Evidence Based Tutoring Models (cont’)
Description of strategies must assure the reviewer that student needs are being met. Note: Homework help, such as working on subjects assigned by the classroom teacher, is not sufficient to meet the requirements of this program. Choose the subjects (reading/math/science) for which tutoring will be offered.
Discuss how the Five Essential Elements of learning to read are addressed: Phonemic Awareness, Fluency, Vocabulary Building and Comprehension.
Provide citations from research to justify the instructional methodology.

Program Evaluation & Accountability (Rubric 8.1)
1. Explain past effectiveness in increasing student
achievement in reading/math/science. Preferably include
achievement data with valid and reliable assessment results
from a state, district or an independent test developer.
Applicant may include measure of improvement using school
grades or may list percentages of students improving one
year or more as a result of the listed program. Data must be
the result of the individual applicant, not a national
organization. If the applicant is a new tutor, state the
professional development in the model selected. (Maximum
length is 3000 characters)

•Acceptable documents include: annual financial statements,
fiscal audits, Dunn & Bradstreet reports, etc.

Tutors
•Tutor names are entered into this section by PDE as they are approved (original clearances have been received by PDE).
•All tutors previously entered will be deleted before the application is scored.
•If you have tutors that have had continuous employment with your company since they have submitted original clearances to PDE, you can submit a letter stating this, listing their names and signed by the CEO or Superintendent in lieu of new clearances. Proof of this employment must be available upon request.

Tutors
Tutors must be on the state approved list before being in front
of the children. Violation of this can result in termination of
the provider’s approved status.
