    A

    drian Simone is a 19year old lance corpo-

    ral who joined theMarines after graduating fromMontville Township HighSchool last year. In mid-July, hisunit, 1st Battalion, 6th Marines,Bravo Company was deployedto Afghanistan. One month in,while on patrol, his unit walkedacross an IED (improvised ex-plosive device). Adrian tookthe brunt end of the explosionand both his legs were severely

    damaged. One of his squadmates violated protocol to stayin position and minimize addi-tional causalities and raced tohis side to stop the bleeding,saving his life. He was taken totwo hospitals in Afghanistanand a hospital in Germany,where the doctors tried to save

    one leg. The damage was too se-vere and both legs had to be am-

    putated at or above the knee.He is now recuperating at a hos-pital in Bethesda, Md. He suf-fered other injuries, but none assevere, and there is no evidenceof spinal cord injury.

    Friends who have visitedAdrian in Bethesda confirmthat his lighthearted personalityand upbeat view of the worldhave not changed.

    f you thought layawayplans had gone the way of

    eight-track players andfloppy disks, think again. Thedeferred payment or install-ment plan, once pop-ular with budget-conscious consumersespecially during theholiday season is mak-ing a big comeback.

    For people whoare unfamiliar withthe concept or were

    born after the 1980s(when installmentplans gave way tocredit card purchasesand other types of financing),layaway is a simple way topurchase merchandise in-terest-free over time. Con-sumers select the items they

    want to buy from a retailer of-fering the plan, make a de-

    posit, which often includes amodest service charge, andpay for the merchandise over

    a specified period. Once theitem has been paid in full, theretailer releases the goods tothe consumer. The only po-tential glitch is if the con-

    sumer fails to make paymentsin full or on time. In that

    event, the retailer has the op-tion of returning the cus-tomers payments (fewer

    service charges) and re-selling the items.

    While theres nodoubt that todays shakyeconomy has contributedto the revival of layawayplans, economists andrepresentatives of severalof the nations leading

    Maxx, Toys R-Us, and Hall-mark insist that the reces-sion is only part of the story.For retailers, layaway plans

    Ronnie is in kinder-garten. Ronnies mo-ther describes him as a

    difficult child. He only wants

    to wear old t-shirts and jeans.

    He argues with his mothereach time she cooks somethingnew for dinner and refuses toeat. He throws tantrums at the

    mall. Shopping for clothes is al-

    most impossible. When play-ing with other children, Ron-nie is bossy. Ronnie ishappiest when he is following

    his routine. He dislikes sur-

    prises. When his mother andfather hug him, he stiffens. Heloves his German shepherd andwill lie on the floor with her for

    hours. Ronnies mother won-ders what she is doing wrong.In school, Ronnie has

    been sent to the principals of-fice several times for hittingother children. He sometimesrefuses to participate in groupactivities. During arts andcrafts, Ronnie usually asks to goto the bathroom.

    During his six year oldcheckup, Ronnies mother de-

    scribed these observations toher pediatrician who recom-mended an occupational ther-apy evaluation. Theevaluation revealed that Ron-nie has a sensory processingdisorder, specifically, a sensorymodulation disorder (SMD).This means that Ronnies nerv-ous system over responds toeveryday sensations.

    A child with a modula-

    tion disorder can appear tohave behavior problems. Re-fusal to participate in activities,rigid adherence to routines andpushing others away are actu-ally strategies used to avoidwhat the childs nervous systemregisters as a threat. Pictureyourself in the kitchen at mid-night. The lights are turned offand you think everyone isasleep. Suddenly someone taps

    your shoulder. In this situation,your nervous system reacts witha fight or flight response. Youjump, your pulse races, you feel

    afraid. For a child with SMD,the fight or flight response istriggered by everyday stimula-tion including the feel of a newshirt or glue on their hands.Being bumped by another childin line may be interpreted asbeing punched. Over reactionto one event is only the begin-ning of the problem for a childwith SMD. Often they are un-able to calm their nervous sys-

    tem and the sensory eventsbuild up over the course of theday. Even if a child has strate-gies to calm themselves, thesemay not appear socially accept-able or may interfere with lifeskills, as when Ronnie spendsarts and crafts time in the bath-room. So what can be done?

