1.3 Intended Use
Indications for Use
ExacTrac is intended for placing patients at an accurately defined point within the treatment beamof a medical accelerator for stereotactic radiosurgery or radiotherapy procedures. Treatmentincludes lesions, tumors and conditions anywhere in the body where radiation therapy isindicated. ExacTrac is also used for monitoring the patient position during treatment.
Intended User
The intended users of ExacTrac are medical staff having received dedicated training for thisapplication, such as:

Known Contra-Indications
To prevent injury to patients wearing pacemakers, contact the pacemaker manufacturerregarding possible hazards resulting from radiation exposure before commencing patienttreatment.

The application is to be used in the treatment room.
Careful Hardware Handling
System components and accessory instrumentation comprise precise mechanical parts.Handle them carefully.
Only trained medical personnel may operate system components and accessoryinstrumentation.
Plausibility Review

1.4 Documentation
Intended Audience
This Site Planning Manual is intended as a reference for the effective and correct implementationof ExacTrac in combination with a Varian TrueBeam Linear Accelerator. This guide is written forcustomers (in particular, customers who have purchased or intend to purchase an ExacTracsystem), as well as all members of the clinical architectural, structural and consulting teams whoare involved in the planning and implementation of an ExacTrac system or parts of it.This guide is to be provided to all relevant parties at the beginning of the planning process andexplained in appropriate detail. Read this user guide carefully and acquaint yourself sufficientlywith the system before planning its implemenation.This Site Planning Manual contains important information about the responsibilities and duties ofall relevant parties as well as a detailed installation description. Pay special attention to thewarnings and cautions provided in this manual.
Reading User Guides
The user guides describe complex medical devices and software that must be used with care.It is important that all users of system, instruments and software:• Read the user guides carefully before handling the equipment• Have access to the user guides at all times
Pre-Installation Information
It is highly recommended to read this document prior to the main system installation. Thecustomer is responsible for finishing all pre-working/pre-assembly steps to ensure a simple andtimely system installation. It is also very important that you inform Brainlab of all informationregarding the hospital’s electrical and spatial conditions.At the end of the document is a checklist you can use to ensure that all listed requirements arecompleted and finalized.If a subcontractor shall perform the required pre-working/pre-assembly steps, the customer isresponsible for providing the subcontractor with this complete pre-installation information includingall warnings.Please note that not all components and items described in this document belong to eachinstalled system. 2.2 Technical Key Features
Infrared Components
The infrared components consist of the following:• Ceiling-mounted infrared and video camera system.• Wall-mounted touch screen monitor (standard).• Control room workspace with monitor, keyboard and mouse.• Compact-sized computer rack for computer, power supplies, electrical safety devices, control
electronics and excess cables (ExacTrac Computer Cabinet).
X-Ray Components
The X-ray components consist of the following:• Two X-ray tubes recessed into the linac room floor.• Two ceiling-mounted amorphous silicon detectors (ExacTrac Flat Panels).• ExacTrac Console.• Dual 65kW X-ray generator with integrated transformer unit.
Optional Components
Some optional components are:• Ceiling-mounted Touch Screen Monitor for convenient handling (optional, standard is wall-
mount solution)• Two X-ray Tubes mounted on-floor in the linac room (optional, standard is in-floor solution).• Tilt alignments of targeting volume via ExacTrac Robotics.

3 SITE SPECIFICATIONS3.1 Treatment Room3.1.1 Overview
General Information
The ExacTrac system shall only be installed in a bunker for medical radiotherapy acceleratorsthat has a stable foundation. Keep vibrations to a minimum since they influence the accuracy ofthe tracking system.
Exposure to high neutron radiation can lead to recurring malfunction and permanentdamage to computer hardware and electronic components. For sites where patients will betreated with energies 10 MV or higher, do not place the computer cabinet in the treatmentroom as it is not designed for exposure to high neutron radiation.
The system must not be exposed to humid environment. Humidity can destroy the systemand can lead to short circuits.
The ExacTrac system is not designed for use in areas where there is a danger of explosion.Moreover, the system must not be placed close to flammable materials, such as volatileanesthetics, detergents or endogenous gases. Observe local regulations to avoid explosive gasmixtures forming during usage and cleaning.Ensure that the tracking volume of the ExacTrac infrared camera system is not reduced/modifiedby other devices after the ExacTrac system has been accepted.Do not install the ceiling mount and device mount in the direct airflow of an air conditioning orheating system.
Do not install any of the electronic components described in this site planning manual(e.g., monitors, cabinets, warning lights, distribution boxes) in the area of the linac’sprimary beam.

