Memorandum

To: All Medical Staff Members From: Medical Staff Services Department Date: 9/17/2020 Re: Flu Season 2020-2021

_______________________________________________________________________________ The Medical Executive Committee has adopted the Torrance Memorial Medical Center flu vaccination policy for the 20/21 flu season. This year the flu vaccine is required for everyone entering the medical center. Due to the COVID-19 pandemic, vaccinations will be offered at the nursing stations and a few other clinical departments. The nursing clinical staff be allowed to administer injections following very specific guidelines. Physicians may receive vaccinations in these clinical areas. A listing of those areas is available in the Medical Staff Services Departmen. In addition, there will be open clinics where physicians may receive the vaccine. The schedule for the clinics will be sent out prior to the start of flu season. Employee Health Services does not have the capacity ensure proper social distancing, so physicians will not be able to receive the vaccine in that department. There are changes to the declination form this year as well. There will be two methods for declining the flu vaccine: Medical Exemption and Religious Accommodation. The medical exemption must be completed by your physician and should be filled out completely including the physician’s signature. The religious accommodation must be completed by you. Please contact the Medical Staff Services Department at extension 4616 to request the form. After the form has been submitted, it will be evaluated by a panel made up of HR and legal counsel. Should you want to submit either of these forms, it must be done prior to 10/15/20. No declination or accommodation forms will be accepted after that date, and all physicians without a medical exemption or religious accommodation will be required to get the flu vaccine or be subject to suspension. 2020 – 2021 Influenza Immunization Policy Frequently Asked Questions To help ensure the health and safety of our patients, visitors, employees and the community at large, Torrance Memorial/Torrance Health Association is implementing its annual flu vaccination program as flu season approaches. For the first time and consistent with industry trends, ALL employees and physicians in all work locations are required to be vaccinated for the influenza virus (flu) with few allowable exceptions. All employees, medical staff, vendors, contracted personnel, volunteers and students must get their annual flu vaccination by Nov. 17, 2020. Those physicians who decline the vaccine without an approved medical exemption or religious accommodation will not be allowed to work and are subject to suspension of privileges. For questions related to COVID-19 and the 2020-2021 flu season, please see the COVID-19 and Influenza FAQs at the end of this document. What will happen if I don’t comply with the Torrance Memorial/Torrance Health Association flu vaccine requirement? Physicians who fail to comply and who have not received a medical exemption or religious accommodation will not be able to work past November 17, 2020 and will be subject to suspension.

Will we be given the flu shot stickers this year? Yes, we are issuing flu shot stickers again, and they must be worn visibly on your badge throughout the entire flu season no later than No-vember 17. If you are vaccinated, you will receive an orange heart shaped sticker with “Flu Season 2020-2021” on it at the time of vaccina-tion. If you have an approved religious accommodation or medical exemption for declining the vaccination you will receive a round, yellow sticker with “Declined Flu Vac 2020-2021” on it. The flu stickers are different colors this year so you will not be able to wear last year’s flu sticker.

Why do we have to wear flu stickers? The flu stickers are the easiest method we have to ensure that all employees and physicians are in compliance with the policy. Public health officials will be auditing hospitals to verify compliance.

Will I be required to show my badge with my flu sticker? To protect staff and ensure compliance with the flu vaccine policy, personnel will monitor badges at medical center entrances during flu season. Physicians may be asked to show their badges if they are not easily visible.

If I wear a badge sticker, is this a HIPAA violation? No, it is not a HIPAA violation. The County of Los Angeles Public Health Order mandates that hospitals enforce the policy through a form of identifier, and other hospitals are using similar identification methods, such as stickers and/or “badge buddies”.

Are other hospitals doing this? Yes, more hospitals are transitioning to mandatory influenza requirements for all employees and physicians, regardless of patient care ac-tivities.

What happens if I don’t wear my flu (or exemption) sticker? For medical staff, contract and agency staff, and Allied Health Professionals (AHP) who are not employees, if a practitioner fails to comply, employees are instructed to report up their chain of command.

