Connecting With Community

The music has ended, and 13,000-plus team members have boarded 60 buses and headed back to work. How will we build upon the connections we made at Connect: Community’s All-Team Experience?

The month of November began with six identical gatherings of employees, providers, volunteers, board members and other partners—three days of energy spilling out of the Indiana Farmers Coliseum and Champions Pavilion at the Indiana State Fairgrounds. It was Community’s first-ever effort to invite every single team member to be part of a common experience—and it was quite the event!

Attendees listened as Bryan Mills, president and CEO, shared how successful the organization has been at creating an exceptional experience with every life touched. They heard stories of the difference we’ve made in our communities, on our teams and for our patients. They celebrated our 60 years of putting Patients First, and looked ahead to how Community will face the changing landscape of healthcare.

They spun the wheel to guide Community’s food pantry donations. They took selfies, danced the pink glove dance and listened as a five-man vocal group demonstrated how each individual on a team plays a vital role in making amazing things happen together. Community team members connected with each other, and with Community’s mission, vision and values.

The connections will continue as the next year unfolds—check out some pictures, click on the links below for some All-Team Experience memories, and flip the page to read a sampling of how Community team members plan to connect.

How Will I Connect?Around

Community PAG

E 2Work hard

, and be

the best

me.

At the All-Team Experience, attendees were invited to consider what action they will take to connect more in the future with patients, team members and the community. Here are just a few of the thousands of responses they wrote and attached to the Connect panels.

Bring a positive

attitude every day.

Connect with a smile.

Show all

patients and

families love

through my words

and actions

change their

day.

Continue to value my teammates —they’re awesome.

Give back to the community.

Leave my problems at the door.

Take a few minutes each day to learn about someone else’s journey.

View each patient as an individual, with specific needs.

Help patients understand their care.

Remember

my patients

are afraid.

Listen to others’ needs.

Never give up on a client.

Let my patients know as I serve them that they are making a difference in my life.

Respect all.

Be aware that I don’t work alone—

I have a great team!

Thank a coworker.

Remember that I am a part of something bigger.

TELL US WHAT THE SURVEY SAYS • • • Community’s annual employee engagement survey is now under way. It’s the perfect opportunity to make your voice heard and create an ever-improving work environment. Nearly 9,000 employees completed the

confidential survey last year, and we’d love to improve on that number before the survey ends on November 20. If you have not yet taken the survey, please check your email for a survey invitation from IBM Kenexa Surveys, and tell us what you think!

PICK YOUR BENEFITS! • • • The time to sign up for next year’s employee benefits is right now, through November 14, and the open enrollment process is simple. In fact, for a lot of employees, there’s nothing to do! With automatic re-enrollment, if you don’t want any changes to your benefits from what you’ve got right now in 2016, your benefits will simply carry over without any action on your part. Just be aware of some exceptions that do not automatically carry over…if your spouse or partner has coverage, you still will have to submit forms verifying that he or she is eligible for benefits in 2017. Also, if you want to sell PTO or elect a medical or dependent care flexible savings account, you still have to re-enroll in those options through the online system.

CONGRATULATIONS TO NURSING EXCELLENCE WINNERS! • • • Winners of this year’s Nursing Excellence Awards are being honored at the annual awards dinner November 11. This year’s winners include:

Evidence-Based Practice: Patricia Poehler, shared surgical services, Community Visionary Enterprises Inc.

Therapeutic Relationships: Wendy Laminack, emergency department, Community Hospital East.

Art and Science of Nursing: Vicki Powell Tippit, NP, pediatric nurse practitioner, Community Hospital East.

Facilitative Leadership: April Brown, clinical educator, Community Hospital South.

Patient-Focused Care: Jonnie Leisure, wound care center, Community Howard Regional Hospital.

Please click on the link below to see a list of all nominees.

TIME’S RUNNING OUT FOR FLU COMPLIANCE • • • Just a reminder that we’re entering flu season, and Community, like all of the other health care organizations in town, has a mandatory flu policy. Everyone must either get a flu vaccination, or obtain a valid exemption for medical or religious reasons, by November 30. This policy is a condition of employment, meaning that if you plan to remain on the Community team after November 30, you must do one of those things to be in compliance. Please check with your supervisor if you have questions.

