Bearings as depicted are based upon Connecticut State Plane Coordinates (CTGS) based on North American Datum 1927 (NAD1927). Elevations as depicted are based upon the National Geodetic Vertical Datum 1929 (NGVD1929).2. Existing conditions, as presented, represent those current as of the date of the field survey.3. Allan E. Adams owns the MMC Property located at 281 Main Street in Durham, Connecticut, which is zoned as MR/FR (Main Street-Residential, Farm-Residential). The residence at 275 Main Street (which is considered part of the MMC Superfund site) as well as adjacent properties at 267, 271, and 289 Main Street are zoned as MR (Main Street-Residential).4. The Contractor shall, at his/her own expense, preserve and protect from injury all property either public or private along and adjacent to the proposed work, and the Contractor shall be responsible for and repair at his/her own expense any and all damage and injury thereto, arising out of or in consequence of any act of omission, neglect or misconduct in the execution of the work, or in consequence of the non-execution thereof by the Contractor or his/her employees or Subcontractors in the performance of the work prior to completion and acceptance thereof. The Contractor shall exercise special care during his/her operations to avoid injury to underground structures such as water or gas mains, pipes, conduits, manholes, catch basins, etc.5. These drawings indicate approximate locations of existing subsurface utilities in the vicinity of the work. The accuracy and completeness of the information is not guaranteed. Before commencing any work or operations which may endanger or damage any subsurface structures, the Contractor is responsible for locating and marking all subsurface structures according to the appropriate regulations prior to commencing any subsurface work. The Contractor will be held responsible for any and all claims for damage to underground structures such as, but not restricted to, water or gas mains, pipes, conduits, manholes or catch basins, due to his/her operation or to the operations of any of his/her Subcontractors.

- Support posts shall be 4 inch by 4 inch cross section softwood, 1.33 lb/LF steel, or hardwood with a minimum cross sectional area of 2 inches by 2 inches. Posts shall be driven at least 12 inches into the ground, on the downslope side of the trench and shall be spaced a maximum of 10 feet. Staple or secure the geotextile to the support posts per manufacturer’s instruction such that at least 6 inches of geotextile lies within the trench, the height of the fence does not exceed 30 inches and the geotextile is taut between the posts. - A trench a minimum of 6 inches deep and 6 inches wide shall be dug along the upslope side of the fence. The support wire fence shall be secured to the upslope side of the fence posts with wire ties or staples. Wire shall extend 6 inches into the trench. Attach continuous fabric to upslope side of fence posts, avoid joints. Where necessary, fasten fabric to posts and overlap the next post. The bottom one foot of fabric shall be placed in the trench and backfilled. Filter cloth shall be fastened securely to the woven wire fence with ties spaced every 24 inches at the top, mid-section, and bottom. - The silt fence shall be inspected at least once a week and within 24 hours of the end of a storm with a rainfall amount of 0.5 inch or greater to determine maintenance needs. Repair fence as necessary. Any sediment deposits shall be removed. - Silt fence shall be removed once the upslope area has been permanently stabilized. Retained sediment shall be removed and properly disposed of.3. All sediment and erosion controls shall be protected and maintained by the Contractor until the disturbed surfaces are stabilized.4. The Site shall at all times be graded and maintained such that all stormwater runoff is directed to soil erosion and sediment control facilities. The Site shall also be graded at all times such that stormwater runoff from work areas does not enter the public roadway or adjacent private properties.5. Stockpiles are not to be located within 50’ of a floodway, slope, roadway, or drainage structure. The base of all stockpiles shall be contained by a silt fence or hay bale barrier.6. Storm drainage outlets will be stabilized, as required, before the discharge points become operational.

1. All sediment and erosion controls shall be inspected periodically and shall be inspected immediately following a rainfall event. Any damage shall be repaired and any accumulated sediment removed as soon as practical.2. All required sediment and erosion control measures will be designed and implemented according to Connecticut’s standards and regulations and shall include but not be limited to the following:Construction Entrances - A construction entrance shall be located at all points of vehicular access to the Site, wherever an unpaved area abuts a public road. The entrance shall be a minimum of 50 feet long (except where the tracked sediments contain less than 80% sand, a 100-foot minimum is required) and 12 feet wide, with points of ingress or egress flared sufficiently to accommodate the turning radius of the construction vehicles used. Stone for the entrance shall be angular stone sized according to the standards set by ASTM C-33, size No. 2 or 3, or DOT Standard Specifications sections M.01.01, size #3. If a geotextile filter fabric is used in place of free draining material, it shall be specifically intended for “road stabilization” applications and shall be used in accordance with the manufacturer’s recommendation. A filter fabric fence shall be installed down-gradient from the construction entrance to contain any sediment-laden runoff from the entrance. - Any mud or sediment tracked or washed onto a public road shall be removed immediately. - If Site conditions are such that the majority of mud is not removed from the truck tires by the gravel construction entrance, then the tires shall be washed before the vehicle enters the road or street. Divert wash water away from the entrance to a settling area to remove sediment. Size settling area to hold the volume of water used during any 2-hour period. - The construction entrance shall be maintained in a condition that prevents tracking and washing of sediment onto paved surfaces. The entrances and sediment trapping areas shall be inspected weekly and after heavy rains or heavy use. When mud clogs the voids of the gravel entrance, the entrance shall be top dressed with new stone. If the construction entrance becomes completely clogged, the entrance will be replaced.Silt Fence: - The following table details the geotextile silt fence slope to length limitations:

NOTES:1- Bedrock monitoring and production wells to be preserved: MW-2 and MW-4. Each well is to bemaintained intact during remediation. After completion of remedial and backfill activities, all wellsshall stand 2 feet above finish grade and shall be fitted with a locking cap and keyed alike locks.2- Well MW-4 shall be converted from a water supply well (for 275 Main Street) to a monitoring well.At the request of the Engineer, a new water supply well shall be drilled for 275 Main Street.3- Item #8. Chain-link fencing, gravel base (below propane tank), and tree timber to be stockpiledon site for re-use during or after the remedial action. Chip and stockpile timber and brush for reuseas soil cover during restoration. Retain chain-link fence for reuse or recycle off-site. Stockpile allloose/clean gravel materials for reuse per the Engineer. Relocate stockpiles as necessary, toaccess soil for excavation.4- Item #9, Dry well, septic tank, septic lines, and water supply lines are to be removed in theirentirety and capped at the outside wall of the 275 Main St. foundation. The leaching field, which isbelieved to be in the area of remedial soil removal, will be treated as waste soil except for the pipes.Leach field pipes (if tar paper or PVC) shall be disposed of appropriately. Clay pipes may be mixedwith soils for disposal. At the request of the Engineer, the septic system is to be rebuilt utilizing onelarger sized septic tank, which will be placed in the former septic tank grave or elsewhere as fit fordesign purposes. This shall not occur prior to July 1, 2011.5- Item #10. Bedrock monitoring wells MW-1, MW-3, and MW-5; former MMC Supply Well; andoverburden monitoring well MW-7 shall be abandoned.6- Item #11. Existing chain-linking fencing shall be removed during excavation as necessary toaccess work area and temporary fencing installed as necessary to secure the work area. At therequest of the Engineer, the Contractor shall provide and install a new chain link fence as indicatedon Drawing C-7 prior to demobilization.7- Item #12. Within the fenced area that separates the 275 Main Street residence from mainportion of the site, no activity other than vehicular traffic on the access road and geophysical surveyshall occur prior to July 1, 2011. This portion of the existing fencing shall also remain until July 1,2011. No soil excavation shall occur in the vicinity of water supply well MW-4 and the associatedpiping prior to July 1, 2011.

Contractor shall anticipate the release of fugitive odors upon breaking of the ground surface cover and the continuation throughout conduct of excavation and backfill work. Costs for the provisions, implementation and demobilization of such measures shall be included in the Contractor’s base bid.8. Contractor shall also provide, maintain and operate all appropriate monitoring devices and equipment required to detect any potential exposures to workers and/or the public during conduct of the work.9. Contractor is required to coordinate all work sequencing, including access of vehicles to the property, with the Engineer.10. The listed elevations for excavation are estimated based on available soil analytical and geologic data. Where depth of excavation is specified as top of bedrock, excavation shall continue until top of bedrock is reached vertically, or as directed by the Engineer.11. The Contractor is responsible for excavation to the vertical and horizontal limits shown. No sloughing will beallowed at or outside of 281 and 275 Main St. property lines. Contractor shall repair at his own expenseany damage as a result of sloughing.12. Sequencing. No excavation within fenced area of 275 Main St. or in area of pipeline and leachfields for thisproperty prior to July 1, 2011. Excavation in wetland shall be conducted between August 1 and September 30th.13. Engineered excavation support systems are shown where required at the excavation limits; however, theselocations do not represent all locations where excavation support may be needed. The contractor is responsiblefor determining the means and methods for achieving the vertical limits of excavation within each excavation area.

Notes:

1. The excavation areas are based on available soil data. The lateral and vertical extent of the excavation areas represent areas within the property bounds with contaminant concentrations that exceed the soil cleanup goals presented in Specification Section 02053. 2. Soils to be excavated and disposed of offsite shall be stockpiled and characterized prior to offsite transport. Alternatively, pre-characterization may be performed. Contractor is responsible for compliance with the substantive requirements of all applicable federal, state and local standards, rules and regulations associated with the transport and disposal of these materials.3. Contractor to provide barricades and/or other access controls around all excavations in accordance with the substantive requirements of all applicable federal, state and local standards and regulations. This includes but is not necessarily limited to OSHA standards. Cost for the provision, installation, maintenance, modification (as needed as work progresses) and eventual removal and disposal (as appropriate) shall be included in the Contractor’s base bid.4. Contractor shall provide and implement all necessary measures to control dust, fugitive odors and emissions during conduct of the work. Contractor shall anticipate the release of fugitive odors dust, and emissions upon breaking of the ground surface cover and the continuation throughout conduct of excavation and backfill work. Costs for the provisions, implementation and demobilization of such measures shall be included in the Contractor’s base bid.5. Contractor shall also provide, maintain and operate all appropriate monitoring devices and equipment required to detect any potential exposures to workers and/or the public during conduct of the work.6. Contractor is required to coordinate all work sequencing, including access of vehicles to the property, with the Engineer.7. The listed elevations for excavation are estimated based on available soil analytical and geologic data. Where depth of excavation is specified as top of bedrock, excavation shall continue until top of bedrock is reached vertically or as directed by the Engineer.

In the event that monitoring wells MW-2 and MW-4 are damaged during the excavation, they shall be reinstalled atContractor's expense. No additional cost shall be incurred by EPA or Engineer due to impact to work schedule associated withthe time for reinstallation of these wells.10. At the request of the Engineer, a new water supply well for the 275 Main St. residence shall be installed and connected to the house.Also at the request of the Engineer, a new septic system shall be installed.11.
