ABOUT THE KALAMAZOO HOARDING TASK FORCE (KHTF) In2013,HoardingDisorderreceiveditsownmentalhealthclassificationwhentheDiagnosticandStatisticalManualforDiagnosingMentalHealthDisorders(DSM-5)wasupdated.Atthewritingofthismanual,researchisstilllimited,butinterestandinsightaregrowing.Itisestimatedthatupto5%ofthepopulationmaybeaffectedbyHoardingDisorder,andwhileHoardingDisorderisaprivatementalhealthcondition,itsignificantlyimpactsaperson,his/herfamily,andcanbeapublichealthandsafetyconcern.KalamazooCountyhasapopulationofalmost257,000accordingtothe2010U.S.Census,whichmeanstherearepotentially13,000personsinKalamazooCountydealingwithHoardingDisorder.ManyKalamazooCountyagencieshaveworkedwithhoardingsituationsindividually,experiencingfrustrationintheirattemptstopositivelyimpactahoardingindividual’sconditionandlifestyle.Hoardingmaybediscoveredinavarietyofways,thruinteractionwithemergencymedicalservicesorFire/Policedepartmentresponse,complaintsofneighbors,concernedfamily,buildinginspection,maintenanceneedsatthehomeorapartment,etc.Ifthehoardingsituationpresentsahealthandsafetyissuefortheresident,thefamily,oranimals,itisvitalthattheappropriateauthoritiesbecontacted,becauseasalliestheycanassistgettingthepersonconnectedwithresourcesthatcanhelp.TaskforcesliketheKalamazooHoardingTaskForce(KHTF)havebeenestablishedacrossthecountryprovidinggroundworkforlocalcommunityresources(agencies,organizations,volunteers,family,andfriends)toworkcooperativelywithindividuals,andoneanothertoaddressthisissue.Researchsuggeststhatamulti-disciplinaryapproachnotonlycreatesanimmediatequality-of-lifeimprovement,butcanproducesuccessfullongtermresults.AssistanceshouldinvolvementalhealthtreatmentbyaprofessionalwhosetrainingandexperienceisspecifictothechallengesofHoardingDisorder.TheKalamazooHoardingTaskForcehasbroughttogetheravarietyofdisciplineswhoencounterhoardingsituationsintheirdailywork,inanattempttoimprovetreatmentandeffectivelyutilizepublicresources.

TheKalamazooHoardingTaskForcewasorganizedto:1. ProvideKalamazooCountyresidentswitheducation,information,andsupportforHoardingDisorder.

2. Create,distribute,andimplementacoordinatedplanincludingaworkflow/decision-makingmatrixfor

communitymembers,agencies,municipalities,andlawenforcementandemergencymanagementservicesforencounteringthosewithHoardingDisorder.

W HAT IS HOARDING DISORDER? HoardingDisorderisapersistentdifficultydiscardingorpartingwithpossessionsduetoaperceivedneedtosavethem.Apersonwithhoardingdisorderexperiencesdistressatthethoughtofgettingridofitems.Excessiveaccumulationofitems,regardlessofactualvalue,occursandcanrangefrommildtosevere.Insomecases,HoardingDisordermaynothavemuchimpactonday-to-daylife,whileinothercasesitmayseriouslyaffectfunctioningonadailybasis.Inextremecases,itcandestroyrelationships,ordamageanddestroyahome,placingone's(oranentirefamily's)housingatrisk.

PeoplewithHoardingDisorderoftendonotperceivethenegativeimpactoftheirbehavior,makingtreatmentchallenging.However,intensivetreatmentcanhelppeoplewithhoardingdisorderunderstandtheirbehaviorandlivesafer,moreenjoyablelives.

Signsandsymptoms

HoardingDisorderaffectsemotions,thoughtsandbehaviorandmayincludethefollowing:

• Persistentinabilitytopartwithanypossession,regardlessofitsvalue;• Excessiveattachmenttopossessions,includingdiscomfortlettingotherstouchorborrowthem,or

experiencingdistressattheideaoflettinganitemgo;• Clutteredlivingspaces,makingareasofthehomeunusablefortheintendedpurpose(i.e.notbeingableto

cookinthekitchenorusethebathroomtobathe);• Keepingstacksofnewspapers,magazinesorjunkmail;• Lettingfoodortrashbuilduptounusuallyexcessive,unsanitarylevels;• Acquiringunneededorseeminglyuselessitems,suchastrashornapkinsfromarestaurant;• Difficultymanagingdailyactivitiesbecauseofprocrastinationandtroublemakingdecisions;• Movingitemsfromonepiletoanother,withoutdiscardinganything;• Difficultyorganizingitems,sometimeslosingimportantitemsintheclutter;• Shameorembarrassment;• Limitedornosocialinteractions.

