Child , Youth & School Services (CYSS) continues to offer a Parent’s

Night Out to families in our military community. Reservations will be

accepted on a first come first serve basis at the Parent Central Services

Office (PSC). To enroll in Parent’s Night Out, children must be registered

with CYSS no later than Wednesday at noon prior to the event.

Reservations may be taken as early as the Monday after the previous event. Parent’s Night Out is offered at

AMR CDC & School Age Center (South) and at Schofield CDC & School

Children should have their dinner prior to arriving. No outside food is allowed. A snack will be served. Children

ages 6 weeks through 5th grade are welcome. CYSS registration is required, and all children must have a current

negative TB Tine (12 months and older) and up-to-date shot records at the time of reservation. Spaces are limited, so

come by the CYSS, PSC Office no later than noon on the Wednesday prior to the Parent’s Night Out. All children

must wear closed toe and back shoes (no sling backs). Please bring your own blankets as it gets cold in the center.

resiliency-building program designed for military

families and children facing the multiple challenges

of combat operational stress during wartime.

FOCUS promotes family strengths and supports

parents to help manage the challenges of military

life. FOCUS was developed at the UCLA Semel

Institute for Neuroscience and Human Behavior

in collaboration with the National Center for Child

Traumatic Stress and Children’s Hospital Boston/

Harvard Medical School.

• Schofield Barracks Tax Center opens 25 January 2012 in bldg 648. Hours of operation are Monday through Friday 0700-1600 by appointment only.

• Fort Shafter Tax Center will open on 26 January 2012 in the Aloha Center. Hours of operation are Tuesdays and Thursdays 0900-1500 by appointment only.

Government-issued IDs that show SSN for EVERY person listed on the return

– If married filing jointly, signatures from BOTH spouses or a power of attorney for taxes (IRS Form 2848)

– If applicable, bring 1099-INT, 1098 mortgage, 1099-MISC, 1099-B, 1099-DIV, 1098-T or 1098-E, childcare statements, and any other documents you may have received marked IMPORTANT TAX DOCUMENT

– Checking account and routing number (voided check)
