Dear Portico Supporters,

Last year, Portico’s nearly 20-year endeavor to help people access affordable health care coverage was validated by federal and state policy makers and the health care industry as a whole. In 2013, MNsure, Minnesota’s health care insurance exchange, officially opened for business, and the concept of enrollment assistance through certified navigators was introduced to the public. Outreach to educate people about their potential eligibility for low or no-cost health care coverage has now transformed from a limited, local model to a nationwide effort. Portico is at the forefront of this work; through training navigators from across the state to providing on-site services at sites across the metro area, we continue to position ourselves as a Metro Region Center of Excellence for Enrollment and Referrals to low-cost health related services. For people who remain ineligible for programs through MNsure, Portico and our hospital partners continue to offer an alternative coverage program.

As the health care environment and the needs of our clients evolve, Portico will continue to respond with innovation and agility. Thank you for your continued partnership and support of our work to ensure all people have access to affordable health care.

About UsPortico is a nonprofit health and human services organization that connects uninsured individuals and families from the Twin Cities metro area and throughout Minnesota to the resources needed to effectively access services and manage health. Portico employs Certified MNsure Navigators who provide outreach services in the community and personalized application and enrollment assistance for programs available through MNsure. Portico also offers a unique alternative coverage program that includes individualized education and guidance for every household on how to use the health care system effectively. Our Health Access Line serves as a first stop in navigating a complex health care system for underserved people. Callers may be transferred to a MNsure Navigator for assistance with MNsure enrollment, receive information about low-cost options for medical, dental and mental health care and coverage, or be referred to appropriate community resources for help with other needs like housing, food and family support.

Portico is a Metro Region Center of Excellence for Enrollment and Referrals 2014 Organizational highlightsNew, Innovative Models of Service:

• Portico began partnering with correctional facilitiesthroughout the state to train soon-to-be-releasedincarcerated individuals on the importance ofinsurance, options for affordable insurance throughMNsure, and how to connect with a navigator.

• To meet the increased demand for services andutilize MNsure’s new technology, Portico developedgroup enrollment sessions – an option that is offeredregularly during open enrollment at sites throughoutthe region.

• A full-time dental coordinator was hired to increaseclient access to low-cost dental services. In 2014Portico provided 566 referrals.

Best Practices: • Added innovative text messaging capabilities to

better reach clients and increase the quality ofservices.

• Trained more than 400 navigators and CertifiedApplication Counselors from across the state.

• Created a MNsure Enrollment and Client CoverageToolbox, a reference guide that navigators can useto help them through the application process,confirm coverage and educate clients on how touse their insurance.

2014 Outcomes Outreach and Enrollment Assistance:• Increased assisted enrollments from 2,300 in FY2013

to 5,400 in FY2014, reflecting a 47% increase inproductivity per navigator.

• Placed certified navigators at 25 sites throughout themetro area, expanding our distribution model from5 sites the previous year. By partnering with targetedcommunity agencies we were able to reach more peopleat locations that were closer to home.

• Received more than 5,600 calls through our healthaccess line.

Coverage and Care Management Program:• 1,294 enrolled in Portico’s coverage program.• 404 transitioned from Portico’s coverage program to

public or employer-sponsored coverage programs.• 62% of participants had a complete physical in the 12

months after enrollment compared with 42% in the yearprior to enrollment.

• After one year of enrollment, the number of participantemergency department visits and inpatienthospitalizations decreased significantly (38% and 36% respectively).

Client Story Last February, Cruz had emergency gallstone surgery on the same day her baby daughter Lupita had surgery to repair a lung malformation. Carlos, a seasonal laborer, had health insurance through his employer but it was dropped several years ago. The family tried to work with the county to get help paying their hospital bills, but they couldn’t get everything resolved, and the language barrier was difficult.

That’s when they contacted Portico Healthnet, knowing that we help the uninsured gain access to affordable health coverage and care through culturally competent services. Portico helps this family renew medical assistance for their three children every six months, including submitting the often complicated paperwork. Recently Portico helped the family enroll in coverage through MNsure.

Cruz appreciates Portico’s assistance in navigating the complex health care system. Portico’s care coordinator often facilitates a three-way call between the care coordinator, the county and Cruz. The care coordinator translates the call so Cruz is informed about what is happening and can learn important self- advocacy skills.

Today, Lupita is learning to walk and her doctor says she is doing well.