How long is this policy in effect? The policy is effective immediately through April 30, 2021, unless extended due to an unusually long flu season.

What is the masking requirement for employees and physicians with an approved medical exemption or religious accommodation? For those employees and physicians with an approved medical exemption or religious accommodation, masks must be worn at all times.

If I have a valid exemption and wear a mask, how often do I need to change it? Masks may be worn continuously (and between patients) until they become moist or visibly soiled, at which point they should be replaced, in accordance with current mask reuse and extended use practices.

I already received my flu shot. How will I get my flu sticker? Please provide proof of vaccination to the Medical Staff Services Department no later than November 17 and you will be provided with a sticker.

What is the process for requesting a medical exemption or religious accommodation? Exemptions may be granted for medical reasons, per guidelines from the Centers for Disease Control and Prevention (CDC), or accommo-dations may be granted for religious reasons. All requests for medical exemption or religious accommodation must be submitted per the instructions below and include all required documentation by no later than Oct. 15, 2020. This timing ensures medical exemptions and religious accommodations can be considered and processed. This deadline cannot be extended.

To request a medical exemption, please have your medical provider complete the Physician’s Verification of Request for Medical Exemp-tion from Influenza Vaccination form that is available from the Medical Staff Services Department. Once the Physician’s Verification of Request for Medical Exemption from Influenza Vaccination form is completed and signed by your medi-cal provider, please submit it to Medical Staff Services. Your request will be reviewed by a panel and you or your physician may be con-tacted for more information before the panel decides whether you meet the criteria for exemption. Once the panel completes the review, you and the Medical Staff Services Department will be notified of the disposition by a member of the panel. A panel member will be avail-able to discuss any questions or concerns. Physicians who would like to request a religious accommodation should call the Medical Staff Services Department and complete the Re-quest for Religious Accommodation form. This will also go for panel review and you will be notified of the outcome.

Will the policy change during the flu season? The policy may change due to the unpredictability of the flu season. There may be a need to ramp up efforts to protect against influenza spread if we experience a severe pandemic. Measures such as revised visitor policies, or other appropriate measures may be considered.

What should I do if I see an employee or physician with an exemption sticker who is not wearing a mask? Politely remind the employee or physician to wear a mask. For example, “I noticed you are not wearing a mask, may I offer you one?” or “Masks are available at the nurse’s station.” Staff members are also asked to report noncompliance up their chain of command.

I have an egg allergy. What are my options for the flu vaccine? The CDC recommends that any age-appropriate flu vaccine can be given to those with an egg allergy. For those with a history of a reaction other than hives, typically the vaccine should be given in a medical setting where a severe reaction can be managed. However, during the 2020-2021 flu season, the current vaccine does not contain components that will negatively affect those with egg allergies. Physicians are screened for allergies before vaccine administration. Please notify your vaccine provider if you have an egg allergy or have any questions related to the vaccine.

I’ve also heard about the quadrivalent vaccine. What is that and should I get it? Quadrivalent (QIV) vaccines are made to protect against four different strains of the flu virus (two influenza A and two influenza B). This year quadrivalent vaccines will be the standard vaccine available at Torrance Memorial/Torrance Health Association, as manufacturers have shifted to making this vaccine more widely available. While quadrivalent vaccines provide coverage against one additional flu strain compared to the traditional trivalent (TIV) vaccine, the benefits of this additional coverage are not fully understood. As in prior years, no preferential recommendation is made by the CDC for the quadrivalent influenza vaccine over the trivalent vaccine.