    A qualified occupationaltherapist uses a detailed parentand child interview as well as

    standardized assessments to de-termine the nature of a sensoryprocessing disorder. There areseveral types of sensory process-ing disorders and they can varyin degree and presentation.Once the specific nature of thedisorder is uncovered, the fam-ily needs to understand what isgoing on. In almost all cases,the parent and child are re-lieved to hear that there are

    reasons behind the behaviorsand that help is readily avail-able.

    Therapy for SMD consistsof age appropriate, motivatingplay oriented activities thataim to regulate the childsnervous system. Various typesof sensory rich activities are

    used including: movement,deep pressure and resistancewhich stimulate the sensory re-ceptors in the muscles andjoints, tactile, visual and audi-tory input. Foods and scents

    can also be used. As the childresponds positively in therapy,a sensory diet is developed forhome and school. This dietincorporates activities andmodifies challenges to enablethe child to increase their abil-ities to not only tolerate, but toenjoy and learn from sensationsthey previously avoided. ForRonnie, the vacuum cleanerturned out to be one of his fa-

    vorite toys. The vibrationhelps to regulate his nervoussystem! His mother, under-standing his challenges, washesnew clothes several times be-fore Ronnie wears them so thatthey felt softer and smell likehis other clothes. She hasfound that Ronnie prefers rawcarrots and string beans to thecooked ones that she had beentrying to convince him to eat.

    Most importantly, she learnedthat Ronnie enjoys bear hugs aslong as she does not stroke hishair. Ronnie and his teacherare pleased that when he standsat the end of the line to avoidbeing bumped, he no longerpushes or hits his peers. She

    complaining of a hot, painfulear. It is the fourth time thisyear shes contracted an ear in-fection and you are desperatefor answers. More antibiotics,tubes in the ears ... what arethe alternatives?

    Middle ear infection, orotitis media, affects two-thirdsof children by two years of age,making it the most commonmedical diagnosis in children.Approximately a third of allchildren will experience atleast six episodes of this disor-der before entering school.Some children have up totwelve episodes in a singleyear. Several bouts of reoccur-ring infections can result inpermanent damage to the

    inner ear.Typical medical treat-ment of otitis media involvesthe prescription of oral antibi-

    otics, commonly amoxicillin.However, research publishedin the Journal of the Ameri-can Medical Association (Dec/91) reveals not only is amox-icillin ineffective for otitismedia, it actually worsens thecondition. There is also theproblem of repeated use of an-tibiotics and the evolution ofsuperbugs, those growingnumbers of antibiotic-resistantbacteria.

    Eventually, despite failureof the antibiotics, the bodysown immune system takes careof the infection and all is well.Or so it appears, until the nextreoccurrance. After severalepisodes of this, it is recom-mended your child has tubesimplanted in their ears, a pro-cedure called tympanostomytube surgery, the most com-

    mon operation for children.But wait, didnt you readsomething about this ... yes,the April 1994 Journal of the

    American Medical Associa-tion published research thatexamined 6,429 cases of tym-panostomy tube surgery andfound less than half of thesesurgeries were appropriate.

    Clearly, chronic or recur-rent otitis media is a serioushealth problem for which tra-ditional medicine has limitedanswers. Faced with this real-ization, many parents seek al-ternative methods oftreatment, including chiro-practic. Indeed, drugless, non-surgical treatment has greatappeal, and a growing body ofresearch is showing chiroprac-tic care can play an importantrole in helping children withchronic otitis media.

    The term polydipsiarefers to excessivethirst. We see this as

    an increase in the amount ofwater being drank, which inturn usually leads to polyuria,which means your cat is uri-nating more often and alarger volume.

    These changes may bevery subtle and slowly pro-gressive or quite obvious. Youmay observe an increase inthe amount of wet litter in acats litter box or even acci-dents outside the litter box.Some cats may begin drinkingfrom a dripping faucet in thesink or from an open toiletbowl. However, if you want todetermine exactly how muchyour cat is drinking, allowhim only one source of waterand subtract the amount leftin the bowl after 24 hoursfrom the amount you put inoriginally. On average, catstake in about 2 cups perday for a 10 pound cat. Thisincludes any water that istaken in from other sources,such as, from canned food.

