Dear Ms Billings:

Lockwood Folding Box Co. is a division of H. S. Crocker Co.,Inc. of South San Francisco, CA. We produce only Folding Cartons

',. at this plant, utilizing the Lithographic printing process on| -sheet fed presses.f| As you will see in the enclosed materials prior to 1985 we' generated "press sludge" which was a product of press washups.

With the addition of two new printing presses, training at the; Philadelphia Lithographic Institute and a concerted effort to| reduce the use of chemicals, we no longer generate liquid effluents• from our washup process. Our procedure for washups now is to use

Purpose: To communicate H. S. Crocker Co., Inc.Lockwood Division hazard communicationprogram to all company personnel.

OBJECTIVE

... To protect the health of our employees.

... To provide the employees with the necessary information concerning

health and physical hazards of the materials used in their operations.

... To comply with Title 29, Part 1910.1200, Subpart Z of the Code of

Federal Regulation (CFR): OSHA Hazard Communication.

... To include flexibility in the compliance program so that changes can

be nade to comply with possible state and local right-to-know laws.

SCOPE

... This compliance program will provide information to the employees of

H. S. Crocker Co., Inc., Lockwood Division of the chemical products

to which they are exposed. It will be accomplished by the following:

..»-A. Listing of all chemical products on the property.

B. Appropriate labeling on containers of all chemical materials used.

C. Making available Material Safety Data Sheets (MSDSs) for all

chemical products on the property.. \ '

D. Employee training to recognize and interpret labels, warnings,

color coding, signs, etc. that are affixed to containei

they can properly protect themselves against potential hazards.

F. This written Compliance Program will be made available, upon

request, to employees, their designated representatives(s), and

to all local, state, and federal officials who have proper

authority.

LISTING OF CHEMICAL PRODUCTS.1

... Since we are not a chemical manufacturer, importer or distributor,

H. S. Crocker Co., Inc., Lockwood Division is not required to assess

the hazards or evaluate chemicals. We shall maintain a list of all

the chemical products used on site. We shall always evaluate to the

best of our ability the potential health exposure of a particular

chemical product before we decide to use it.

... We will provide a system under which the Purchasing Department will

obtain MSDSs from all suppliers of chemical products. This system

would include the following:

A. Sending form letters to suppliers-requesting information/MSDSs.

B. A flagging system to ensure that MSDSs ate received and kept current.

C. Maintenance of MSDS files that would be available to employees,

their representatives, local jurisdictional authorities and health

or nedical officers as required by the regulations.

D. A purchase requisition noting that the proper labels are either to

be attached to all containers received, or to be sent.with ±he

order, and that the supplier certifies that'all MSDSs and labels

comply with the standard.

... A master list of hazardous chemicals will be maintained for reference

in the operation management's office. This list will be expanded as

new chemicals are ordered and/or received. All new chemical products

will be appropriately labeled, and a MSDS obtained before receiving -

material at H. S. Crocker Co., Inc., Lockwood Division.

LABELS

... Material received at H. S. Crocker Co., Inc., Lockwood Division shall

be properly labeled. If labels are not provided, we shall contact the

supplier to get the specific labels. Information contained on labels

must not conflict with federal, state or local laws and/or regulations

in labeling requirements. These labels should provide the following

information:

A. Identity of the chemical products or substances in the container;

B. Hazard warnings;

C. Name and address of the manufacturer or other responsible party.

... The labels must.not be removed and will be replaced if illegible.

... All containers of chemical products, including laboratory bottles,

solvent cans and dispensers must be labeled. For smaller containers

(less than one gallon or 3.7 liters), labels must be consistent with

the standards that are specified above. Only those chemicals that can

be classified as "immediate use", which means that the hazardous chemicals

through a pipeline, labels with the required information will be

affixed to the line at the discharge point (valve).

... In those cases where a chemical product other than that specified on

the container label is placed in the container, we must relabel the

container to accurately reflect the hazards of the chemical product

that has been substituted.

TRAINING

All employees in the regulated areas will receive training in the handling of

'chemical products. There will be an annual review of the training program,

and a list of each employee's training schedule will be maintained. The

training program will provide instruction in the following areas:

... The operations of the work area where chemical products are present

including both routine and non-routine jobs;

... The location And availability of the MSDSs;

... Interpretation of MSDSs data and of the labeling system;

personnel who have employees assigned to work on our plant premise

in areas where potential exposure to chemical products exist must be

informed of chemical hazards, availability of MSDSs and appropriate

protective measures.

