards and Specifications as approved by the DEQ.

5. Duane Mann – Southern Area Construction Engineer is designated as the RLD for this land disturbance activity.

11. All engineering calculations supporting the design of erosion and sediment control measures are contained in the project drainage file located in the VDOT Salem District Hydraulics Section and will be made available for review upon request during normal working business hours.

12. The temporary erosion and siltation control items shown on the ESC Plan for this land disturbing activity are intended to provide a general plan for controlling erosion and sediment within the project limits. The ESC Plan is based on field conditions at the time of plan development and an assumed sequence of construction for the project. The contractor, in conjunction with the VDOT Project Engineer and/or ESC Inspector, shall adjust the location, quantity and type of erosion and sediment control items required based on the actual field conditions encountered at the time of construction and the actual scheduling and sequencing of the construction activities. Significant changes to the proposed ESC Plan (e.g., those that require an engineering analysis) shall be submitted to the applicable District Hydraulics Engineer for review and approval. Any changes to the proposed ESC Plan must be noted on the designated record set of plans which shall be retained on the project site and made available upon request during normal working business hours.

13. The areas beyond the project's construction area are to be protected from siltation. Perimeter controls such as filter barrier, silt fence, diversion dikes, turbidity curtains, etc. shall be installed prior to any grubbing operations or other earth moving activities.

14. Temporary earthen structures such as dikes and berms are to be stabilized immediately upon installation. Stabilization may include temporary or permanent seeding, riprap, aggregate, sod, mulching, and/or soil stabilization blankets and matting in conjunction with seeding.

15. All channel relocations are to be constructed during the earliest stage of

construction and shall be constructed in accordance with all applicable permit requirements and shall be constructed in the dry wherever possible. Stabilization or vegetation shall be established before flow is redirected through the constructed area as directed by the Engineer.

SECTION III SWPPP

1. All documents related to the SWPPP for this land disturbance activity shall be maintained at the activity site and shall be readily available for review upon request during normal working business hours. Such documents include, but are not limited to, the construction plans (or other such documents), the ESC Plan, the post construction SWM Plan (if applicable), the VDOT R&B Standards and Specifications, Supplemental Specifications, Special Provisions and Special Provision Copied Notes. Documents related to stormwater pollution prevention which are not a part of those documents referenced above, such as a copies of the VSMP Construction Permit coverage letter (when applicable) and the DCR General Permit For Discharges Of Stormwater From Construction Activities (when applicable) and those required to be developed by the contractor for stormwater pollution prevention associated with any support facilities being included in the VSMP Construction Permit coverage for this land disturbance activity are to be maintained at the activity site with the other SWPPP documents. Where no facilities are available at the activity site to maintain the SWPPP documents, they are to be kept by or with the designated RLD at a location convenient to the activity site where they would be made available for review upon request during normal business working hours.

2. Other than construction at the activity site, there are no discharges associated with industrial activity (e.g., from dedicated asphalt plants or dedicated concrete plants) produced by this land disturbance activity or (where applicable) included in the VSMP Construction Permit coverage for this land disturbance activity.

**3. Documents identifying the locations of support facilities (e.g., waste or borrow areas, material or equipment storage areas etc.) associated with or (where applicable) included in the VSMP Construction Permit coverage for this land disturbance activity shall be provided by the contractor in accordance with Section 107.16 of the VDOT R&B Specifications and will be maintained with the other SWPPP documents for this land disturbance activity.

A description of all pollution control measures that will be implemented as a part of this construction activity to control pollutants in stormwater discharges are identified and described in the contractor supplied documents, the construction plan set (or other such documents) and applicable sections of the VDOT R&B

6. Requirements for the prevention of the discharge of solid materials, including building materials, garbage, and debris, to surface waters of the state, except as authorized by a Clean Water Act 404 permit, are contained in Sections 106.04, 107.02 and 107.16 of the VDOT R&B Specifications.

7. Requirements for compliance with applicable state or local waste disposal, sanitary sewer or septic system regulations are contained in Sections 106.04 and 107.18 of the VDOT R&B Specifications.

**8. A description of construction and waste materials expected to be stored on-site, or at off-site support facilities included in the VSMP Construction Permit coverage for this land disturbance activity, and a description of controls to reduce pollutants from these materials, including storage practices to minimize exposure of the materials to stormwater and practices for spill prevention and response, shall be supplied by the contractor and maintained with the other SWPPP documents for this land disturbance activity.

**9. For any support areas that will be included in the VSMP Construction Permit coverage for this land disturbance activity, a description of potential pollutant sources from such areas and a description of controls and measures that will be implemented at those sites to minimize pollutant discharges shall be supplied by the contractor and maintained with the other SWPPP documents for this land disturbance activity.