3.2 Cabling3.2.1 Overview
General Information
Position all components so that you can use the shortest cable whenever possible.Determine the correct length of X-ray tube power cable necessary to connect the X-ray generatorto the X-ray tubes. Calculate an extra 3 m for the cabling within the X-ray generator to avoid toomuch tension between the cables and the circuit points.In most hospitals, conduits are required for ExacTrac cables due to hospital and local electricalregulations and laws. Below is a table listing the requirements for these conduits.NOTE: Brainlab does not require you to store ExacTrac cables within conduits. However, some ofthe supplied cables are not plenum rated. Therefore, it may be necessary for some customers toinstall all cables within conduits.
You are responsible for obtaining installation permits with respect to applicable local orregional standards. This might affect cable routes, mains power and data cablesspecifications, connection to potential equalization and where you install systemcomponents.
Be aware that fire protection engineering aspects (plenum rating) may be affected wheninstalling cables and system components. Install cable conduits and cables so that theyare not a tripping hazard and the cables will not be damaged.

Wall fixation
In earthquake endangered regions, the com-puter cabinet must be fixed to the wall. An an-gle bracket is supplied with the computer cabi-net.

Avoid placing the cabinet in the treatment room. If this is unavoidable, ensure a maximumdistance between the cabinet and the linac and ensure the cabinet is outside the primarybeam of the linac.
Exposure to high neutron radiation can lead to recurring malfunction and permanentdamage to computer hardware and electronic components. For sites where patients will betreated with energies 10 MV and above, do not place the computer cabinet in the treatmentroom as it is not designed for exposure to high neutron radiation.NOTE: Ensure sufficient spatial clearance before rigging the computer cabinet.

Safety Notes
It is mandatory to get approval and/or clearance from a stress analyst before cutting thefloor box recesses, as building statics will change.
In case the linac is already installed, ensure that the linac base frame is not damagedduring recess construction.
Until the floor box is installed, the empty pit must be marked with warning signs orcovered.
The aluminum cover must rest on stable (concrete) floor minimum with 10 mm.
All dimensions for positioning floor boxes are based on an isocenter height of 1295 mm +/-20 mm (finished floor line – isocenter).

Brainlab provides all hardware parts, except bolts for the concrete ceiling, as local conditions andbuilding construction varies. The hospital or contractor is responsible for:• Installing all ceiling and wall mounts according to local work safety regulations.• Providing appropriate fixation with bolts and anchors according to local regulations.• Assuring correct installation by signing a compliance statement during the acceptance
procedure. Ideally, the statement of compliance should be signed before starting installation.

Recommended Position of the Ceiling Mounted Monitor
• Brainlab recommends positioning the ceiling-mounted monitor at a longitudinal distance of1800+500 mm and a lateral distance of 1250+500 mm from the isocenter. The position dependson other existing ceiling-mounted objects.
• Brainlab recommends positioning the ceiling-mounted monitor on the side of the couch that isopposite to the bunker entry.
• Risk of collision with gantry is still possible but not critical, as the spring arm will give way.Nevertheless, the longitudinal distance should not drop below 1.5 m.
• The monitor will reach the primary beam area. Therefore, instruct your staff to park the monitorin a safe position.
• Adjust the maximum distance between the black interface plate (at height of suspended ceiling)and the ceiling-mounted distribution box to the available cable length of 4 m between the twodevices.
• The horizontal arm must not collide with the camera. To avoid collision, do not simultaneouslyinstall the arm at the same height as the camera and closer than 1250 mm to the isocenter.

5.3.2 Ceiling Frame Installation
General Information
A ceiling frame is only necessary for concrete ceilings > 3.4 m.
The ceiling and intallation plates must be installed by experts in drilling and sawingconcrete. You are responsible for selecting fasteners in accordance with the “load factor”tables (Ondal installation manual) and the safe execution of the fastening work.

How to Install the Ceiling Frame

• Use four heavy-load anchors.• Secure the plate using four fasteners
defined by your hospital’s stress ana-lyst.
• Tighten fasteners according to theirspecifications.

Calculate the length of the tubes andthreaded bolts (+100 mm longer) and cutto size in accordance with plane-paralleland angular requirements.

Screw the hexagon nut approximately 25mm onto the threaded bolt and install thespring washer, then screw on the upperfixing disk.
Screw the threaded bolts in completely, otherwise the ceiling armcan fall down.

The dual generator contains two single generator high-power supplies, which remain in low powerconsumption during standby. During exposure, only one high power supply is active. Computer Cabinet Power Input Main Switch
For service and maintenance of the ExacTrac system (computer cabinet), a power input mainswitch with two switching contacts for the simultaneous interruption of both lines (L & N) ismandatory. The contact opening gap must be at least 3 mm. Major load on mains is the ExacTracisolation transformer unit. The main switch should be able to manage high inrush currents whenpower is on, as it is required for intended inductive loads (e.g., transformers and electro motors).This switch is not provided by Brainlab.Maximum inrush current is 8 x IN.

8.2 Emergency STOP Interlock8.2.1 General Information
If a total system shut down (‘system power emergency OFF’) is desired or required by localregulations, an external circuit must be installed in addition to the mandatory emergency STOPcircuit at the mains of the ExacTrac computer cabinet (1.0) and generator cabinet (2.0). For moreinformation, see the following sections.
Emergency STOP either via the External interlock input (building installation) or from linacinterface is mandatory. The emergency OFF circuit via system power only is not sufficient.
Effects of Emergency STOP (EMO Outputs)
The ExacTrac emergency STOP prevents hazards that could occur from risks arising from theintended operational states. The following table shows the effect that hitting the EMO button hason ExacTrac:

Inthis case no building installation is required. 8.3 System Power Emergency OFF - ETX8.3.1 Introduction
A system power emergency OFF can additionally be installed, if local authorities requestExacTrac to completely shut the system down during an "emergency OFF". In this case, theExacTrac system will immediately power down whenever an emergency button is pushed.
It is mandatory to install one of the two emergency STOP options listed above in additionto the system power emergency OFF circuit.The system power emergency OFF ensures that in the case of an emergency OFF not onlymotion and X-ray related risks are stopped, but also that the system is completely separated fromthe mains voltage.The entire power supply of the ExacTrac System (computer cabinet (1.0) and X-ray generatorcabinet (2.0a)) shall be connected to the emergency OFF circuit. In this way, the ExacTracsystem will be immediately powered down whenever an emergency button is pushed.You or your contractor will be responsible for designing and providing the system poweremergency OFF circuit and buttons.The following image shows an example circuit for system power emergency OFF interlock:

General Information
For Varian Truebeam linacs, ExacTrac receives the motion enable signal from the TrueBeamthrough cable 8a (see Figure 37).NOTE: If a collision is detected by the linac, the motion enable signal will not be activated with theVarian logic. In this case, collision detect is indirectly recognized by ExacTrac and not read as aseparate signal.
NOTE: The same cable provides the emergency OFF signal from the linac and the emergencySTOP signal from ExacTrac (see page 83).

8.4 ExacTrac Warning Lights
The ExacTrac Warning Light Signal
The ExacTrac warning lights warn the user with flashing light that X-ray exposure is enabled bythe ExacTracsoftware in specific steps of the positioning workflow (e.g., when X-ray exposure ispossible). Flashing warning lights indicate that X-rays can be emitted when you press the triggeror exposure button on ExacTrac’s X-ray console.
Responsibilites
The customer is responsible for:• Verifying if the X-ray warning light is compliant to local safety regulations prior to installation.• Deciding if the warning lights shall be installed or not and where they shall be located.• Installing the X-ray warning light cables and boxes.
Brainlab is responsible for connecting the warning lights to the ExacTrac system.
Alternatives
You can choose from the following alternatives:• Alternative A: Use the ExacTrac warning lights (contained in the pre-installation shipment).• Alternative B: Provide your own warning light installation for ExacTrac.

Network Connection
The network connection is required for:• Patient data import and export, depending on the setup even in real-time.• ADI connection to the linac (positioning, patient loading, treatment authorization, etc.
A stable, reliable and safe network environment/connection needs to be provided in orderto guarantee safe and effective use of the ExacTrac system.
Network Performance
• The network speed shall be minimum 10 Mbit/s (Brainlab recommends 1 Gbit/s).• The answering time shall be less than 100 ms.• The network shall be fully switched.
Brainlab requests an external firewall. Network security is your responsibility or the responsibilityof a respective responsible organization.

Distribution Box Wiring Plan
You should provide a distribution box and must bring the power supply (5 lines: L1, L2, L3, N, PE)to the generator. The cable should reach to the bottom of the generator plus 3 m for slack and thecross section should not exceed 35 mm² (4AWG).NOTE: A distribution box is not mandatory to operate the generator and patient positioningsystem. However, for electrical safety reasons, applicable local regulations and work convenience,a distribution box is strongly recommended.
