Health Enhancement Program (HEP)The Health Enhancement Program is a component of the medical plan

that is administered by WellSpark, formerly Care Management Solutions.

You and your enrolled family members must get age-appropriate exams and screenings in order to remain compliant.

A list of all required preventative screenings by age interval can be found on page 6 of the Connecticut Partnership Plan 2.0 fact sheet.

In order to manage your health and HEP requirements, you can visit www.cthep.com.

In the event that you or an enrolled dependent becomes non-compliant, your premiums will be $100 per month higher and the in-network deductible of $350 / individual ($1,400 per family) will apply.

FMLA What is FMLA and how does it work?

The Family and Medical Leave Act (FMLA) guarantees Norwalk Public School employees up to 12 workweeks of unpaid leave each year with no threat of job loss. It also requires that employers covered by the law maintain the health benefits for eligible workers just as if they were working. At the employee premium cost share.

COBRA The Consolidated Omnibus Budget

Reconciliation Act (COBRA) is a health insurance program that allows an eligible employee and his or her dependents the continued benefits of health insurance coverage in the case that employee loses his or her job or experiences a reduction of work hours

ESS ENROLLMENT PROCESS

To access Employee, Self Service, ESS, go to the NPS main web page and click on “FOR STAFF”. Next, click on Employee Self Service, or you can go directly to, https://norwalkct.munisselfservice.com/. Once you’re on the ESS main page “Norwalk, CT Self Service”, you will see Log-In in the upper right-hand corner, click on the Log-In to create your account. For ESS Account issues, contact payroll, 203-854-4060.

To Create your Account you will need a Username/ID and a temporary password:Username/UserID: last name, first initial (never changes)

Password: Temporary Password: Summer123456 - you will be required to change to a permanent, personal password, when you first log-in.

Once your password has been created, click on “Employee Self Service”, this will take you to the home page. To enroll in benefits, click on Benefits in the left margin of the home page. This will bring you to the Benefits page where you will see the below three coverages. Click on “make selection” for each coverage in which you wish to enroll. You can choose any combination of the following three coverages:

Medical & Prescription Coverage – choose United Healthcare Vision Coverage – choose Anthem Blue View Vision Dental Coverage – choose Delta Dental of New Jersey

If you are adding a spouse and/or dependents, you will need to supply a “Marriage Certificate” for spouse, Birth Certificates for each of your children/dependents and a copy of the Social Security card. Once your enrollment choices are complete, click on “submit choices” on the main page to finalize your selection(s) and you will receive a confirmation page. You may print this out for your records. Copies of supporting documentation, i.e., Marriage Certificates/Birth Certificates should be sent to my attention via U.S. Mail, interoffice mail, or scanned and emailed to [email protected]. Enrollments cannot be processed without these documents.