SOME UPDATES ON THE PERFORMANCE APPRAISAL PROCESS • • • The 2016 performance appraisal process has now officially started for most employees! Throughout this process, you have the opportunity to ask for feedback from your coworkers, reflect on the contributions you have made this year to fulfilling Community’s mission and your professional goals, and how you have lived the PRIIDE values through the important work you do each day. The first step is to use the PRIIDE values feedback form (previously known as the PA feedback form) to ask for feedback from your coworkers. PRIIDE values feedback forms are due December 31, 2016. Click on the link below to learn how to use the PRIIDE values feedback form. There are also links to review the PRIIDE values guides for staff and leadership roles. Meanwhile, “Notes” is a new feature in MySuccess Center allowing employees to capture information or feedback throughout the year about themselves or another employee. Notes can be kept private or shared with others, including your supervisor. This is a great tool for you to capture key accomplishments throughout the year that you can easily reference when it’s time to complete your performance appraisal forms. Click on the link below to review the Notes Quick Reference Guide. Also related to the PA process, the staff self-assessment forms will be available beginning December 1. Look for more information in the coming weeks! (Employees hired on or after September 26, 2016, are not required to participate in this year’s performance appraisal process.) For technical questions about using the PRIIDE values feedback form or the Notes tool, please contact the service desk at 317-355-5658.

IBJ RECOGNIZES ANNE MURPHY • • • Community’s senior vice president of government relations, Anne Murphy, recently was honored as part of the Indianapolis Business Journal’s “Women of Influence” recognition. Anne has been on the Community team since 2010, part of an influential career that also has featured roles with the Health and Hospital Corporation of Marion County and in state government, including a term as secretary of the Indiana Family and Social Services Administration.

STONECIPHER AMONG THE BEST & BRIGHTEST • • • Congratulations to Lori Stonecipher, executive director for clinical informatics, recently named a winner in the Health & Life Sciences category of the Indy’s Best & Brightest award program. The program was created by Junior Achievement to recognize Central Indiana’s outstanding young professionals, age 40 and under, who are making their mark in and around Indianapolis.

KOKOMO CHAMBER HONORS COMMUNITY HOWARD TEAM MEMBER • • • Lisa Holaday, community relations coordinator for Community Howard, was recently named Business Person of the Year by the Greater Kokomo Chamber of Commerce. Her honor was part of the 102nd annual Celebration of Commerce & Community awards program.

TEAMWORK: EVEN THE BATHROOM OFFERS OPPORTUNITIES • • • Teamwork can be described as a group of people working together cohesively, toward a common goal, creating a positive atmosphere and supporting each other to combine individual strengths to enhance collective performance. It’s important to also acknowledge that our patients and families not only want to see us function as a high-performing work team—they want to be actively included as a member of the team, too! Teamwork, in fact, is one of the drivers of employee engagement. And high employee engagement helps us build upon our brand promise of Exceptional Care. Simply Delivered. There are many benefits teamwork has to offer, including stress reduction. Here’s a story shared by Jan Bresson, Community Way coach with the Office of Patient Experience, about an employee on the Community North ICU who helps create a great environment for his peers.

As you can imagine, working 12-hour shifts on the ICU with highly critical patients can be stressful. One of the ICU patient care coordinators—Ron Gert, R.N.—creates a “happy bathroom” as an escape and oasis for the team. The bathroom is decorated each season/holiday with appropriate decorations, lights, props and scenery. Most recently, the bathroom was decked out in a Halloween theme. The team shares compliments and notes of appreciation…here’s one example: “I believe being able to decompress and take a break helps us all provide a much better patient, family and guest experience!”

SERVICE PILLAR GOALS FOR 2016

We will achieve at least 1% improvement over our baseline scores in multiple

service measures, including how likely it is that patients

would recommend our hospitals and ambulatory

facilities, as well as how they rate our providers and our emergency departments.

COUNTDOWN TO COMMUNITY HOWARD’S CARECONNECT LAUNCH • • • In just a few short months, Community Howard will become part of the Community’s Epic Systems CareConnect operation. As part of the CareConnect implementation, MyChart will be made available to patients using Community Howard facilities. Having Community Howard on the same electronic medical record system used across most other Community facilities has significant advantages for seamless patient care, including quality and safety enhancements, and financial and operational efficiencies. Standardizing regulatory measures across the network helps Community realize a competitive advantage, and Knowledge Management can streamline operations by supporting a common electronic medical record system. Community Howard patients and clinicians will also benefit from the enhancements and customizations that have been made to the Epic applications. Training begins later this month, and go-live is scheduled for February 1, 2017.

ENHANCING CARE FOR SENIORS • • • By 2019, Community expects 20 percent growth in the care of patients 65 or older, and there are predictions that 85 percent of the incremental healthcare spend will be concentrated within this age group. We also know the largest growth of patients in this age group is expected within the North region. Community North this month is launching two pilot programs for aging patients in the acute care setting—the Geriatric Evaluation and Management

Team (GEM) and the Nurses Improving Care for Health System Elders Program (NICHE). Both programs will start on the medical/renal/oncology unit using geriatric technology, home-based tools and resources to promote a fully integrated model tailored for the aging population. Upon pilot completion, we anticipate fully integrating this program into our care approach for patients moving forward.