Covid-19 and Influenza: FAQs

Please note: As understanding of COVID-19 evolves, please review the Centers for Disease Control and Prevention website for the most up-to-date information about COVID-19 and the flu. What is the difference between the flu and COVID-19? Influenza (flu) and COVID-19 are contagious respiratory illnesses that share similar symptoms, but they are caused by different viruses. Symptoms of both illnesses range from no symptoms (asymptomatic) to one or more of the following:

• Cough • Fatigue (tiredness) • Fever or feeling feverish/chills • Headache • Muscle pain or body aches • Runny or stuffy nose • Shortness of breath or difficulty breathing • Sore throat Some people with the flu or COVID-19 may experience vomiting and diarrhea, though this is more common in children than adults. Symp-toms currently found to be unique to COVID-19 are changes in – or loss of – taste or smell. COVID-19 also has been found to be more con-tagious than the flu among certain populations or age groups. How do I know if I have the flu or COVID-19? Both COVID-19 and the flu can cause similar symptoms to varying degrees, ranging from no symptoms to those featured above. If you de-velop the above symptoms, please stay home and call Employee Health Services (EHS) for guidance on self-monitoring and COVID-19 testing. Is it possible to have COVID-19 and the flu at the same time? Yes. It is possible to test positive for the flu (as well as other respiratory infections) and COVID-19 at the same time though understanding around the likelihood of contracting both is still evolving. Is it safe to come to work to get my flu shot? Yes. We continue to implement safety precautions to ensure our facilities, including flu clinics, are a safe place to receive care. These in-clude screening and temperature checks upon entering the facility, universal masking, physical distancing, and thorough cleaning and dis-infection. We also continue to monitor the latest COVID-19 data and coordinate closely with public health authorities to ensure we max-imize protection for our patients, visitors and staff members as the pandemic evolves. Should a flu vaccine be given to someone with suspected or confirmed COVID-19? No. For the safety of staff members, visitors and patients, people with suspected or confirmed COVID-19 should wait until they have met the criteria to stop isolation before getting their flu shot. For the latest guidance on what to do if you have suspected or confirmed COVID-19 and require your flu shot, contact Employee Health Services. Does getting the flu vaccine affect the chance of getting COVID-19? Because influenza viruses and coronaviruses are different, a flu shot won’t protect against COVID-19. However, flu shots can indirectly assist with the response to the pandemic. Flu shots help healthcare workers who are treating COVID-19 patients by reducing the number of flu cases they also need to treat. Additionally, as both the flu and COVID-19 share similar symptoms, the flu vaccine reduces unneces-sary COVID-19 testing and helps healthcare workers identify COVID-19 patients more effectively. Is someone who had COVID-19 or the flu protected from future COVID-19 or flu infections? A prior flu infection does not protect someone from future flu infections, because there are several different flu strains and the flu virus changes over time. At this time, we do not yet know if being infected with COVID-19 protects you from future COVID-19 infections.

Protecting our patient’s data with Okta Hello all, As your Executive Director of Medical Informatics, I am your physician liaison to all things Cerner Millennium. With an experienced team of informaticists and service specialists supporting me, we are available to assist you with questions or ideas regarding use of the electronic medical record across both the acute and ambulatory locations of the Torrance Memorial Health System. Protecting our patient’s data from cybercriminals, hacking or ransom attacks is of utmost importance to our organization. Thanks to the efforts of Todd Felker, our Director of Information Security, and his team, we are mov-ing forward with a new version of Multi Factor Authentication (MFA) to allow you to access this vital information. Todd and his team monitor them system 24 hours a day and are always prepared to fend off an attack. To keep data secure, MFA uses several different “factors” to prove you are who you say you are. This can be something you know, like a password, something you have, like a phone or tablet, or something you are, like a fingerprint, facial recognition or retina scan. Often multiple sources are used for extra layers of protection. You are likely familiar with the most frequently type of MFA in use across the internet, the six digit code sent to your phone to access a website. Cybercriminals are always finding ways to intercept these numerical codes, so research into additional ways of identifying the user must outpace their tactics. To keep our system as secure as possible, we have implemented two new measures. The first is changing the web-site address you use from outside the hospital, from the old connect.tmmc.com to the new remote.tmmc.com. You will need to download a new version of the Citrix server to access this new website. The second step is to utilize an application called Okta. You will preregister with Okta, and that will provide you with your access code when you log in. You will no longer have to contact the help desk for a code or remember security questions and answers. Okta will also be used for access to Teams, OneDrive and Outlook, as well as across the hospital with the rounding program Sentact. You can download the Okta application to your phone for easy access when you are outside the hospital. Inside the hospital things will remain the same. For those of you who are ePrescribing, we will continue to use Google Authenticator as we do now. I would wholly encourage every physician to sign up for ePrescribing. has to be enrolled in Okta. In order to ensure that you can quickly authenticate on Thursday morning, please make sure you are enrolled at https://TMHS.Okta.com before the 24

th.