    If you determine thatyour pet is drinking exces-sively or just think its morethan usual, make an appoint-ment with your veterinarian

    because polyuria and polydip-sia (drinking and urinatingmore) can be an early sign ofa serious disease. These caninclude: kidney failure, dia-betes mellitus, hyperthy-roidism (overactive thyroid),liver disease, electrolyte ab-normalities, pyometra (uter-ine infection), other lesscommon endocrine disordersand diabetes insipidus.

    Several diagnostic tests

    may be needed to determinethe cause of your catspolyuria and polydipsia be-cause many different diseasescan cause these signs. Any

    visit to your vet should beginwith a complete medical his-tory and thorough physicalexamination. Initial testingmay include: measurement ofwater intake at home, com-plete blood count (CBC),blood chemistry tests (includ-ing electrolytes), urinalysis,urine culture and sensitivity,thyroid hormone (T4) con-centration and abdominal x-rays or ultrasound.

    In older cats, the threemost common disease condi-tions seen in practice thatcause polyuria and polydipsiaare kidney failure, diabetesmellitus and hyperthyroidism.

    Diabetes mellitus diag-nosis is based on high bloodglucose concentration andthe presence of glucose, andoften incompletely brokendown fat products called ke-tones in the urine. This dis-ease is very treatable longterm. Initial treatment de-pends on the severity andmay include hospitalizationwith intensive fluid and elec-trolyte therapy in conjunc-tion with administration ofshort-acting insulin or outpa-tient management with long-acting insulin.

    Kidney insufficiency orfailures diagnosis is based onserum biochemistry tests andurinalysis. Treatment is basedon rehydrating the cat, tryingto slow the progression of thekidney disease by dietarymodification and treatingcomplications such as hyper-

    tension, nauseaand urinary tractinfection.

    H y p e r t h y -roidisms diagno-sis is based onmeasurement ofthyroid hormoneconcentration viabloodwork. Diffi-cult cases may require special-ized blood tests or imaging.Treatment consists of drugtherapy or radiation or lesscommonly surgery. This dis-ease is also very treatable longterm. Treatment with ra-dioactive iodine (I-131)will require referral to a spe-cialty clinic or teaching insti-tution in your area.

    Optimal treatment ofany persistent medical condi-tion depends on establishingthe correct diagnosis. Thesymptom group polyuria andpolydipsia has many differentpotential causes, and it is im-portant to identify the under-lying cause before beginningtreatment. Please visit yourveterinarian as soon as you

    notice any changes becausewith an early diagnosis youoften get a better outcome.

    As one of the mostcommonly injuredstructures in the

    knee, the meniscus is uniquebecause of its blood supply.For both the medial and lat-eral menisci, the blood supplyis from the outside in. Thecloser to the center of theknee, the less of a blood sup-ply there is to the meniscus.From the treatment and heal-ing point of view, the bloodsupply anatomy is critical.The outer one third of themeniscus has a good bloodsupply where healing canoccur, while the inner twothirds has a poor blood supplywhere healing almost neveroccurs. In orthopedic surgerytextbooks, we call the outerone-third the red zone, andthe inner two-thirds the

    white zone.Whereas most fractures

    and many ligament tears willheal on their own, tears of themeniscus in the inner two-thirds have a very difficult

    time healing back because ofthe poor blood supply. Espe-cially in patients who havecomplex tears, or tears thathave multiple little tears in

    the region, patients are oftenfighting a losing battle as theywait for healing to occur. Al-though we typically try acourse of physical therapy inpatients with meniscus tears

    before considering surgery, wealways advise patients thatthey may feel less pain butmeniscus tears often remain.Furthermore, these tears mayget bigger over time, and maycause adjacent cartilage dam-age with its unstable or irreg-ular torn surface.

    The surgical treatmentof meniscus tears is performedarthroscopically, or with min-imally invasive technique.The two most common pro-cedures are repair and partialresection. Meniscus trans-plantations are performed inthe rare patients who meetthe strict criteria. Only pa-tients who have tears in theouter one third of the menis-cus are candidates for ameniscal repair. In thenineties, the orthopedic com-

    munity tried repairs in theinner two thirds with low suc-cess rates. The poor bloodsupply in the inner two thirdsmeant that, despite a strong

    repair construct, the tear didnot heal back. Second opera-tions were often needed, andthere were a lot of grumpy pa-tients.