ATHLETIC TRAINER HONORED BY PEERS • • • Brian Gerlach, a certified athletic trainer with Community Sports Medicine, was selected as the winner of the 2016 Professional Excellence in Athletic Training award by the Indiana Athletic Trainers Association (IATA). Gerlach is the head athletic trainer for Indy Eleven professional soccer team. He was presented with his award during the IATA’s Annual Fall Business Meeting and Clinical Symposia in Muncie last month.

QUALITY PILLAR GOALS FOR 2016

We’re seeking at least 1% improvement over baseline

in our “harm score,” with a target of 1.693 or better, and in readmissions, with

a target of 12.12% or better. We’re also tracking

five measures of ambulatory quality.

CONNECTING WITH INSURANCE: IT’S GOOD FOR EVERYONE

We’re into another enrollment period for the Affordable Care Act’s health insurance marketplace, and Community is once again working on behalf of our patients who would benefit from signing up for insurance. Our partner in this effort is The WellFund, an insurance program that helps make the connection. Through a phone and mail campaign, The WellFund is reaching out to Community patients who don’t presently have insurance, helping them explore their insurance options. In some cases, Community is able to help these patients afford their premiums, and that, in turn, provides them greater access to the care they need. Last year we helped some 850 people afford their coverage, and we expect to reach a similar number this year as they sign up for 2017 coverage. It’s a win-win—we’ve found that every dollar we spend helping these uninsured patients get covered saves us eight dollars in uncompensated care. That’s clearly a plus for Community’s financial picture, but most important, it’s helping improve the health, well-being and quality of life of those in our community.

JOE KESSLER EARNS IBJ HONOR • • • Congratulations to Joe Kessler, Community’s chief financial and chief administration officer, who has been honored by the Indianapolis Business Journal as a finalist in its annual CFO of the Year recognitions. In his tenure with Community, he has been recognized for his innovative approach, always open to looking at things from a different perspective. He has helped the organization maintain a sound financial footing amid a changing, unpredictable industry climate. That’s a remarkable achievement in its own right, but for Joe Kessler, the most important goal is to continually advance Community’s ability to focus on the patient and improve the experience of that patient.

GROWING CHOICES, GREATER ACCESS THROUGH WALGREENS COLLABORATION • • • What’s behind the recently announced agreement in principle to have Walgreens acquire Community’s Wellspring Pharmacy operations next month? Continued growth in access to services for patients and employees alike. Here are answers to some frequently asked questions:

• Wellspring Pharmacy Locations: Under the agreement, the three hospital-based Wellspring Pharmacy locations would become Walgreens locations in December. They would close as Wellspring Pharmacy on December 15 and reopen as Walgreens on December 19. The two pavilion-based Wellspring Pharmacy locations in Noblesville and on Shadeland Avenue would close December 13, and their customers would be served by existing Walgreens locations nearby.

• Pharmacy Benefits: Those on Community medical insurance plans would be able to fill prescriptions at any of the more than 8,200 Walgreens locations, paying the lowest available copays offered by the plans. That vast growth in access points would be a big plus for many employees, especially those who presently don’t have a Wellspring Pharmacy nearby.

• Our Ongoing Collaboration: The agreement would expand upon the existing collaboration between Community and Walgreens. Already, employees can use their Community medical insurance at convenient Healthcare Clinic at Walgreens locations, paying the same copay as a regular primary care visit. And Walgreens clinic patients in need of follow-up care already enjoy a more streamlined transition to Community services. Community and Walgreens will continue to explore ways to build upon the collaboration through services and technologies.

• Growing Options for Patients: With the proposed Community-based Walgreens locations, patients would enjoy the convenience of filling their prescriptions at Walgreens as they leave an appointment on our East, North or South campuses, or as they head home from the hospital. Refills would be more convenient for them, because there’s likely a Walgreens near their homes. And Walgreens intends to significantly grow the bedside delivery program that Wellspring Pharmacy has developed for hospital patients.

• Some Procedures Will Change: The courier service that Wellspring Pharmacy has provided to some Community office locations would not be available through Walgreens, but other convenient delivery options could be arranged with the help of a Walgreens associate. Payroll deduction would not be available at Walgreens, but flexible spending or health savings account debit cards are accepted for prescriptions and other eligible items at Walgreens and would continue to be under the new agreement (if you’re interested, be sure to sign up for an FSA or HSA during open enrollment).

