Disposing old equipment

Take responsibility to find out if there are any ‘items of concern’ in equipment that you’re going to dispose. Can’t simply throw everything away any more.

•Toxic materials, such as PCBs, dyes, batteries, etc. must be disposed properly.

New State Law: No electronics may be thrown in the trash.

If you are unsure, contact us. The safety committee can help.

Syringes

Considered PIMW* – must not be disposed in trash.Waste transfer station workers can’t tell the difference.

Syringes must always be disposed in a Sharps container...

UCLA fatal tert-butyl lithium fire - Dec, 2008

Very brief summary

•While dispensing t-BuLi, syringe plunger popped out of syringe body, material splashed onto her clothes.•The material ignited. (It’s pyrophoric)•Knocked over an open flask of Hexane – which also ignited.•Fire ignited her polyester sweater, severely burning her arms and upper torso.

UCLA fatal tert-butyl lithium fire - Dec, 2008

Complex case, nonetheless a few basic lessons can be related:

•PPE! No lab coat worn•Amount of material vs. capacity of container 50mL/60mL•Unrelated chemicals in work area•Know the location of, and use emergency equipment•Think about what else you’re wearing: Synthetic materials like polyester described as being “analogous to solid gasoline.”*•Don’t take things for granted (more later)•Undoubtedly more.

Texas Tech Nickel hydrazine perchlorate explosion – Jan, 2010

Very brief summary

•Students synthesized 10 grams of material instead of 100 milligrams max. •5th year grad student removing ‘lumps’ in 5g portion of the material with a mortar & pestle.•Set the materials down and removed goggles.•Returned to give it one more stir, neglecting to don goggles.•Material exploded, causing the loss of 3 digits on left hand, lacerating right hand, perforating left eye, scratching right eye, etc.

Texas Tech Nickel hydrazine perchlorate explosion – Jan, 2010

Complex case, nonetheless a few basic lessons can be related:

•PPE! Removed eye protection before last ‘stir’ then failed to put it back on.•Researcher’s space was disorganized, items not labeled. Conflicts over space, cleanliness, use of chemicals.•Students were apparently told there was a 100mg limit – no documentation to support this.•No notes taken during the experiment, uncertainty over the material synthesized.•Undoubtedly more.

•Injuries•Questions of training, supervision•Documentation of training and restrictions imposed on students essentially nonexistent.•TTU overhauls its safety structure.•Acknowledgement that lab standards focus on health and exposure hazards, but physical hazard information is way behind, or missing

• Seemingly endless, minute safety details make sense and protect researchers, but are not necessarily complete.

• Familiarity can be the enemy – you do some things every day.

• Almost inevitably, perception of risk may change, but the risks do not.

One of the big challenges in keeping safety measures prominent in the minds of researchers

• Try not to take things for granted.• Reporting of incidents and near misses are a way

for everyone to learn.• SOP template includes a signature line for

documentation of training – this isn’t by chance.

• Without documentation, essentially no training has occurred in the eyes of investigating bodies.
