Sedimentation Rate Testing Change

   Dear Doctor:  I would like to inform you of a change in the sedimentation rate testing in our Hematology Laboratory.  Effective, October 13, 2015, all sedimentation rate assays will be performed by the AliFax® Roller 20 PN. This automated method uses stopped flow kinetic analysis with capillary photometry to measure the optical density of red blood cell aggregation.  The Roller 20PN provides several advantages over our current method, the modified westergren technique:

1. It requires less volume of whole blood. 2. It is not susceptible to the interfering substances which affected the modified

westergren method, i.e. plasma proteins, temperature, hematocrit, MCV results, and plasma viscosity.

3. The stability of the sample is 48 hours refrigerated.  The susceptibility of the modified westergren to interfering substances became very apparent during the validation of the Roller 20PN. Correlation bias testing percentages ranged from -22 to -6.4% for patient results. It is vital that we provide you with this information so that you can properly evaluate current and previous test results. There are no laboratory standards for comparative baseline testing for sedimentation rates.  There have been normal results generated on the Roller 20 for patients with multiple myeloma. The international guidelines for the diagnosis and management of multiple myeloma do not recommend sedimentation rate testing in this population. Sedimentation rate testing, if required, should be performed by a different sedimentation rate method.  Please note: Sedimentation rate results will now chart under “Sed Rate”. This may change where you review results on your EMR.  Sincerely,

Fire Safety Procedures - RACE Rescue patients in endangered fire area. Alarm: Use the nearest fire alarm pull station. Dial 88 to report more information. Contain the fire and smoke by closing all windows and doors. Evacuate the impacted area. Locate the fire alarm pull stations, fire extinguishers and emergency exits in your area. Remember their locations.

There have been instances of drivers ignoring Stop signs at crosswalks and/or driving too fast when approaching and entering parking structures. Please use caution when going from daylight into a parking structure due to changes in visibility. Please be mindful of pedestrians when driving on Torrance Memorial’s campus. Your assistance in keeping our environment safe for patients, visitors and staff is much appreciated.

OUT OF CONCERN, MAY I ASK MY CO-WORKER / PATIENT HOW HE/SHE IS DOING, THE REASON FOR THE VISIT OR INFORM OTHER STAFF MEMBERSABOUT MY CO-WORKER / PATIENT? ANSWER: No. You should NOT ask your co-worker / patient how they are feeling, the reason for their visit or any other question relating to their visit, procedure, treatment or medical condition. And No, you should not inform other staff members or friends about the patient. Sharing, discussing or disclosing patient information for non-TPO reasons is a HIPAA Privacy Breach. All patient medical information (employees and non-employees) is confidential and protected. Patient PHI comes in many forms;

Seeing a patient in a clinical department waiting to be seen; Performing or assisting with a procedure or treatment on a patient; Hearing information being discussed in your department or other area; Viewing information on paper, schedules, tracking boards, etc., the computer or electronically; and Oral communication.

Every employee has an obligation to protect patient information. If you see a co-worker, friend or family member here as a patient, you should leave it up to the discretion of the patient to speak to you about their visit, treatment, procedure or condition. Protection of patient confidentiality is a State and Federal Regulation and an important practice that should be part of our daily activities. PROTECTING EVERY PERSON’S PRIVACY IS A KEY PART OF QUALITY CARE PHI = PROTECTED HEALTH INFORMATION = Patient name, room number, address, phone number, SS#, diagnosis, procedures, insurance information, email, photographs, etc. TPO = Treatment, Payment or HealthCare Operations.