*10. By completing and submitting the SWPPP Certification form LD-445E, the RLD, or his authorized representative, certifies that all documents identified herein to be supplied by the contractor will be reviewed, approved and included with the other SWPPP documents for this land disturbance activity prior to start of work in those areas identified by such information.

The name of the VDOT individual(s) responsible for the inspection of the erosion and sediment control measures on the land disturbance activity is identified on the LD-445E form which will be maintained with the other SWPPP documents for this land disturbance activity (Note: Individual(s) shall be certified through the DCR ESC Inspector Certification Program).

12. For those land disturbing activities requiring coverage under the VSMP General

Permit for the Discharge of Stormwater from Construction Activities, the SWPPP shall be made available for review upon the request of the DCR, the EPA, local government officials or the operator of a municipal separate storm sewer system (MS4) receiving discharge from the construction site.

For those land disturbing activities requiring coverage under the VSMP General Permit For The Discharge Of Stormwater From Construction Activities, the VDOT RLD shall post, or have posted, a copy of the General Permit coverage letter and a copy of a completed LD-445A form, noting the name and contact information for the VDOT person responsible for the land disturbing activity and its SWPPP, outside the project’s construction office along with other Federal and State mandated information. Where there is no construction office (e.g., a maintenance activity), the permit coverage letter and the LD-445A form are to be maintained with the other SWPPP documents for the land disturbing activity.

14. For those land disturbing activities requiring coverage under the VSMP General Permit for the Discharge of Stormwater from Construction Activities on or after July 1, 2009 that have not been previously covered by a VSMP permit, the SWPPP shall be made available for review by the public upon request. Such reviews shall be at a time and publicly accessible location convenient to the VDOT and shall be scheduled during normal business hours and no less than once a month.

SECTION IV POST CONSTRUCTION STORMWATER MANAGEMENT

1. This land disturbance activity does not require permanent water quality SWM facilities because the proposed construction portion is less than one acre of disturbance and the remainder is considered routine maintenance.

2. The permanent SWM facilities proposed to meet the water quality/quantity requirements for this land disturbance activity are listed in Section VI.

**5. The name of the individual(s) or contractor(s) responsible for the installation and

maintenance of the pollution control measures shall be supplied by the contractor and maintained with the other SWPPP documents for this land disturbance activity.

6. Requirements for the prevention of the discharge of solid materials, including building materials, garbage, and debris, to surface waters of the state, except as authorized by a Clean Water Act 404 permit, are contained in Sections 106.04, 107.02 and 107.16 of the VDOT R&B Specifications.

7. Requirements for compliance with applicable state or local waste disposal, sanitary sewer or septic system regulations are contained in Sections 106.04 and 107.18 of the VDOT R&B Specifications.

**8. A description of construction and waste materials expected to be stored on-site, or at off-site support facilities included in the VSMP Construction Permit coverage for this land disturbance activity, and a description of controls to reduce pollutants from these materials, including storage practices to minimize exposure of the materials to stormwater and practices for spill prevention and response, shall be supplied by the contractor and maintained with the other SWPPP documents for this land disturbance activity.

**9. For any support areas that will be included in the VSMP Construction Permit coverage for this land disturbance activity, a description of potential pollutant sources from such areas and a description of controls and measures that will be implemented at those sites to minimize pollutant discharges shall be supplied by the contractor and maintained with the other SWPPP documents for this land disturbance activity.

*10. By completing and submitting the SWPPP Certification form LD-445E, the RLD, or his authorized representative, certifies that all documents identified herein to be supplied by the contractor will be reviewed, approved and included with the other SWPPP documents for this land disturbance activity prior to start of work in those areas identified by such information.

*11. The name of the VDOT individual(s) responsible for the inspection of the erosion and sediment control measures on the land disturbance activity is identified on the LD-445E form which will be maintained with the other SWPPP documents for this land disturbance activity (Note: Individual(s) shall be certified through the DCR ESC Inspector Certification Program).

12. For those land disturbing activities requiring coverage under the VSMP General

Permit For The Discharge Of Stormwater From Construction Activities, the SWPPP shall be made available for review upon the request of the DCR, the EPA, local government officials or the operator of a municipal separate storm sewer system (MS4) receiving discharge from the construction site.

*13. For those land disturbing activities requiring coverage under the VSMP General Permit For The Discharge Of Stormwater From Construction Activities, the VDOT RLD shall post, or have posted, a copy of the General Permit coverage letter and a copy of a completed LD-445A form, noting the name and contact information for the VDOT person responsible for the land disturbing activity and its SWPPP, outside the project’s construction office along with other Federal and State mandated information. Where there is no construction office (e.g., a maintenance activity), the permit coverage letter and the LD-445A form are to be maintained with the other SWPPP documents for the land disturbing activity.

