1.0 Summary1.1 Introduction

The Contract ER420QP is for the provision of Qualified Person(Supervision)Services

for the main contract ER420 - which was awarded to ULT Corporation Pte., Ltd. in 15

October 2012 . ER420QP services commenced on 01st

Mach 2013 and due to complete

by 30th

Nov 2013, to be followed by a 12 months servicing of the contractors defect

liability period.

The ER420QP Contract reference is as follows:

1.2 Status of Main Contractor works

a) The Contractor has carried out the following works in May 2013:--

i. The Contractor carried out Earth Cutting and Soil Disposal

( To date 1,467 truck loads of soil disposal had been done).

ii. First Batch were Tested for Welded Wire Mesh

( Size B8 and B6, to test B10 subsequently)

iii. Soil Samples were tested.

iv. Geo-Technical Instrumentations were installed on site:

New Road Link Slope Area 1 - Inclinometer

1 - Piezometer

5 - Settlement Markers

1,500mm Wide Drain 2 - Inclinometer

2 - Piezometer

6 - Settlement Markers

1 - Vibration Meter

v. Water Absorption Test done for 900mm wide Drain.

vi. The Contractor casted the slab and wall of 900 mm wide RC Drain at Upper

Changi Road MCO2 West Bound which is approximately 45 Linear meters.

vii. The Contractor casted the slab and wall of 900 mm wide RC Drain at New Road

Road MCO1 North Bound which is approximately 65 Linear meters.

viii.Concrete cube samples were tested for this month of May for Grade 30

Concrete and Grade 40 N Concrete.

ix. Sheet Piling works started on 26 April 2013 for 1,500mm Wide Drain

( Area 1 - CH60 to CH96).

x. Excavation works started at 1,500mm wide RCU Drain at Sheet Piling Area

( Area 1 - CH60 to CH96).

xi. 1,400mm wide x 1380mm deep RCBC Base Foundation started casting on

25 May 2013.

1.3 QP(S) Worksa) QP(S) -C&V Engineering PTE. LTD. Commence QP service on 1st March 2013.

b) QP(S) supervised /observed/reported on the following : --

i. Inspection for Casting of 900mm wide RCU Drain.

ii. Inspection for Casting of 1400mm wide RCBC Drain.

iii. Delivery of 1,500mm wide PC RCU Drain.iv. Delivery of 1,200mm wide PC RCU Drain.

v. Inspection for Earth Works.

vi. Inspection for Sheet Piling Works.

vii.Witness Geotechnical Instrumentation Installation and Monitoring.

viii.Review Method of Statements/Test Reports submitted by Contractor.

ix. Safety and Quality of Works within the project site. (Prevention of

Mosquito Breeding on site).

2.0 Scope of Main Contract and QP(S) Works

2.1 Main Contract Works

The Main Contract ER420 had been awarded to ULT Corporation Pte Ltd in January 2013.

The Main Contract works consists of :

Construction of New Road Between Upper Changi Road and Upper Changi RoadEast.

Construction of New Junctions in the Upper Changi Road and Upper Changi RoadEast.

Construction of Rigid Pavements at the Junctions.

Road Widening a Part of Upper Changi Road.

Milling and Patching works in Upper Changi Road and Upper Changi Road East.

Construction of RCU Drains, RC Box Culverts and RC Sumps.

Construction of Two Bus Bays and Bus Shelters Slabs.

Installation of railings.

Miscellaneous works.

All temporary and monitoring works related to the projects.

2.2 QP(S) Works

The Contract ER420QP is for the provision of Qualified Person(Supervision) Services for

the main contract ER420 - The scope of works is shown in Appendix 1A - QP(S) scope

of works.

QP(S) services include the provision of Professional Engineering Supervision services as

required under the Authoritys requirements and /or the requirements of other authorities

and agencies and to comprise:

Provide qualified site supervision staff as required under the Building Control Act tosupport the QP(S);

Ensure that the construction works are carried out in a safe manner in

accordance with best construction practices; and

Any other professional engineering services as set out in the scope of Services.

APPENDIX 1A - QP(S) Scope of Works

QP(S) is to provide the required Qualified Person (Supervision) [QP(S)] to

a) provide professional engineering supervision services as required under the Authoritysrequirements and /or the requirements of other authorities and agencies;

b) provide qualified site supervision staff as required under the Building Control Act to

support the QP(S);

c) ensure that the construction works is carried out in a safe manner in accordance with

best construction practices; and

d) Any other professional engineering services as set out in the scope of Services.

QP(S)s primarily role is to ensure safety and quality construction by the main contractor andserve as the technical advisor to LTA on the matter of supervision . As no formal contract exists

between The Main contractor and QP(S) , all dealing are through LTA and no changes in the

design is to be accepted during the supervision stage , unless the QP(Design ) and LTA have

approved it.

As the main contract is a Design and Build contract , the main contractor has engaged the design

firm (NBH Consultant) to undertake the full design based on LTAs tender development drawings

and also based on further data/information collected by the main contractor.

The main contractor had also engaged Er. Ng Boon Hoo as PE for temporary works as well as PE(TERS) for temporary earth retaining structures .

At the same time , LTA has also engaged Accredited Checker ( AC) namely ECAS-EJ Consultants

Pte Ltd to provide the required check on permanent design of the works, as well as for the

checking of the AIP documents. Where necessary, an AC will also be engaged by LTA.

QP(S) role is mainly to carry out supervision duties and checking services for method statements

and other proposals submitted by ULT Corporation Pte Ltd. The temporary works which are not

deemed to be checked by an AC are also to be checked by QP(S). However, the permanent

design check , obtainment of the permit to carry out structural works from the BCA, and

modifications, if any, at construction stage are to be handled by the QP(D) .

The full scope of works for QP(S) is as described in the contract document for ER420QP

6.1 Main contractor construction works

a) Earth Works, Soil Cutting and Soil Disposal, Sheet Piling Works, Excavation works for

1500mm wide RCU Drain, Casting of 900mm wide RCU Drain, casting of 1400mm wide

RCBC Base Foundation,(Excavation, Installation, C5 Benching Works)

6.2 Safety and monitoring work

a) No non-conformity notice ( NCN) were issued.

b) It is intended to initiate SQN( Safety and Quality Notice) for minor safety and

quality infringements for this project. As of today no SQN were issued aswell.

c) The safety and quality matters were mostly taken up during the PGI and other

routine work walk around.

d) The Contractor doing Safety Tool Box meeting on a daily basis and Sheet Pile Workers

were informed to be very careful whenever working closer to an existing PUB water

line which is a 500mm diameter Asbestos Pipe located along the existing Foot Path at

Upper Changi Road.

6.3 Area of concern

a) SAFETY REVIEW - We have frequently reminded ULT Corporation Pte Ltd that safety

matters particularly at Sheet Piling Area are always top of concerns. So far there

were no incidents reported.

b) INTERNAL PROCEDURES - The Contractor has been reminded to ensure that Method

Statements are submitted at least 28 days before commencement of construction

activities to give sufficient time for the Method Statement to be reviewed. This is

because there are always comments from the LTA and QP(S) which needs to be

addressed before the Contractor is allowed to commence works.

c) UNAUTHORISED WORKS None to report for the month of May of 2013.
