Does This Patient Have
Ascites?
Karla Vanessa Rodríguez Jiménez
CASE 1 A 44-year-old man with cirrhosis is admitted with fever but has no obvious source of infection.
CASE 2 A 57-year-old woman presents with an adnexal mass and recent weight gain but otherwise feels well.
CASE 3 A 65-year-old man with a history of myocardial infarction is admitted for decreased exercise tolerance, increased abdominal girth, and ankle edema.

For example, in a patient with a low pretest probability of ascites (<20%), the absence of recent ankle edema decreases the probability of ascites to less than 2.5%.
Ascites is unlikely when patients report no increase in abdominal girth, and ascites is very unlikely in male patients who report no history of recent ankle swelling. ..
PROBABILIDAD PRETEST
EL SIGNO DE LA OLA, LA MATIDEZ CAMBIANTE Y EL EDEMA PERIFERICO DEMOSTRARON SER AQUELLOS
CON UN INDICE DE PROBABILIDAD ELEVADO.
MIENTRAS QUE LA AUSENCIA DE ABULTAMIENTO EN FLANCOS, MATIDEZ EN FLANCOS, MATIDEZ CAMBIANTE O EDEMA PERIFERICO DISMINUYERON DE MANERA IMPORTANTE EL INDICE DE VEROSIMILITUD
CONCLUSIONES:
1. LOS DATOS CLINICOS MAS IMPORTANTES PARA DESCARTAR ASCITIS ES LA AUSENCIA DE EDEMA DE TOBILLO , ASI COMO NO INCREMENTO DE PERIMETRO ABDOMINAL AUNADO A AUSENCIA DE MATIDEZ EN FLANCOS, MATIDEZ CAMBIANTE O FLANCOS ABOMBADOS.
2. LOS DATOS CLINICOS MAS IMPORTANTES PARA REALIZAR EL DIAGNOSTICO DE ASCITIS SON : SIGNO DE LA OLA POSITIVA, MATIDEZ CAMBIANTE O EDEMA PERIFERICO.3. EL SIGNO DE “PUDDLE” ES DFICIL DE REALIZAR ASI COMO INCOMODO Y NO SENSITIVO PARA CANTIDADES PEQUEÑAS DE ASCITIS POR LO QUE
NO SE DEBE DE REALIZAR.