What’s happening? This Thursday, September 24

th, we are switching the Multi Factor Authentication (MFA) software on all

remote access to the new TMMC / TMHS standard. The new software is called Okta and it will not only make us more secure, but it will also provide an improved user experience. If you used SMS for your MFA verification previously, you have been auto-enrolled in Okta already. What do you need to do? In order to connect to TMMC via Remote Access, you must be enrolled in Okta. There are two options: 1: Okta Verify Mobile App 2: SMS via a mobile phone. (TMMC Mobile Heartbeat Red Phones cannot be used for verification) Okta Verify is a little more secure, but SMS is the easiest and quickest way to enroll. If you do not enroll before Thursday (and you were previously NOT using SMS), the first time you try to access Re-mote.TMMC.com after we make the change, you will be prompted to enroll. The enrollment is quick and easy, but it will require you to have your cell phone handy when you do enroll. Use the attached guide and follow the instructions to enroll at https://TMHS.Okta.com Why can’t I use the security questions anymore? There is no way to transfer the old security questions from the old software to the new software. There are only two options, SMS (receiving your MFA codes via text) or by the Okta Verify application that you can install on your mobile phone. What if I used SMS (text) before to receive my code on my cell phone? If you used SMS with our old MFA software, we automatically enrolled you into Okta in advance. Almost done, just a couple more things…. When you connect to Remote.TMMC.com, instead of seeing the previous login screen:

COVID 19 Antibody Healthcare Workers Study

RESEARCH STUDY: HEALTHCARE WORKER ANTIBODY TESTING

Cedars-Sinai is conducting a research study that aims to understand how antibody testing could help protect healthcare work-ers in the era of COVID-19. In addition to studying how well antibody testing works, researchers are also looking at how measures of antibodies and other biological indicators relate to exposures and health outcomes across our workforce. Participation in the study is voluntary. If you would like to participate, you will be asked to sign a research consent, take a sur-vey and undergo a blood test. Staff members are asked not to participate if they are experiencing symptoms of illness. Infor-mation on the study and how to enroll, can be found on the attached Flyer. Key information for participants: o Participants will be asked to fill out a consent form and a survey prior to their appointment. Email confirmation of the completed consent form must be brought to the appointment. o An appointment is required. Participants will receive an email address to request an appointment time. Participants will be contacted by phone once this email has been sent to set up the appointment date and time. o Test results will be provided via mail once Torrance Memorial Medical Center receives them. Participants will also have the option to enroll in a sub-study investigating genetic variants that may be related to COVID-19 immunity or resistance. Those who opt in to this voluntary sub-study will allow their blood specimens to undergo genetic testing. No results from the genetic analyses will be returned to any participants. If you do not wish to take part in a research study but are interested in antibody testing, please contact your primary care physician to discuss other antibody testing options.

The Coronavirus Risk Association and Longitudinal Evaluation (CORALE) Study BASE Healthcare Workers Sub-Study

HEALTHCARE WORKERS NEEDED FOR RESEARCH We are working to understand how antibody testing could inform our ability to keep healthcare workers safe and healthy in the era of COVID-19. In addition to studying how well antibody testing works, we are also looking at how measures of anti-bodies and other immunity biomarkers are related to exposures and health outcomes across all areas and at all levels of our workforce. ELIGIBILITY CRITERIA To participate, you must be 18 years or older and actively employed as a healthcare worker at a Cedars-Sinai facility that is caring for patients with COVID-19.