• Assisting Wellspring Employees: Walgreens and Community are working to ease the transition as much as possible for current Wellspring Pharmacy employees. We are working with Walgreens to ensure, to the greatest extent possible, that Wellspring Pharmacy employees will be able to find positions within Walgreens if desired, or within Community.

Please click on the link below for more information, and if you’re a current Wellspring Pharmacy customer, you may receive additional information in the mail.

CENTER FOR JOINT HEALTH OPENS AT COMMUNITY HOWARD Community Howard Regional Health opened its new Center for Joint Health last month. As with the Center for Joint Health locations at Community East, North and South, it’s a dedicated unit at Community Howard for hip and knee joint replacement patients. The nine-bed center’s concept of wellness is executed throughout the entire program, which incorporates advanced, minimally invasive and rapid-recovery surgical techniques. The environment maximizes patient recovery through education, a culture of early mobility, family involvement and group interaction. Alongside a joint care coordinator, physical therapist, nurses and social worker, patients at the Center for Joint Health are navigated through the surgical and rehabilitation process from beginning to end—including their transition out of the unit and back home. Visit eCommunity.com/jointcenter to learn more.

MDLIVE TELEVISITS: ONLINE FOR THE COLD AND FLU SEASON • • • Cold and flu season is on the way, a perfect time to activate your MDLIVE account! MDLIVE provides anytime access to board-certified doctors and pediatricians from where it’s most convenient for you—home, office or on the go—for non-emergency medical conditions. After registering, you can have a virtual consult with one of MDLIVE’s experienced doctors on average in less than 10 minutes via secure online video or phone. MDLIVE is offered to Community employees at a rate of $39 during the soft launch. For those on the network’s Gold plan, there’s a $20 copay. Those on the Silver plan pay the $39 rate but can use HSA funds to cover the visit. Click on the link below to sign up, or call 888-792-1147 and choose Option #1. There are also links below to questions

and answers, as well as a video example of an MDLIVE televisit. And if you have additional questions about MDLIVE and your Community benefits, please contact ProHealth Customer Service at 317-621-7565.

GROWING CONNECTIONS WITH WOMEN • • • Community Health Network hosted a women’s event titled “It’s Time for Me!” last month. The event offered free health screenings, including blood pressure checks, A1C screening, breast exam, visible veins, facial sun screening and bone density. Also on the bill were health information stations, a physician panel discussion and a heart-healthy breakfast. With 91 people registered to attend, there were 137 free screenings provided, and attendees received valuable health information to manage their health and that of their family. Physician panelists were Kristina Box, M.D., Christina Kim, M.D., Krishna Malineni, M.D., and Nan Oscherwitz, M.D.

FOUNDATION LANDS $1.5 MILLION GIFT, ITS BIGGEST EVER • • • Local philanthropist Tony Alderson recently made a $1.5 million gift to Community Health Network Foundation, the largest donation from an individual in the foundation’s 40-year history, to support Community’s cancer patients. Alderson’s donation will be used to develop a comprehensive plan to implement and grow the integrative oncology program—which offers holistic services such as art therapy, yoga, tai chi and massage—across all six of Community’s Central Indiana regions. Made on behalf

of his family—both immediate and extended—Alderson’s gift will be recognized on the campus of Community North by naming the connecting walkway between the hospital and the new Community Cancer Center North the “Alderson Family Bridge of Life.” Click on the link below to learn more about why this gift is so meaningful to him.

HOLIDAY HELP FOR COMMUNITY FAMILIES • • • With holidays approaching, there are many reasons to be thankful. But there are also people in need, some on our Community team, who are dealing with the added stress this time of year can bring. That’s the reason behind Community’s Holiday Help program. Because of the generosity of Community

employees who signed up to provide holiday gifts, more than a thousand children ages 18 and younger were supported during the 2015 holiday season, and requests have already been made for assisting 700 this year. If your family could benefit from assistance this year, enrollment for the Holiday Help program runs through November 25. If you and your work team would like to adopt a Community family, please sign up by December 5. More info is on the Holiday Help page on InComm. (Similar programs are available at Community affiliate hospitals).

STILL TIME TO SERVE360° IN 2016 • • • The year is quickly coming to an end, but there are still plenty of ways you can volunteer between now and then! In fact, the holiday months are often some of the busiest times at our local food pantries, and we support many on a weekly basis. To learn more or register, visit MySuccess Center/MyLearning and search by “Serve.”

COMMUNITY PILLAR GOAL FOR 2016

Our expectation is that all Community leaders

will take part in at least one Serve360° volunteer

opportunity during 2016.