    There are different

    meniscal repair techniquesout there, but among themost common is the suturetechnique. We arthoscopi-cally introduce a suture withtiny plastic hooks on theirends. The plastic hooks cap-ture the capsule tissue behindthe meniscus and the suture isthen tied down to repair thetear. Especially for tears thatare 1 cm or less, the suture re-pair technique has reliablesuccess rates. For larger tears,such as bucket handle tears,the success rates drop and weare more hesitant to do a re-pair in these patients.

    More commonly, pa-tients often have tears in theinner two thirds of the menis-

    cus, or the white zone. Thesepatients are treated with apartial resection. Using amotorized shaver, we resect

At the time of thereport, all four patients hadbeen free of otitis media forperiods ranging from fivemonths to four years withoutantibiotics or surgery. Theseoutcomes were verified byparental reports, medical ex-aminations, school attendancedata and hearing tests.

    How can chiropractichelp chronic ear infections?Many cases of recurrent otitismedia are thought to be theresult of blockage or restric-tion of the small lymphatic

    chains that drain fluid fromthe middle ear. This blockageforces fluid to build up in themiddle ear, creating an invit-ing environment for bacteriaand viruses.     It can take up to twoweeks or more to train an olderdog to take to the new pottyarea. Puppies may adapt a littlefaster. Collecting urine andfeces and placing it in the dogsarea can help set up a scentmark that may make it easierfor dogs to learn thats where

    theyre supposed to go. For thetraining period, pet ownersshould accompany their dogsto the new area on a leash; donot leave the pet unattendedin the yard. He or she may re-vert back to the normal spotbeing used for relief.

    With time and patience,pet-lovers can cohabitate withtheir pooches and still enjoy anice lawn. It just takes a little

    With so many peoplelooking for alter-natives to costly

    holiday fare, newspapers, mag-azines and online cooking sitesare overflowing with greatideas. But the key to hosting asuccessful seasonal meal is notsimply finding interestingrecipes and purchasing lower-

    cost foods, but in creating andorganizing a menu that worksfrom start to finish. The easiestway to do that, make a planand stick to it.

    Share the cooking andthe cost. Rising food costs andtighter budgets have promptedmany people to host potluckholiday dinners. As the host,you might prepare the maindish and have your guests fill

    in the rest of the menu. Eitherask participants to contributetheir signature salad, side dishor dessert, or plan a meal alonga particular theme and giveguests choices from among alist of menu items.

    Simplify. There is no rule

    stating that holiday dinnershave to be fancy, so dont beafraid to plan a menu that hasfewer and less costly compo-nents. For example, serve a

    hearty and flavorful turkeystew or soup in place of a moreexpensive roasted turkey. Pair

    it with a simple salad andloaves of crusty bread, andthen finish up with home-made apple cobbler. Yourguests will leave happy andwell nourished, and you willhave served up a flavorful andfrugal holiday feast.

    Cook and freeze. Another op-tion is to build your menuaround a few dishes that canbe prepared ahead and frozen.Cooking ahead saves money

    by spreading out food expen-ditures and avoiding last-minute shopping, which often

    leads to spending moremoney.

    Go meatless. Vegetariandishes can be far less expen-sive, especially when feedinga crowd. Consider replacingthe turkey or roast with apasta or risotto dish, roasted

    Go ethnic. While tradi-tion tends to rule at Thanks-giving, Christmas and NewYears menus are wide open.

    How about a Mexican fiestaor an Italian feast? With alarge pan of enchiladas or abig bowl of spaghetti andmeatballs, its easy to feed acrowd and stay on budget.

    Stick to homemade. As arule, anything you make athome will cost less andlikely tastes better thanprepared foods from the gro-cery store or a restaurant. If

    you stick with simple recipesthat dont require an array ofexotic ingredients, yourhomemade holiday meal willhit the spot and you wont hitthe ceiling when you add upthe food bill.