HEALTH. SAFETY & EMERGENCY PROCEDURES

... To ensure that sufficient and required information is available and

accessible during emergencies, or in the event of a spill in the work

area, or beyond the property line of H. S. Crocker Co., Inc., Lockwood

Division the following information will be available to local health

and jurisdictional authorities if requested or required:

Dear Customer:

Due to current stringent federal, state, and local regulations governingthe operations of all treatment, storage, and disposal facilities, DelawareContainer Co., Inc., in order to remain in strict compliance, must place intoimmediate effect certain necessary changes in our operation. These changesare designed to insure legal compliance in our operation and make our serviceto you more efficient.

As listed below, we have taken into consideration various categories ofcustomers. Your organization falls into one of these. Each category explainswhat your organization must do in order to insure compliance on your part andalso that of Delaware Container Co., Inc.

A. Existing Customers

1. Current Waste Streams-Included are those streams which have beenapproved for disposal at Delaware Container prior to 12/31/84. Priorto 4/1/85, all such streams must be recertified for the calendar yearending 12/31/85. Recertification for disposal at Delaware Containerwill require:a. Analysis by an outside certified laboratory to insure proper

manifesting;b. .Submlttal* to Delaware Container of a"-fully completed Delaware

Container Waste Data Sheet,- a copy of the outside laboratoryanalysis, and a one-quart representative sample of the waste. THEWASTE DATA SHEET MUST BE SIGNED BY .THE GENERATOR. No sample willbe accepted unless the Waste Data Sheet is so signed.

2. New Waste Streams-Included are those streams which existing customersnay generate, due to changes in manufacturing processes, etc. Suchstreams will'require:a. Sampling;b. Analysis by an outside certified laboratory to insure proper

Manifesting;c. Submlttal to Delaware Container of a fully completed Delaware

Container Waste Data Sheet, a copy of the outside laboratoryanalysis, and a one-quart representative sample of the waste. THEWASTE DATA SHEET MUST BE SIGNED BY THE GENERATOR. No sample willbe accepted unless the Waste Data Sheet is so signed.

B. New Customers M 20.0 3 UO1. This applies to organizations that solicit, or are soliciDelaware Container Co., Inc. for transport and/or disposal ofindustrial wastes.

2. Such organizations must:a. Sample each waste stream;b. Have each waste stream analyzed by an outside certified laboratory

to insure proper manifesting;c. Submittal to Delaware Container of a fully completed Delaware

Container Waste Data Sheet, a copy of the outside laboratoryanalysis, and a one-quart representative sample of the waste. THEWASTE DATA SHEET MUST BE. SIGNED BY THE GENERATOR. No sample willbe accepted unless the Waste Data Sheet is so signed.

3. Financial Termsa. Assuming your waste streams have been approved for disposal at

Delaware Container, a certified check covering transport,disposal, and load verification fees must be submitted to ourdriver with the first load. You will be notified of this chargeprior to pickup;

b. All subsequent pickups will be invoiced to your organization withterms of net 30 days from date of invoice;

c. One-time-only pickups will require a certified check, as stated in3.a. above.

C. Less-Than-Truckload Generators1. This section applies to all organizations who ship:

a. forty (40) drums or less per load;b. Less than 5,000 gallons bulk liquids per load;c. Less than eighteen (18) tons bulk sludges per load.

2. Pricing for less-than-truckload generators will be done on a case bycase basis,i .

D. Transport, Disposal, and Laboratory Fees.1. Due to unforeseen changes in the marketplace, ever increasing

operating costs, and compliance with state, local, and federalregulations, it may become necessary to increase these ratesperiodically. . All increases will be prefaced with at least thirty(30) days written notice.

2. All written quotations from Delaware Container will be firm forforty-five (45) days from the date of the quotation letter.

E. Special Cases1. A special case could arise if:

a. Your organization does not fall into any of the above categories,and thus would requie special consideration;

b. Emergency cleanups would be subject to case by case consideration.

F. Labeling and .Manifesting _,1. .All shipments into Delaware Container Co., Inc. must be accompanied by

a Pennsylvania Hazardous Waste Manifest, fully and properly completed.-2. Drum Shipments

a. A31 drums must be marked and labeled «c_a*-rdlngJ^tqJ&current"DOT/EPA.^-regulations;

b. .The reference number supplied by Delaware-Container Co., Inc. to a, particular waste stream must be stenciled onto the TOP AND SIDE ofeach drum or the drum(s) will not be accepted;

(PCE.Y)ETHYLENE GLYCOLS: Dihydric alcohols are physically & chemicallyrelated to glycerol. The glycols that this product contains have lowvolatility and consequently present very little- hazard fran inhalation ofvapors. Severe and even fatal poisoning has cccured following ingestion ofethylene glycol. Under ordinary conditions of industrial use, these glycolsare not considered to be toxic. Eye contact may produce slight pain andirritation.