14. For those land disturbing activities requiring coverage under the VSMP General Permit For The Discharge Of Stormwater From Construction Activities on or after July 1, 2009 that have not been previously covered by a VSMP permit, the SWPPP shall be made available for review by the public upon request. Such reviews shall be at a time and publicly accessible location convenient to the VDOT and shall be scheduled during normal business hours and no less than once a month.

SECTION IV POST CONSTRUCTION STORMWATER MANAGEMENT

1. This land disturbance activity does not require permanent water quality SWM

facilities because the total project disturbance is less than 1 acre.

2. The permanent SWM facilities proposed to meet the water quality/quantity requirements for this land disturbance activity are listed in Section VI.

3. A description of all post-construction stormwater management measures that will

be installed during the construction process to control pollutants in stormwater discharges after construction operations have been completed is included in the construction plan set (or other such documents) for this land disturbance activity.

INTRODUCTION

Improvements will be performed in Montgomery County. Interstate 81 (I-81) is a Rural Interstate. The annual average daily traffic is 23,467 vehicles with 24% trucks. The purpose of this project is to plant mix a portion of Interstate 81 NB in Montgomery County. The project will include mainline resurfacing, pavement line markings, installation of pavement markers and guardrail improvements where applicable. The project will improve subgrade drainage and yielding subgrade issues. Phase 1 will involve spot patching in left side shoulder adjacent to outside lane patches. Phase 2 will involve full depth patching of the outside lane. Phase 3 will mill the outside lane and place back SM-19.0D asphalt. Phase 4 will install underdrains. Phase 5 will mill outside shoulder from MM 123 to MM 124.6 and place back IM-19.0D asphalt. Phase 6 will mill inside lane and place back IM-19.0D asphalt. Phase 7 will install and replace guardrail. Phase 8 will place asphalt surface course and permanent pavement markings. Phase 9 will install rumble strips.

Allowable Work Hours:

Single lane closures will be allowed Sunday thru Thursday from 10:00 pm in the evening until 6:00 am the following morning during all phases except Phase 2.

Phase 2 hours will be as indicated below:

Sunday night from 7:00 PM until Monday at 6:00 AM of the following

week with the work continuously prosecuted in the northbound lanes. From 10:00 PM until 6:00 AM daily, all traffic will be detoured, from the collector distributer (CD) road at Exit 118C onto the detour and return to the Interstate at Exit 132.

At all other times, from Sunday thru Monday, traffic on the collector distributer (CD) road will be split, the left lane returning to the Interstate and the right lane staying on the detour. Trucks, 11 feet or less, will be signed to stay on the Interstate and cars will be signed to use the detour. Wide loads, exceeding 11 feet in width, will be signed to use the detour. PUBLIC COMMUNICATIONS PLAN Project Description: See above. Traffic Impacts: During construction, northbound traffic on I-81 will have only one travel lane available during certain work activities and be fully detoured during other work activities. Goals: Interstate 81 carries large volumes of traffic daily. Salem District Public Affairs will issue a traffic alert and post online in advance of the work. Also, notice of the work will be included in the weekly Roadwatch report that is issued each Friday and posted

TRANSPORTATION OPERATIONS PLAN This plan is not required on this project. However, a contact list of local emergency response agencies must be kept and maintained throughout the project life cycle. Traffic Backup Notification Emergency contact for this project shall be 911. The Traffic Operations Center (540-375-0170) shall be notified of traffic backups, related to the work, by the project staff. The TOC will utilize available systems (cameras, sensors, etc.) to monitor the work area and all adjacent areas. The TOC will make entries into systems that feed this information into 511. The TOC will utilize assets such as variable message signs to alert motorists of lane closures and other incidents that may impact travel. PCMS boards shall be equipped with CDMA capabilities. The TOC will also make notifications to department staff. The staff notifications shall include but not be limited to the Duty Officer, Area Construction Engineer, Project Maintenance of Traffic Coordinator, Project Manager, Resident Administrator, District Work Zone Safety Coordinator, Regional Traffic Engineer, Regional Traffic Operations Manager, Regional Incident Management Coordinator, and Public Affairs Manager of any incidents and expected traffic delays. The TOC shall be responsible for intra-agency notifications to entities such as but not limited to VSP, Local 911, and other affected agencies. A review of all major incidents, as determined by the Regional Incident Management Coordinator, shall be accomplished within 48 hours of clearance of the incident. VDOT Project Staff, Regional Operations Staff, Contractor Staff, and emergency responders shall be represented at these meetings. The following is a list of Local Emergency contact agencies:
