Genova Lab Test Instructions for the GI Effects Stool test Easy Pay Insurance

What you should receive:

1. 1 lab kit2. 1 requisition form on the outside of the lab kit.3. Stool kit to collect the 3 day stool sample.4. Please avoid all antimicrobial herbs, probiotics, digestive enzymes and HCL (hydrochloric acid)

for 7 days before conducting the test. You may resume the supplements when the test iscompleted.

5. Pre-paid FedEx envelope to send the lab test back.6. Instruction on how to conduct the test and what supplements to avoid and for how long.

What you need to do to receive a discount on your Stool test:

1. You can find the lab requisition form already filed out online in the members area website.2. Fill out the lab requisition form, including credit card information and or insurance information.

If you have an eligible insurance, there is only a $114 fee for your test.3. All insurances are eligible for the above discount except the insurances mentioned below. In

other words, if your insurance is not mentioned below, you are eligible for the discount.• Aetna, Humana, Principal, Kaiser, Tuft, Blue Cross Blue Shield of North Carolina

ONLY, Blue Cross Blue Shield Federal Employee Program.4. If your insurance is not eligible, Dr Justin has special discounted cash prices available as well.

Please call the office for more information on this.5. Please fill out and replace the requisition form in the lab kit with the requisition form in the

current document. You will be charged significantly more money if the form in this documentisn’t used.

6. Complete the 3 day stool test and send the specimen back to the lab in the pre-paid FedExenvelope.

What you need to do next: 1. Dr Justin will go over your lab results with you as soon as they are in. We typically receive

results within 3-4 weeks. 2. Make sure you have an appointment scheduled with the Dr Justin to review the results.3. You will receive at least two separate bills on yellow paper from the lab within a few months

of completing the test. These bills are not to be paid and can be completely ignored as long as the $114 lab copay was paid above.