HoardingDisorderisdifferentfromcollecting.Peoplewhohavecollections,suchasstampsormodelcars,deliberatelysearchoutspecificitems,categorizethemandcarefullydisplaytheircollections.Althoughcollectionscanbelarge,theytypicallydon’tcauseathreattohousingsafetyandstability,whereashoardingisgenerallyunorganized,mayinvolvemanydifferentcategoriesofitemsandnodesiretodisplaytheitems.Peoplewhohoardanimalsgenuinelybelievetheyarehelping,orrescuingtheanimals,regardlessoftheconditionstheymaybeproviding.

IfyouoralovedonehassymptomsofHoardingDisorder,talkwithadoctorormentalhealthproviderassoonaspossible.Ashardasitmightbe,youmayalsoneedtocontactlocalauthorities,suchaspolice,fire,publichealth,childoradultprotectiveservices,oranimalwelfareagencies,especiallywhenhealthorsafetyisinquestion.Theseorganizationscanbeanallyinhelpingthepersonmakeapositivechange.

• Reviewtheflowchartandclutterimageratingscalesinthismanual.• Intheeventthatadults,children,oranimalsarethreatenedbyunsafeconditions,contactthe

appropriatelegalauthoritiesasnoted.• **Note-Inorderforadultprotectiveservicestobecomeinvolved,theadultinquestionhastopresent

withavulnerability.Vulnerabilityisdefinedas:“aconditioninwhichanadultisunabletoprotecthim/herselffromabuse,neglect,orexploitationbecauseofamentalorphysicalimpairmentorbecauseofadvancedage.”(MCL400.11)Ifthereisnoimmediatedanger,assistyourfriendorfamilymemberwithfindingaprofessionalwhoisexperiencedwithHoardingDisorderorhoardingissues.Offertohelporganizeaclean-upprocesswhenthetimeappropriate.PeoplewithHoardingDisorderarechallengedwithinitiatingactivityandmakingdecisions,sokeepgoalssmallandattainable.Helpgatherotherswhoarewillingtohelpwiththeclean-up.

o NOTE**Careshouldbetakentoeducateanyhelpersonsafetystrategiessuchaswearinggloves,hardsoledshoeswithcoversifnecessary.Cautionshouldbetakentostepslowlyandbeawareofhowsecurethefloorfeelsbeneathone’sfeet.Environmentalhealthshouldbecontactedregardingtheappropriatedisposalofpotentiallytoxicorharmfulsubstancessuchaspaint,medications,sharpsandmore.

• Don’tpassjudgment.Thisisamentalhealthdisorder.Keepinmindhowembarrassingaconditionlikehoardingisandbeunderstandingoftheperson’sneedsandfeelings.

• Digdeepforpatience.Ahousefilledwithbelongingsdidnotgetthatwayovernightandwillnotbecleanedoutovernight.Mostofthosetelevisionshowsshowing‘quickcleanouts’areonlyatemporaryfixandoftenthehomeisfilledupagainwithinafewyears.PatienceisanintegralpartofhelpingsomeonewithHoardingDisorder.

• Understandthatclean-upisaprocess.MostpeoplewithHoardingDisordersaytheybegantokeepunneededitemsbecausetheythoughtmaybetheywouldbevaluableinthefutureorbecausetheyhadsentimentalvalue.Evenafteraperson'shomehasbeentotallycleaned,he/shewillcontinuetodealwiththesameissuesthatledtohoardinginthefirstplace.

• Itemsthathavebeenidentifiedtobethrownoutshouldberemovedfromthepremisesthesameday.

• Usethismanualtoassessandconsiderthesafety,health,andseverityoftheissue.Calltheagenciesorbusinessesoftheresourceguidewithinthismanualformoreinputandassistance.

6. Playtheroleof“goodcop”and“badcop”withtheindividual.7. Letthemknowthatyouhavetodoyourjob,butyoudon’twanttohavetoseparatethemfromtheir

• Primaryresponseentitydeterminesifassistanceisneededfromotheragenciesororganizations,eitherbasedoninspection/interviewandtheirprofessionalstandards.

• Immediateactionistakentoaddressanyneglectorabuseconcerns,anyunsafeorhazardoussituations(suchasunsafestructureorlackoftoiletfacilities),orfirehazards.
